Conditions of registration: 1. Service users to include up to 13 (OP) Old age Date of last inspection 20th September 2004 Brief Description of the Service: Metfield Grange is a small, friendly, family run residential home, which provides personal care and support for 13 elderly people. Both Mr and Mrs Wallace are registered as Managers/Owners and live on the premises. Both are involved in managing the day to day running of the home, working hands on with both the residents and staff, providing supervision, support and direction. Nursing care is provided by the nursing care service, when required. The home is located within a quiet residential area, within easy reach of the town centre of Southport. A local bus route provides access to all the amenities of the coastal resort. These include a large shopping area, cinema, theatre, parks and lake. The accommodation comprises of 3 double rooms and 7 single rooms. There is a stair lift, which provides access to the first floor. There is a communal lounge and a dining room located on the ground floor. All areas are provided with emergency call points. Access is provided to front and rear landscaped gardens. This is an overview of what the inspector found during the inspection. The unannounced inspection of Metfield Grange Care Home took place on 21st September 2005, over 5.5 hours and involved examination of records, discussion with staff, relatives, visitors and residents, a tour of the premises and general observational practice. 30 standards were assessed at this inspection. Case tacking methods were used, which focused on a small group of residents whom were randomly chosen for this purpose. All records relating to these people were inspected along with the rooms they occupied in the home. Residents, visitors and staff were invited to discuss their experiences of the home. Survey forms were left for visitors to the home and residents to complete with their comments. All comments received during the inspection were positive and observational practices showed that staff appeared to have full knowledge and understanding of the needs, choices and capacities of all the residents in their care. What the service does well:
Metfield Grange has a very pleasant relaxed, homely atmosphere. The managers live on the premises with their family and one resident commented, “It is lovely to hear the children playing in the garden”. The managers both confirmed they ‘aim to create a happy home ‘ for their residents to live. Residents and a relative interviewed confirmed that it is managed efficiently and staff provide a good standard of care. One relative commented, “It is very homely, staff are very caring and I am always made welcome”. The routine of the home is based very much around residents’ wishes and relatives are encouraged to become involved and visit when they like. One resident is to celebrate her 100th birthday and the home is putting on a special party and has invited family, friends and the mayor. The resident said, “I am looking forward to it very much”.

Residents’ needs are assessed to ensure the home can care for them. Following admission a plan of care is drawn up and this records key areas including social history, general welfare and social care. The documentation is organised, easy to read and subject to regular review to ensure records are up to date. Care staff are prompt to report any problems as they arise and care files evidenced hospital appointment and medical referrals at the appropriate time. Staff were observed as being very polite and offering good levels of assistance to residents with their personal care and with meals. Residents interviewed commented, “All the staff are very nice”, “I enjoy living here and can’t find anything wrong with it”. “They will get me anything. I like Tesco oat cakes and they bring them in for me”. One relative said, “The girls are all very good and look after the residents. The home is very well maintained and colour schemes attractive. The manager ensures all areas are decorated to provide a pleasant, homely environment. All areas were observed to be clean. Mr Wallace, manager, has recently completed NVQ Level 4 and is awaiting his certificate. The home provides a range of activities, which include – bingo, BBQ’s, quizzes, gentle exercise sessions and entertainers. What has improved since the last inspection?
An ongoing maintenance programme is in place, which ensures that the repairs and redecoration are completed. Since the last inspection decoration has taken place in a number of bedrooms, radiator covers fitted throughout and window restrictors in place.

Medication is now provided in ‘blister pack’ and records are made at the time of the administration. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,6. Pre admission assessments carried out by the manager to ensure that the home can meet the needs of the residents. EVIDENCE: All residents have an individual care file. Assessment documentation for 3 residents was seen and this had been completed in detail. Observation, feedback from residents, management, staff and viewing of 3 care plans demonstrated that the home provides the care and support required to meet individual need. Positive comments were obtained from a number of residents and a relative regarding the care and support provided. Residents commented, “The staff are all very nice”, “I am very happy here” (Residents). “It is very homely, staff are very caring”(Relative). Care plans are reviewed monthly and action to reflect changing need is recorded. Access to specialist services is available and records showed that regular visits are maintained by health care services when required. On the day of the inspection a resident was being reviewed by her social worker from the placing authority. Relatives, staff and the resident attended to review her care needs.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10. Residents health, personal and social care needs are addressed in care plans and care needs are met effectively. This ensures a good overall standard of care in the home. Medicines are administered according to the home’s policy and procedure. Staff were observed providing care and assistance to residents in a polite and sensitive manner. EVIDENCE: 3 care plans viewed where found to contain up to date information on the residents needs. The manager ensures that all care plans are reviewed monthly to monitor changing needs and the action required to meet those needs. Observation throughout the inspection demonstrated the caring approach and relaxed atmosphere present. Residents interviewed confirmed that they are happy with the care provided. “Staff help me to bathe in a very caring way”. Positive comments were received on the pleasant and approachable attitude of the staff. One resident said, “They are all very kind”. A relative who visits weekly said, “They are very caring”. All residents are registered with a local GP. All visits from health care services are recorded within the daybook and monitored within the care planning system. These are

