Conditions of registration: 1. 2. 3. Two service users under 50 years of age may be admitted. The total number of service users accommodated must not exceed 34. The Commission for Social Care Inspection is notified in writing of the date on which both people are discharged from the home at which time the registration will revert to admissions of people over 50 years of age. 7th February 2006 Date of last inspection Brief Description of the Service: Marjorie Cobby House is registered to provide care and accommodation for up to 34 people over 50 years of age for short stays or rehabilitation. There are no permanent or long stay residents accommodated at the home. The home is located near to the centre of Selsey, West Sussex and the accommodation is on two floors served by a passenger lift. There is a courtyard garden, which is well maintained. All residents have their own room although none have ensuite facilities and there is a range of communal space including a separate room for residents who smoke. Part of the premises is dedicated for the rehabilitation service and suitable kitchen facilities are provided. Residents are referred by health and social services and therefore are subject to a financial assessment to determine the fee payable by them. Rates therefore vary between £77.65 and £414.30 per week depending on the outcome of the financial assessment. Marjorie Cobby House DS0000037445.V323514.R01.S.doc Version 5.2 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. The visit for this key unannounced inspection took place over five and a quarter hours. The registered manager Mrs Lawrence and senior staff were present throughout the inspection and provided the information required. The inspector examined information held on the CSCI service file for the home since the last inspection in February 2006, and read the previous two inspection reports, the Service User Guide and the Statement of Purpose. During the inspection the inspector spoke with many of the residents and two members of staff. The inspector undertook a tour of the premises and looked at four care plans and three staff files. Various record books, policies and procedures were also examined. What the service does well: What has improved since the last inspection? What they could do better:
Fifty per cent of the permanent staff at Marjory Cobby House have an NVQ Level Two qualification or above. However, the home also uses bank and agency staff and these staff have not yet reached this target, but the home is working towards this. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3, 5 and 6. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. An up to date Service User Guide and Statement of Purpose are available to prospective residents. The needs of residents are assessed prior to them using the service. Residents referred for intermediate care are helped to maximise their independence and return home. EVIDENCE: The inspector examined four resident’s files during the visit. All included a Health and/or Social Services assessment that had been obtained prior to the resident being admitted. The assessment forms the basis of individual care plans. Residents at Marjory Cobby House stay for short periods of time for intermediate care, (rehabilitation), respite or a period of assessment and recuperation before moving back to their own homes or into a residential

