The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Recent remodelling of the home has created a bright and pleasant environment for service users. The home is very spacious and there is ample room for service users to be private. The care staff were observed to be assisting service users discreetly and respectfully. There has been a good level of training for staff in the care of people with dementia and there is an understanding about promoting a sense of wellbeing in service users. This should ensure that service users` needs are met by a confident and competent staff group.

What has improved since the last inspection?

The premises have been improved considerably and service users now have numerous seating areas and places to walk round. The manager has looked closely at the nutritional needs of older people with dementia and choice has been introduced at all meal times. This will make an important contribution to the health of service users and will assist them in remaining as independent as possible. The employment of an activities leader will make sure that service users have access to stimulating and therapeutic events.

Care for service users in category DE(E) will be provided in the flat designated specifically for this purpose only. Date of last inspection 22/03/05 Brief Description of the Service: Morrell House is a care home run by City of York Council and registered to provide a service for 21 older people of either gender aged over 65 years who do not have any specialist requirements and 8 people with dementia care needs. At the time of inspection, there is an application pending final approval to use all the places for people with dementia and to cease offering a servcie to older people who do not have specialist needs. The home has undergoing extensive refurbishment and building work in preparation for this change. Morrell House was purpose-built approximately 30 years ago and is located on the outskirts of a residential area on Burton Stone Lane. The accommodation is provided in single rooms on two floors. The upper floor is accessible via passenger lift. This inspection was carried out on 29 July 2005. It was unannounced and took 6 hours to complete. The majority of time was spent within with the service users in the home at the time of the inspection and with the staff on duty. There were opportunities to have discussions and observe staff interaction with the service users. A sample of service user files was examined and a check was made as to whether the home had complied with recommendations from the last inspection. Feedback was given at the end of the inspection to the registered manager. What the service does well: What has improved since the last inspection? What they could do better: As the home is currently half full, the Registered Manager is concentrating on making sure that new admissions are made smoothly and sensitively. This should help lessen the impact of a major change in the life of prospective individual service users. To determine whether staff have been properly checked and are suitable to work in Morrell House with older people, staff records need to be kept by the manager in the home. To further protect service users from abuse, all new staff should only start work in the home once confirmation of Criminal Records Bureau and POVA first (Protection of Vulnerable Adults)checks has been obtained. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 & 10 Service users health and social care needs are met. EVIDENCE: The care needs of each service user are set out in individual service user plans. A sample of service user plans was inspected and they were complete, providing background information and a clear description of the day-to-day task that service users can accomplish and those where assistance is required. The manager has been organising professional support for the home in anticipation of it providing care exclusively for people with dementia. A local GP with an interest in the mental health of older people has offered to hold a weekly surgery in the home. There is a CPN who will provide ongoing support and an outreach nurse and the local psychologist for older people have provided training sessions for staff. Staff confirmed that they had participated in in-house dementia care training. Staff interaction with service users was observed to be calm and patient and respectful. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 -15 Service users are able to follow a lifestyle that suits their individual needs and they are able to maintain social contacts. EVIDENCE: Service users said they were happy living at Morrell House. The relative of one service user was very pleased with the standard of care offered to her mother and said it exceeded her expectations. Service users are able to maintain contact with family members. There is an in-house church service. An activities organiser has been employed and will lead all staff in engaging service users. At the time of the inspection there was an informal gathering to mark the retirement of a member of staff. All service users had been invited and enjoyed the occasion. The manager is aware of the need to promote a sense of well-being in service users and conversation with service users would suggest that this is being done successfully. Observation of the lunchtime meal provided evidence that all service users were able to manage without assistance. The meal was well-presented and was unhurried. Choice is available at every meal. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 - 26 Service users live in a safe environment which is pleasant and comfortable. EVIDENCE: There have been extensive alterations to the home and new seating areas and a walkway have been created. New furniture, carpets and curtains have been obtained. The accommodation is very spacious and includes several lounges and dining areas and a large reception area. All bedrooms are intended for single occupancy and 8 have newly installed en-suite shower and toilet facilities. The bedrooms are comfortably furnished and have been decorated with personal items such as photographs and ornaments. Throughout there is good natural lighting. There is a secure garden. There are certificates relating to the safety of the premises. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 -30 Service users’ needs are met by a competent, well-trained staff group but vetting of new staff needs to be tightened up. EVIDENCE: The staffing levels are adequate and there is provision for them to be reviewed once the service reaches maximum occupancy. There will be 3 waking night staff which may be increased to 4 if required. Each day shift will be overseen by the registered manager or deputy and there will always be a care leader on duty. An activities co-ordinator has been recruited. The staff have received a good level of training in the care of people with dementia. A relatively new member of staff confirmed that she had completed induction training and in her first week spent her time shadowing experienced staff. The staff records are not maintained by the manager in the home. And at least one newly recruited member of staff was started prior to a Criminal Records Bureau check having been carried out. This is not uncommon practice in the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for standard(s) 31 & 32 Service users live in a home run by an experienced and suitably qualified manager. EVIDENCE: The manager of the home has completed an NVQ4 management and a BTEC Diploma in the management of care. She and the deputy manager have recently attended a number of conferences and have acquired a range of publications concerning the health and social care needs of people with dementia. Staff confirmed that they had participated in in-house dementia care training and said that the management team were approachable and knowledgeable.
