Service Users to include up to 27 (OP) The service should have a suitably qualified and experienced manager who has been approved by the CSCI Date of last inspection Brief Description of the Service: Mayfield Nursing Home is a 27 bedded home which provides nursing care to older people (i.e. over 65). It is situated in the Eccleston Park Area of Prescott. This is a quiet residential area, which is close to a main road with good connections to the M57 motorway and the near by town of St Helens. Local shops are a short car journey away. The building is a detached Victorian house, which has been converted so that it is accessible for wheelchair users. This is an overview of what the inspector found during the inspection. The unannounced inspection of Mayfield was undertaken on 28th July 2006 and was carried out over a six-hour period. The inspector met with the manager and deputy three staff members and 18 of the 25 residents in placement. Records care files, policies procedures and other documentation was examined and a tour of the premises was carried out. Fieldwork included speaking with 2 resident’s family members and case tracking four residents, which involved reading all documentation relating to the residents daily living and speaking with the residents and staff who were associated with their care What the service does well: What has improved since the last inspection?

The manager has dealt with the requirements from the last inspection and has ensured that residents social needs have been identified, and medication fridges have thermometers. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1.3. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to the service. The home carries out a full assessment of need on each resident prior to admission being agreed and provides clear information about the service provision of the home to enable people to make an informed choice about living in the home. EVIDENCE: Mayfield Nursing Home DS0000005464.V295408.R01.S.doc Version 5.2 Page 9 Mayfield Care Home provides nursing care for older Service Users with a RN (Registered Nurse) on duty 24 hours a day. The Statement of Purpose and Service User Guide are available for current and prospective Service Users and both documents are on display in the main hall. The Service User Guide now includes information regarding terms and conditions and contract details of the Home Three residents and a relative advised they are fully aware of the service and facilities available. Residents files viewed held contracts and all residents revealed they were aware of the fees and other charges in the home. Policies of the home state that prospective residents can visit the Home prior to taking up their placement and meet with other residents and staff. One resident who had recently been provided with a place at the home confirmed that s/he had visited the home twice prior to making the decision to move in. Records show residents were not admitted to Mayfield until the manager or deputy manager had carried out a full assessment of need. Staff advised that this assessment process enables the home to gain valuable information and forms the basis for the plan of care. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7.8.9.10. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Health and personal care needs are addressed and care -planning documentation fully identifies how care practices will be carried out to meet assessed need. EVIDENCE: All care records were looked at and four care plans were examined in detail. The plan of care is detailed and covers all aspects of physical and social care. Arrangements are in place for meeting the health and personal needs of the resident and care practices carried out with their involvement of the resident and /or their representative. Records show the plan of care is reviewed with the residents on a regular basis to reflect changing needs and files hold detail of the agreement and signature of the residents who agree to the changes.

