The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home has a strong staff team that is committed to improvement and quality for service users. The environment is clean and homely. The outside area has been well maintained and provides a relaxing area in which to sit out weather permitting.

What has improved since the last inspection?

The home has a new Manager who will be applying to become the Registered Manager. Staff training is ongoing. Staff supervision and appraisals are being undertaken. Re-decoration of the premises is ongoing.

What the care home could do better:

Amendments need to be made to the Statement of Purpose and Service User Guide. Full comprehensive pre-assessment information needs to be completed. Action needs to be taken to ensure that the care planning system provides all the information as required by regulation. Issues needs to be addressed in relation to promoting service users dignity and privacy. Comprehensive daily food records need to be maintained. Review Infection Control procedures and practices. Complete Induction documentation as appropriate.

To admit one (1) Service User with physical disabilities, whose date of birth is 26/01/1950. 17th August 2006 Date of last inspection Brief Description of the Service: Montagu Court is a large privately owned property providing residential care and support for up to thirty service users who require varying degrees of assistance. The home is a family run business, with the proprietors having a great deal of input into the day-to-day running of the business. Staffing within the home comprises of the registered owners, manager, deputy manager, senior care staff, care and ancillary workers. A home manager is employed who is responsible for the general maintenance of the building. The home is located within a short walk of Cliftonville shopping centre and promenade. The Manager confirmed at the inspection visit that the fees range from £295 to £365 per week. The key inspection visit was unannounced and carried out on Tuesday 24 July 2007 between 11.00 and 17.30. The Inspector spoke with the Registered Person, the Manager who has been at the home for approximately three months, the Deputy Manager, several of the staff on duty and several service users. Various records were viewed and an accompanied tour of some areas of the premises was undertaken. The AQAA documentation that homes are required to complete was only recently sent to the home, so information that would normally be used to inform the inspection process was not available to be included in this inspection report. Due to the short notice given to plan the visit, although Service User Surveys and Relative Surveys have been sent out they were not returned in time to be included in this report. A new Manager took up post at the home three months ago, and will shortly be applying to become the Registered Manager of the home. The Registered Provider, the Manager, the management team, and staff are working hard and committed to meeting the requirements of regulation. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 1, 2,3, and 6 were inspected at this visit. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The home’s Statement of Purpose and Service User Guide do not provide service users and prospective service users with all the information they need to make a decision about moving into the home. The needs of service users are not always fully assessed prior to admission and Service Users may not feel confident that their needs will be met. It is not the general policy of the home to admit service users on a short-term basis, however a designated bedroom is currently being used for respite care. Montagu Court Rest Home DS0000047079.V345905.R01.S.doc Version 5.2 Page 9 EVIDENCE: The Statement of Purpose and Service User Guide were seen for the home and both documents need reviewing to reflect recent changes and to include all information required by regulation. Contracts are provided by the home, and following discussion it was agreed that the fees payable would be incorporated into the contract together with the addition of the bedroom number to be occupied. People have their needs assessed prior to becoming a service user at the home. Completed pre-assessment documentation was seen and discussed with the Manager who agreed to review the documentation in order to provide comprehensive information that can then be used appropriately to inform the setting up of the initial service user plan if the person is admitted to the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 7,8,9 and 10 were inspected at this visit. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The care planning system meets the requirements of the national minimum standards and regulations. However, it was found that some information in relation to health matters was not recorded and that this could be detrimental to the well being of service users. The care of service users may be compromised because regular reviewing of the service user plans was not being appropriately maintained and in two plans seen had not been undertaken. Service users are protected by the home’s policies and procedures for dealing with medication. Service users may not always be treated with privacy and dignity. Montagu Court Rest Home DS0000047079.V345905.R01.S.doc Version 5.2 Page 11 EVIDENCE: Three service user plans were seen, and discussion took place with the Manager in relation to the lack of information contained in the monthly reviews, and that not all appropriate risk assessments had been recorded. It was also evidenced that reviews had not been recorded for two recent