A guide to the home is left in each bedroom for new residents. Marjory Cobby House has twelve beds dedicated to intermediate care (rehabilitation). Accommodation for intermediate care is situated in a separate area on the first floor of the building. The accommodation includes two domestic kitchens and two lounges. Specialist equipment to assist with rehabilitation and promote independence was evident. Specialist services from relevant professionals, including occupational therapy and physiotherapy, are provided. The period of stay is six weeks and this is made clear to people before they arrive. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents’ health and personal care needs are set out in individual care plans. Health care needs are fully met by the home and community health services. Residents are able to self medicate following an assessment. Residents feel they are treated with respect by the staff at the home. EVIDENCE: The records of four residents were examined during the visit. Each resident has an individual care plan that sets out their health, personal and social care needs. Care plans are under constant review due to the nature of the services provided at Marjory Cobby House. All residents staying at the home for rehabilitation and interim care have regular reviews with the home and social services prior to discharge. Daily records seen during the visit show that community health services are utilised for residents who need them. A Nurse Practitioner holds a surgery at the home once a week. A district nurse visits the home twice a week, or more
Marjorie Cobby House DS0000037445.V323514.R01.S.doc Version 5.2 Page 11 frequently if required, and a chiropodist also comes to the home. All residents are registered with the local GP surgery. Residents are able to administer their own medication following a risk assessment. Lockable storage is provided in residents’ bedrooms for medication to be stored in. The home’s medication policies and procedures were examined during the visit and found to give clear guidance. Only senior care staff who have received training are able to administer medication. Medication records were examined and seen to be accurately recorded. No controlled drugs were being used at the home on the day of the visit. Residents said that personal care is given in the privacy of their own rooms or bathrooms and that their dignity is respected at all times by staff. A telephone is situated in the hall for the use of residents. It is in an enclosed cubicle to maintain privacy. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents are satisfied with the lifestyle they experience at the home. They are able to maintain contact with family, friends and representatives as they wish. They are helped to exercise choice and control over their lives. Wholesome and appealing meals are provided in pleasant surroundings. EVIDENCE: Residents said they are able to spend their time how they wish within the home. Some group activities such as bingo and musical afternoons are provided. Individual activities such as manicures, foot massages and walks are provided. Residents said that they are able to have visitors at any reasonable time. Two visitors spoken with said that they are made to feel welcome and staff are happy to answer questions for them. Residents are able to receive visitors in the privacy of their own rooms or in one of the lounges.
Marjorie Cobby House DS0000037445.V323514.R01.S.doc Version 5.2 Page 13 The ethos of Marjory Cobby staff is to promote independence. This is reflected in care plans and in discussions with staff. Finances are handled by residents or their advocates. The home will hold small amounts of money for residents if they wish. The money is kept in the safe and residents sign for it when they use it. If a resident is unable to sign it is signed and witnessed by two staff members. Residents have their care plans in their rooms so they are able to access their records. Residents are able to bring small objects with them into the home, but as Marjory Cobby House provides short stay accommodation it is impractical for residents to bring larger personal possessions such as furniture. All the residents spoken with told the inspector that they enjoy the food provided for them. They said they are given a choice of meals. On the day of the visit one choice for the lunchtime meal was roast lamb, roast potatoes and fresh vegetables followed by bakewell tart and custard. Every resident has a diet sheet that is completed with him or her when they arrive. This ascertains their likes, dislikes and any special dietary needs. This is then passed to the kitchen staff. On the day of the visit special dietary needs of the residents included pureed food, diet controlled diabetes and fortisips. Pureed meals have each food item pureed separately. The menu is displayed in the dining room. The registered manager said that the fresh fruit, vegetables, meat, bread and milk are bought from local suppliers. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents feel that their complaints will be listened to and acted upon. As far as possible residents are protected from abuse by the home’s policies. Procedures and staff training. EVIDENCE: The Commission has received no complaints in respect of the home. The home has a clear complaints procedure that was seen during the visit. Copies are kept in the hall for anyone to access. Residents spoken with said they would discuss any problems with the staff team or the manager. They felt if they had a problem it would be dealt with appropriately. The home has clear Adult Protection guidelines in place and a copy of the West Sussex Adult Protection Procedures is available in the office. West Sussex County Council provides adult Protection training. Staff spoken with during the visit had a good understanding of what their role is in reporting suspected abuse. The three staff files examined all held appropriate Criminal Record Bureau and Protection of Vulnerable Adult checks in place. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Marjory Cobby House provides a safe, well maintained environment for service users that is clean, pleasant and hygienic. EVIDENCE: Marjory Cobby House is a detached property close to the shops and amenities of Selsey. Accommodation for intermediate care, (rehabilitation), is situated on the first floor and is accessible via a passenger lift. The service provides a very well maintained, safe, comfortable environment, which has all the specialist equipment and adaptations needed to meet individual resident’s needs. As well as a good selection of general aids such as hoists and variable height beds, the home also ensures that equipment is individualised for each service user. A new ceiling tracking hoist has recently

On the day of the visit the home was clean, hygienic and free from offensive odours. Decoration is adequately maintained throughout the home. The grounds are accessible to residents and include shrubs, patio and seating areas. They are well maintained. Laundry facilities are situated away from food preparation and storage areas. The home has recently purchased an electric sluice. Since the last inspection the gas boiler and fire alarm system have been replaced. Beds and carpets have also been replaced in some rooms. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users’ needs are met by the numbers and skill mix of staff. They are in safe hands and protected as far as possible by the home’s recruitment policies and practices. EVIDENCE: The staff rota was examined during the visit. There are between four and six care staff on duty throughout the day. More staff are present during busier periods in the mornings and evenings. The amount of staff on duty is flexible, more staff are available to be on duty if there are many residents with high needs. There are two waking night and one sleeping in staff on duty during the night. In addition to care staff the home employs domestic staff, cooks and a part time handyman. Fifty percent of permanent staff have the NVQ level 2 or above. The home is working towards this target with bank staff. They all contained two written references, Criminal Records Bureau and POVA checks. The home has a thorough recruitment policy. Staff receive induction training from West Sussex County Council and in the home. Staff spoken with during the visit said they have received mandatory health and safety training. Staff records seen show that some staff have also had training in using hoists, advanced personal care, diabetic awareness and other areas relevant to the needs of the residents. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 and 38. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The manager is experienced and competent to run the home. An effective quality assurance system is in place. Safeguards are in place to protect the interests of the residents. As is reasonably practicable the health, safety and welfare of residents and staff are promoted. EVIDENCE: The registered manager is a qualified social worker with many years experience in the care field. She has a professional management qualification and is currently completing the Registered managers Award. Staff and residents spoken with during the visit said that they feel well supported by the manager.

Reports of these visits are sent to the Commission for Social Care Inspection as required by regulation 26 of the Care Homes Regulations. Finances are handled by residents or their advocates. Staff receive training in Health and Safety. Evidence was seen that regular safety checks are carried out by people qualified to do so.