A medication policy and procedure is in place and available to staff for reference. All medication is securely stored and signed for on administration. It has been recommended that all care staff be provided with medication training to update them on procedures. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15. There is a sociable, relaxed and pleasant atmosphere in the home and residents interviewed were happy with the daily routine. Social activities are provided and visitors are made welcome. The home offers a well-balanced choice of nutritious meals. EVIDENCE: The home provides a flexible approach to the daily living and activities. A range of activities include – gentle exercise, card making, karaoke and BBQ’s. One resident expressed that she likes to sit and chat to other residents but also watch TV in her room. “I don’t like too much going on. There is just enough here”. Another resident said “I can please myself when I go to bed”. A number of residents access the local services, which include day centres. One resident goes on holidays with the Parkinson Society and attends the centre weekly. One resident is to have her 100th birthday celebration and the home is organising a party for her family, friends and the mayor is to attend. The resident confirmed that she is “really looking forward to it”. A pleasant, relaxed and comfortable atmosphere was present during the inspection. Residents chatted freely with each other, the care staff on duty and the managers. Relatives are made welcome and call in freely. One visitor expressed “I call

A resident confirmed that her family could visit anytime of the day to see her. Residents have the choice to eat in their own rooms or in the dining area. A varied and appetising menu is in place. The food is ‘home cooked’ and the residents were observed to have a relaxing lunch in the dining room. The home is well stocked with both fresh and frozen food. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 The home has a complaint procedure and residents were confident that their concerns would be listened to and acted upon. EVIDENCE: A complaints policy and procedure is in place. No complaints have been made since the last inspection. Residents and a relative commented that they would have no problem discussing any concerns with the managers and are confident any complaints made will be investigated. One Service User informed the Inspector “I have no complaints. The managers both live on the premises and are available to speak daily to the residents and discuss day-to-day issues. Visitors’ are made welcome and call in daily to see the residents and one visiting relative said, “If there were any problems I would tell Linda and she would sort it out”. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,24,25,26. The home offers very comfortable ‘homely’ accommodation and all areas are well maintained. This contributes to a good quality of life for the residents. EVIDENCE: A tour of the premises demonstrated the home continues to provide a high quality, well-maintained, comfortable, homely and clean environment for the residents. The grounds are kept tidy, safe and attractive. The front and rear gardens are accessible, with the exception of 2 steps at the front entrance. The front entrance provides a welcoming approach with its flowers and pot plants. Since the last inspection improvements have included – fire escape tiled at the top-level area, decoration of a number of rooms, radiator covers fitted and window restrictors are now in place. A large, comfortably furnished, smoke free lounge area is used daily by the residents. A brightly decorated dining room provides a pleasant room for the residents to eat. The residents were observed to be relaxed and comfortable within their environment.

Positive comments were received from the residents and a visiting relative. “Very homely”, (Relative). “I love my room and have everything I need”.” (Resident). The home provides one assisted bath and a shower. 3 toilets are provided in close to resident’s rooms. Discussion with the managers confirmed that the carpet in the ground floor toilet is to be replaced by more suitable flooring. No en-suite facilities are available. A sluice is available in the basement area. A stair lift provides access to the first floor and a call system is available in all rooms. Discussion with the manager confirmed that an assessment of the premises by a suitably qualified person is yet to take place. This has been incorporated within the recommendations of this report. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,28,29,30. The staff employed are competent, caring and trained to do their job. The home’s recruitment and selection procedures and records need to be more robust and the necessary information obtained. EVIDENCE: Residents spoken to were pleased with staffing arrangements and felt there are enough staff about to help them. Residents’ comments included, “They are all very kind”, “Nothing is too much trouble.” One newly appointed staff file was viewed. This must contain the necessary information to demonstrate the correct recruitment and selection procedure is followed, i.e. 2 written references, CRB (criminal record bureau check) and the interview documentation. Staff are encouraged to enrol on NVQ courses and a senior carer has recently completed NVQ Level 4. Statutory training, such as manual handling and first aid is provided by Care West training and is to be brought up to date for all staff employed. The manager and a senior carer are to receive a training update on safe handling of medication. Staff interviewed commented, “We are all a family here”, “I love it here it is so homely”. “The managers are great and always there if I need advice”. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,34,35,37,38. The home is well run and well managed in the best interest of the residents. EVIDENCE: Mr Wallace has recently qualified in NVQ level 4. Both Mr and Mrs Wallace are the registered managers and are in day-to-day contact with the staff, residents and visitors as they live on the premises. A pleasant, homely atmosphere is pleasant and the residents, staff and visitors commented on this. “We are all part of a family”. (Staff), “It is very homely”, (Relative). “They can’t do enough for us”, (Resident). As it is a small home and the owners live on the premises they are available to discuss anything with the relatives and residents. Both residents and a relative interviewed said they would discuss any issues with the managers should they have any.

Records are made of all accidents and injuries. Services in the home i.e. gas and electric are regular checked and serviced. Records viewed confirmed this. The manager is appointee for one resident. Discussion with the social worker, who was attending a review at the time of the inspection, confirmed her satisfaction with this and is to monitor the finances regularly during her visits. Policies and procedures are in place and the manager has been recommended to review these on annually to reflect changes in legislation. Statutory training for all staff must be kept up to date, i.e. manual handling and food hygiene.