This is carried out under the guidance of the qualified staff. Residents spoken with advised that visiting health professionals and others are able to consult with Service Users in the privacy of their own room. Residents said they were registered with a GP (General Practitioner) of their choice and had access to dentistry chiropody and other services beneficial to their health and well being. Medication records and storage systems viewed identified the medication was well managed. Residents spoken with said they were very pleased with the standard of care received and commented on the polite and caring attitude of staff. Comments included: ”Staff do everything they can to make me feel happy and I want for nothing.” “The staff do their very best to make me comfortable and I could not fault them for their care”. “I would prefer to be at home but that is no reflection on the staff here, they are really wonderful”. Staff spoken with identified they were trained and committed to the provision of good quality care. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12.13.14.15 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Activities and interest are arranged but the activities programme should now be developed to suit the needs of the current residents. Food provision is varied well presented and of a high standard. EVIDENCE: Mayfield Nursing Home DS0000005464.V295408.R01.S.doc Version 5.2 Page 13 Residents said that they are able to make individual choices about various aspects of their care e.g. getting up in the morning, retiring at night, going out with relatives and which activities they would like to participate in. Staff said visitors are welcome and people were seen popping in at various times during the day to see their relatives and friends. Records show the Home maintains links with community-based clubs and members of the clergy visit. Meals are served in the dining area of the lounge or in resident’s own rooms if preferred. The menu showed the food on offer is well balanced and appetising with a choice provided at all times. Residents spoken with advised how much they enjoy the food and that there is a good choice and observations of residents eating lunch revealed they were given plenty of time to enjoy their food and chat with each-other. Mayfield continues to provide an activities programme and residents social interests and are noted in the care files. However, because of the variation in the capabilities and interest of the residents it was noted that some residents did not feel that the activities in place fulfilled their need. This was discussed with the manager and staff and it was agreed that the activities programme would be reviewed to meet changing need. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16.18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staff are trained and knowledgeable in all aspects of adult protection and the complaints system in the home is know to residents and their families, is accessible and residents are confident that any complaints will be listened to and quickly acted upon. EVIDENCE: The home has a complaints policy. This was displayed in a prominent area of the home. Residents said they knew how to complain and although none of the residents spoken with had made a complaint they said they felt sure that staff would listen and act immediately to rectify the situation. The complaints book was viewed. No complaints have been made against the service, to the service or CSCI since the last inspection. Viewing this book showed that any complaints made are recorded, dated, and along with actions taken and outcomes. Staff spoken with identified they have received training in the protection of vulnerable adults and had gained full knowledge and understanding of the policies and procures involved to protect people form all forms of abuse. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19.26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is clean pleasant and hygienic and has a clear maintenance programme to ensure that residents live in a safe environment. EVIDENCE: A tour of the premises revealed the home was clean and hygienic and free from any unpleasant smells. Documentation provided by the manager revealed that she has overall responsibility for the housekeepers, laundry and kitchen staff and she undertakes a monthly quality assurance audit to ensure the home is fit for purpose. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27.28.29.30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staffing levels are adequate with a skill mix of nursing and care staff. Residents are supported by the homes recruitment and selection policy EVIDENCE: Staff personnel files and training records were viewed. Discussions were held with the manager and the deputy manager. The manager and deputy manager advised that the homes recruitment and selection policy was clear and enabled all applicants to be treated in a fair and equal way. In discussion a recently appointed staff member advised that s/he had been asked to complete an application form and provide 2 references and Criminal Records bureaux (CRB) check. S/he advised also that the interview process had been structured and on appointment s/he had been provided with a contract and job description. S/he advised that a thorough induction had been provided and a mentor had been allocated to enable any questions or concerns to be dealt with quickly.
Mayfield Nursing Home DS0000005464.V295408.R01.S.doc Version 5.2 Page 19 Each staff personal file viewed contained the information required to meet the care home regulations 2000.The files were organised to a high standard so information could be found quickly. The manager stated that all new staff are allocated to a mentor. A qualified nurse oversees this process. Records exist to prove this statement. New Care staff said they attend an induction day, which covers the topics that are advised by T.O.P.P.S and also have a mentor to which they can gain support if required. The deputy manager confirmed that she acts as training officer for the service. Records showed that training topics are offered on an almost monthly basis and that a great variety is available. The training records showed that the training offered is designed to assist staff to meet the needs of older people. Staff advised that other training is also offered to enable qualified staff to maintain their professional status. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31.33.35.38. Quality in this area is good. This judgement has been made using available evidence including a visit to this service. The manager exhibited good management skills and identified she works in the best interests of the residents. However it is important that she makes application to become the registered home manager as a matter of urgency. EVIDENCE: The registered manger of Mayfield had recently left the home and Jayne Shillcock had been appointed in her place. Although this person has not yet attended the offices of CSCI for her registration interview, she displayed full knowledge and understanding of the care home management process and

However it is a requirement that the service should have a suitably qualified and experienced manager who has been approved by the CSCI and advice was given to ensure that the manager submits a registered managers application to CSCI as a matter of urgency. Ms Shillcock stated that she has many years experience in caring for the older Service User and it was noted that she is fully supported by a Deputy Manager and full compliment of qualified staff. Residents advised that the manager and staff are very approachable and provide every comfort possible in the Home. Residents said they have residents meetings and can discuss any issues they may have or make any suggestions for changes to the home. Staff said they too have meetings in which they can be open and discuss any areas of concern or any areas that can be improved upon. Staff said they feel supported by the management team and “that the home is a very nice place to work in”. Ms Shillcock advised that she has completed numerous courses in Management and is a registered RGN. Staff advised that they receive mandatory training, including Manual Handling, Health and Safety, Infection Control, First Aid and Basic Food Hygiene. Records show other courses are also arranged to meet the nursing, social and safety needs of the Service Users. Documentation provided shows that satisfactory maintenance and essential service checks are in place to ensure that the premises are fit for purpose.
