CQC found this care home to be providing an Excellent service.

The inspector found no outstanding requirements from the previous inspection report, but made 3 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

Although the visits made were limited in focus, we were able to make some other observations. We noted that the people who use the service were supported by staff in a professional manner with all staff interacting with individuals in a friendly and positive manner. Explanations were given to individuals when staff could not immediately attend to their needs and there appeared to be a relaxed feeling within the building. The Acting Manager was able to provide all the documents we asked for including staff rotas, supervision records and care plans and we were able to talk to staff in private during our visits. The service fully co-operated with us during the visit.

What the care home could do better:

The service must ensure that when shortfalls in staffing occur, the Care Quality Commission is notified without delay. The service must ensure that the whistle blowing procedure is reinforced to the staff team. The service must gain an overall indication of the dependency levels of the people who use the service on a monthly basis so that planning of responses to staff levels can be made to ensure that people`s needs are met.

A lounge/dining room is provided on each floor with televisions, videos, piano, and music centres available. The home is double glazed throughout and ramp access is available at the front entrance. A lift serves all floors. All accommodation offered is in single rooms and the majority have the benefit of en-suite facilities. A selection of bathrooms, showers and toilets are provided on each floor and assisted bathing facilities are available. The small front and side gardens are well maintained and car parking is provided at the front of the building. Fees range from local social service rates to #481. Care Homes for Older People Page 3 of 9 What we found: The purpose of this visit was to respond to complaints made in respect of staffing levels within the service as well as the way the service had managed a safeguarding referral in respect of a member of staff working in the service. The complaints about staffing levels had included details of how the care provided had been adversely affected by this. We conducted two visits to the service. The purpose of the second visit was to gain more evidence in respect of staffing rotas and to talk with staff in order to gain their experiences. The first visit to the service was unannounced. We looked at the management of a safeguarding issue that had been raised. We found that the issues did not constitute a referral under Local Authority procedures since it did not involve actions that would constitute abuse rather it raised the need for the member of staff to receive further training and supervision in the way the person interacted with the people who use the service. Evidence was provided to us to suggest that appropriate action had been taken and that the person continued to receive supervision from their manager. No further concerns have since been raised in respect of this person. We looked at staffing rotas and management returns completed by the manager over September and October 2009. At the time of the visits, the service were supporting 41 people and it was anticipated that this was to increase that week given that the service had receive further referrals. For October 2009, rotas suggested that staffing levels in the morning achieved a ratio of one member of staff to five individuals. This changed to ratios of one member of staff to seven individual in the afternoon and at night one member of staff to every ten individuals. Rotas suggested that these levels had been achieved throughout October 2009. The same rotas suggested that for every morning there were seven members of staff on duty with this changing to six people in the afternoon. We looked at management returns for September 2009. These are documents completed by the Acting Manager indicating staffing levels within the service for each week of the month. These indicated that hours provided to each person per week varied from 30.9 hours of care provided to each person up to 34.75 hours during that month. While the service does complete a monthly assessment of the dependency levels of each person recorded within their care plan, this system does not provide an overall view of the dependency levels of people. The collating of information on a monthly basis must be done so that planning responses in staffing levels to changes in need can be achieved to best meet the needs of individuals living at Mother Redcaps. This must also be done in order to ensure that the differing needs of the people (i.e. dementia care, nursing needs, social care needs) using the service is taken into account. We interviewed staff members specifically about staffing levels within the service. In all cases staff considered that there had been recent occasions where staffing levels had not been maintained and as a result they considered that they had been unable to provide a good quality standard of care to the people who use the service. An example was given that on one recent but unspecified date, three care staff had been on duty covering the Care Homes for Older People Page 4 of 9 entire service. We had not been made aware of this alleged event. As a result the whistle blowing procedure must be reinforced to staff and if shortfalls in staff do occur, the service must also inform us if and when such shortfalls occur.
