Conditions of registration: Date of last inspection 26th October 2005 Brief Description of the Service: Northgate House is a family owned care home that is registered to provide care and accommodation for 25 service users over the age of 65, including those with dementia. In recent years the building has been extended to provide attractive en-suite accommodation. There is a well-tended garden and patio area located at the rear of the premises that provides a safe and secure area for service users. There is a car park for staff and visitors. All areas of the building are accessible to service users via the use of ramps and a stair lift. The home is located close to the town centre and to local amenities, including shops, pubs, clubs and cafés. Northgate House DS0000019701.V284597.R01.S.doc Version 5.1 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This inspection was unannounced and took 4 hours including preparation time. Not all of the National Minimum Standards were assessed on this occasion. Those covered in this and the most recent inspection (26th May 2005) include all of the key standards relating to the welfare, health, safety and protection of service users. During the inspection, a tour of the premises was undertaken, and the inspector spent time talking with residents and observing their daily lives and checking progress on the requirements and recommendations from the last inspection. The home had a welcoming atmosphere and staff were helpful and polite. All of the residents spoken to confirmed that they were satisfied with the care they received. One resident described it as a “Home from Home” whilst another complimented the caring nature of the staff and the respect and dignity that was shown by staff towards residents. One bedroom however had a strong malodour, and a requirement was made in this regard. Good progress had been made in implementing the requirements from the previous inspection. What the service does well: What has improved since the last inspection?
Senior staff have received accredited training in the safe use and handling of medication and a pharmacist has visited the home to give advice safekeeping, storage and administration of medication. The manager has started his Registered Managers Award and senior staff have enrolled on an occupational Health and Safety course. A further resident bedroom has been decorated. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 The homes procedures for the safekeeping and administration of medication to service users were being satisfactorily maintained. EVIDENCE: The home had medication policies and procedures in order to protect the residents and discussion with staff confirmed that they had received training in the safe use and handling of medication. Since the last inspection additional accredited training had been commissioned and delivered to all of the senior staff, and a local pharmacist had visited to give advice and develop protocols in this regard. A random inspection of the medication supplies was satisfactory and the home’s records accorded with the supplies kept within the home. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 The home was safe and generally well maintained although some areas were in need of decoration via the home’s maintenance programme. One bedroom needed attention to deal with an unpleasant odour. EVIDENCE: The home was domestic in nature and pleasantly furnished and an ongoing programme of maintenance was in place. Since the last inspection, a further bedroom had been decorated, but some of the hallway carpets were in now need of upgrading and refreshing. One resident bedroom had a strong malodour and a requirement was made in this matter. The laundry was tidy and precautionary measures had been taken for combating MRSA infection. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28, 29, 30 The staff were receiving appropriate training in order to ensure the welfare of residents was ensured. EVIDENCE: Discussion with residents and staff and observation of the interactions within the home indicated that the staff were competent to do their jobs and that the residents were in safe hands. Residents complimented the caring nature of the staff and one made a point of emphasising the respect and dignity that was afforded to residents. The file of a newly recruited staff member confirmed that satisfactory recruitment procedures were being followed. A comprehensive programme of staff training was in place resulting in over 75 of staff having achieved NVQ 2. In addition one staff member was undertaking NVQ Level 3 and two are working towards Level 4. Domestic and administrative staff are included within the home’s NVQ programme. The manager stated that in addition to the accredited medication undertaken since the last inspection, senior staff members had also enrolled on an occupational Health and Safety certificated course. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 38 The home was being appropriately managed in order to ensure that the health and safety of residents and staff was safeguarded. EVIDENCE: Discussion with residents and staff indicated that the home was being well run. Staff described the manager as being “approachable” and discussion with him confirmed that he was in the process of undertaking his Registered Managers Award. The manager confirmed that he had discussed this with his course assessor, in order to ensure that this will cover the necessary care modules that are needed for managing a care home. Records inspected were being satisfactorily maintained and evidence was seen that staff training was being implemented. Staff confirmed that they were receiving supervision in order to assist them to do their jobs. The manager stated that he maintained close contact with the Registered Providers of the home in order to keep them updated.

As indicated previously (standard 28) senior staff had completed accredited medication training since the last inspection and were now in the process of undertaking an occupational Health and Safety course.
