To admit one (1) Service User with physical disabilities, whose date of birth is 26/01/1950. 12th July 2005 Date of last inspection Brief Description of the Service: Montagu court is a large privately owned property providing residential care and support for up to thirty service users who require varying degrees of assistance. The home is a family run business, with the proprietors having a great deal of input into the day-to-day running of the business. Staffing within the home comprises of the registered owners, registered manager, deputy manager, senior care staff, care and ancillary workers. A home manager is employed who is responsible for the general maintenance of the building. The home is located within a short walk of Cliftonville shopping centre and promenade. This is an overview of what the inspector found during the inspection. This was the homes second annual inspection which was undertaken unannounced. A tour of the ground floor was undertaken, service users and staff spoken with and records viewed. What the service does well: What has improved since the last inspection? Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 and 9 The care planning system provides staff with the information they need to ensure all of the service users needs are fully met. The systems for medication administration, storage and recording are poor, potentially placing service users at risk. EVIDENCE: Individual care plans viewed identified service user involvement in the writing of and consent to the contents of the plan. Clear guidelines for staff to follow are held within each care plan, to ensure that all needs are fully met. The Registered manager and senior care confirmed that all members of staff had recently undertaken training in the safe handling of medication. A number of concerns were raised with regards to the management of insulin. The storage facilities for insulin were reviewed and found to be unsuitable. The registered owner confirmed that a new lockable medication fridge had been ordered and was due to arrive shortly. It was identified and confirmed by members of staff that insulin was being administered by staff who had not undertaken training.

Medication was found unlabelled, with no instructions as to the dosage or service user it had been prescribed for. Medication administration records were examined and discrepancies between the records and the drugs stored were found. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 The home’s complaints policy ensures service users views are listened to and acted on. Arrangements for the protection of service users are satisfactory protecting them from possible risks of abuse. EVIDENCE: A copy of the homes complaints policy and procedure was viewed and found to contain detailed information in a format suitable for the service user group. Copies of this policy are displayed within the entrance hall and within the statement of purpose and service user guide. The registered manager confirmed that there had been no complaints made to the home since the last inspection. The registered owner spoke of four members of staff having undertaken training in adult protection since the announced inspection, with a further four booked to attend in the coming weeks. Staff spoken to were aware of the correct procedure to follow when reporting possible incidents of abuse. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Recent improvement has significantly improved the appearance of the home creating a comfortable and safe environment for those living and visiting the home. Infection control measures are in place, which promotes the well-being and health of service users and staff. EVIDENCE: A tour of the ground floor of the home was undertaken, a good standard of decoration and furnishing was found throughout. The smoking lounge has recently been redecorated and now provides an attractive and welcoming environment for those residents who choose to use this room. Staff files viewed at the last inspection confirmed that all staff have attended infection control training. Liquid hand soap was present in areas where hand washing takes place.