admissions although it was indicated that care needs had changed. The Manager agreed to address these issues. It was also agreed that the service user plans would in future contain all relevant information in relation to the person, and the recording of information about the individual seen kept in other folders would be stopped. It was also found that a number of the records seen were not signed and dated, and the Manager agreed to take immediate action in relation to this issue. In relation to the records relating to health care needs, there was insufficient information for one person who receives catheter care, and no information about another person who is seen regularly by the District Nurse to have dressings changed. There was also discussion about the recording of regular weights especially for one person where it was indicated that the nutritional intake needed monitoring, the nutritional assessment form had not been completed and there was only one weight recorded and this was some weeks after the person was admitted to the home. The Manager agreed to address these issues. The medication records were seen, and on the whole were appropriately signed and up to date. The medication storage was seen to be in order to meet the needs of the current group of service users. The Manager stated that staff able to administer medication had undertaken appropriate medication course linked with the NHS Trust. The Deputy Manager carries out a regular audit of the medication. Although people that live in the home expressed that they were treated with privacy and dignity, two issues were discussed in relation to privacy and dignity, the first was in relation to the call bell system located in the dining room as this has a facility that allows for two way conversation. It was discussed that a person’s privacy and dignity may be compromised by conversation being overheard by anyone in the Dining Room at the time. The maintenance person said that he would be able to de-activate this part of the system, and it was agreed that immediate action would be taken to address this issue. The second issue was in relation to the two en-suite bathrooms that may be dually used as communal bathrooms. There were no locks seen on the interconnecting door or the door out onto the corridor and therefore may Montagu Court Rest Home DS0000047079.V345905.R01.S.doc Version 5.2 Page 12 compromise the privacy and dignity of service users. The Registered Person and Manager agreed to address this issue. People that live in the home were clean and well dressed. Service users that spoke with the Inspector indicated that they were contented and their needs were being met. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12,13,14 and 15 were inspected at this visit. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users’ social, cultural, and religious needs are being met. Service users maintain contact with whom they wish. Service users are helped to exercise choice and control over their lives. Service users benefit from being given meals in the home that are mainly good, offering both choice and variety, and catering for special diets, although appropriate records of the food provided need to be maintained in order to provide a sound judgement under this standard. Montagu Court Rest Home DS0000047079.V345905.R01.S.doc Version 5.2 Page 14 EVIDENCE: Service users in conversation indicated that they were satisfied that their social, cultural, religious and recreational needs were being met. Some described continuing with hobbies and interests they had before they entered the home. The activities programme was seen, and on the day of the inspection visit the Registered Person was taking service users out in the minibus. The homes visitors book indicated that visitors come and go at a selection of times. Visitors were seen in the home at the time of inspection. Service users have recorded meetings so that they can have input into the home. Service users can bring items of their own into the home to personalise their own rooms. People that live in the home indicated that the meals were wholesome and filling. Mealtimes are social occasions with most people choosing to eat in the dinning room. The daily record maintained of the food provided did not meet the requirements of regulation and the Manager said that a comprehensive record including all relevant information would now be maintained. This judgement has been made using available evidence including a visit to this service. Service users can be confident that their complaints and concerns will be properly addressed by the home. The Manager stated that there have been no complaints made since she took up the post of manager. She confirmed that any complaints would be investigated and action taken to address the issues raised. The Manager agreed to update the information in relation to the Commission seen on the complaints notice, and to include appropriate information in the Statement of Purpose and Service User Guide. A selection of compliments cards were seen stating for example in ‘thank you for looking after our relative’, another relative commented ‘that their relative was always very happy whilst at the home’ and a further example thanked the Registered Providers and all the staff at the home for all the care, help and kindness. Staff have had training in relation to adult protection. Staff spoken with showed an understanding of abuse. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 19,21,22,23,24,25,26,27 and 28 were inspected at this visit. