Care Homes for Older People Page 3 of 8 What we found:
We carried out a short inspection at Nazareth House on 26 March 2009. This was to see how admissions to the home are managed and how care is given to people with high support needs. The manager was not on duty at the time of our visit and we spoke with senior staff on the three floors where people are accommodated. References to us or we in the report represent the Care Quality Commission. During our visit, we walked round the home and checked various records to see what services the people in Nazareth House receive. We use a system called case tracking to help us to look at services from the point of view of some of the people who use them. We track peoples care to see whether the service can meet their individual needs. We looked at the checks done about the care needs of a person who had moved in to the home most recently. This person had returned to Nazareth House from hospital a few days before our visit. There was no record that staff from Nazareth House had visited the person in hospital to carry out an up to date check of her needs before she returned to the home. However staff had kept in touch with the persons family and ward staff while she was in hospital and had recorded her progress until discharge. Records show that a senior member of staff had discussed this persons needs with ward staff on the telephone before she returned home and arrangements were made for the district nurse to visit her on return. A copy of the check on her care needs done by ward staff at the time she was discharged from the hospital had been given to Nazareth House when the person returned to the home. These checks had provided information about the persons general condition and care needs at the time she was discharged from the hospital and moved back into the home. We checked the care records of six people who live at Nazareth House to see the care they receive. We spoke with some people about the care they receive and their views about the home have been taken into account in this report. We found that the support needs identified in the check done about a persons care needs had not been clearly addressed in the care plan so that staff had no clear instructions about how to look after the person properly and safely. We found that in most of the files we looked at, care plans lacked detail. Although care plans had been reviewed recently by staff, they did not fully address each persons needs. Examples of this are regarding pressure care, catheter care, moving and handling and external application of prescribed creams. We found that risks to peoples safety had not been addressed or recorded, for example regarding people who may be at risk from leaving the building and losing their way, due to short term memory loss. A number people have been identified as needing bed rails to protect them from falling out of bed. There was no record of the bed rails having been checked regularly to make sure that they were safe and did not have gaps which could cause entrapment. What the care home does well:
We met several people who live in Nazareth House and they all looked well cared for regarding their personal grooming and clothing. People appeared relaxed in their home and were spending time in the lounges or in their bedrooms. There were plenty of books and newspapers available to them and a notice board to give them information about what was happening in the home. Each lounge has a television and music system. The bedrooms we visited were personalised and comfortable. A visitor told us Care Homes for Older People
Page 4 of 8 that their relative is well cared for in Nazareth House and they are more than satisfied with the service. Care records we looked at showed us that information is kept about peoples spiritual needs, which confirmed that their individual preferences and beliefs in this are being met in Nazareth House.