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The home is a safe and reasonably maintained environment. Whilst the home was mainly clean and pleasant at the time of the inspection visit the health and safety of service users may be compromised because of poor practice with regard to good infection control practices. Montagu Court Rest Home DS0000047079.V345905.R01.S.doc Version 5.2 Page 18 EVIDENCE: The home continues to undergo a programme of continual improvement. The external areas of the home are attractively planned and provide a pleasing area for service users to sit out weather permitting. As previously reported the home has provided thermostatic valves on all hot taps, preventing the risk from burns. All doors have been fitted with automatic closures, ensuring safety in the case of fire, whilst also enabling service users to have their bedroom doors open if they choose. The programme to cover radiators has been completed, preventing the risk from burns. The bathroom dually being used as the hairdressing room was discussed as it was seen that electric points were being used in this area. The Manager agreed to review the environmental risk assessment for this bathroom. As part of the business plan for the home an appropriate area needs to be designated for hairdressing purposes and suitable equipment provided. New boilers were seen in operation within the kitchen area, it was very warm in this area at the time of the accompanied tour and the Manager confirmed that an upgrade of this area is included in the business plan for the home. The Manager was advised to look at the environmental risk assessments for the areas that include the staff room that links through to the smoking room and then links through to the laundry area. Some storage items seen need to be removed and the Manager agreed to address this issue. Other issues that may need to be addressed include possible risk of fire and procedures used to maintain good infection control. The Manager should ensure that liquid soap and paper towels are available in all appropriate areas. The home was generally clean and free from offensive odours. The Manager needs to review the policies and procedures in relation to infection control and action is being taken to ensure that the home uses the correct category clinical waste bags for the disposal of clinical waste, and that this clinical waste is appropriate stored. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 27,28,29 and 30 were inspected at this visit. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has appropriate staff numbers for the needs of the current service users. Service users are in safe hands. Service users are protected by the home’s thorough recruitment procedures. Staff are trained and competent to do their jobs. Montagu Court Rest Home DS0000047079.V345905.R01.S.doc Version 5.2 Page 20 EVIDENCE: The staff rota was viewed, and from discussion it was indicated that there are sufficient staff on duty to meet the needs of the current group of service users. Observation showed that staff worked in an unhurried manner with the people that live at the home. Staff files were viewed and held all the information that they should. The home seeks references and criminal records checks prior to staff commencing employment. Less then 50 of staff are trained to NVQ Level 2. The standards express that this should have been completed by 2005. However, this is because some trained staff have left, and the Manager confirmed that currently one member of staff is undertaking NVQ Level 3 and that three staff members are being put forward to start NVQ Level 2. All new members of staff undertake an induction, which is line with skills for care. It examines carer’s levels of understanding and competency. However it was found that for a new staff member the induction documentation was not sufficiently completed for the length of time they have been working at the home, and the Manager agreed to address this issue. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 31,32,33,34,35,36,37 and 38 were inspected at this visit. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has a competent management team and the home is run in the best interests of the service users. Service users financial interests are safeguarded. The health, safety and welfare of service users and staff are mainly promoted and protected. Montagu Court Rest Home DS0000047079.V345905.R01.S.doc Version 5.2 Page 22 EVIDENCE: The Manager is qualified, competent and has many years experience in a management capacity. The home has an open culture and input is welcomed from staff, service users, health care professionals and visitors. The home now has investors in people award. The processes for ensuring secure facilities are provided for the safekeeping of money and valuables on behalf of the service user were discussed with the Registered Provided and indicated that sound procedures were in place. Although at the time of the inspection visit the Manager was unable to provide an up to date staff training matrix, a telephone call a couple of days later provided information in relation to staff training that indicated that staff have undertaken mandatory training sessions, and that further sessions are planned. The manager ensures as far as reasonably practicable the health and safety of staff service users and visitors and ensures safe working practices, however some risk assessments need updating. The home has realises the importance of good quality assurance practices, and sound systems are being put in place to ensure the safe guarding of people that live in the home. There is continual self-monitoring by the Providers who are at the home on a regular, almost daily basis.
