Under Annex 2, a maximum of 23 service users requiring nursing care who fall into the category of either OP or PD A max of 28 service users requiring personal care of the category OP Staffing for service users requiring nursing care will be in accordance with the Notice issued dated 22 January 1996 4th October 2005 Date of last inspection Brief Description of the Service: Northwood provides long and short-term care for a maximum of 28 residents who are elderly and have physical need for which they need nursing care. The registered persons are Mr and Mrs O’Neill. Mrs O’Neill takes responsibility for managing the home on a day-to-day basis. The home is a converted and extended Victorian property, which has retained its style and character. It is located in a residential area within close proximity of all local amenities (shops, churches, bus route and public houses) and less than a mile from the centre of Blackburn. Bedrooms at the home are a mix of single and shared rooms, some of which have en-suite facilities. There are a number of lounge and dining areas located on the ground floor. A passenger lift serves both floors. Residents have access to the large garden and patio area. Car parking space is located to the front of the house. This is an overview of what the inspector found during the inspection. This unannounced inspection took place on the 21st February 2006. Much of the information gained was obtained from talking to residents and staff members. The views of residents were obtained on a variety of topics. Two residents were case tracked. Case tracking gave the inspector an overall view of the specific care for the individual resident by checking the plans of care, other documentation and talking to residents and staff. Two staff members were talked to about care issues. The general manager was present throughout the inspection. Some of the views have been reported collectively with specific comments contained within the body of the report. The inspector took detailed notes during the inspection, which have been retained as evidence. Staff were directly and indirectly observed carrying out their tasks and interacting with residents. Paperwork examined included plans of care, assessment documentation, policies and procedures or documents relevant to each standard. Core standards – inspected and met on 4th October 2005 were not re-inspected if evidence was not found to warrant further investigation. A tour of the building was conducted. There had been one complaint made to the Commission for Social Care Inspection since the last inspection, which had been suitably investigated by the service. What the service does well:
The assessment process was good and ensured residents needs could be met at the home. The relationship between staff and residents was good. Residents (including the two case tracked residents) said, “I am happy here”, “all the staff are very nice – it’s like being at home”, “staff are very friendly and they help you” and “nowts too much trouble for them”. The interaction between residents and staff provided a good atmosphere for residents to live in. Food was described as “good” and “plenty of choice” by residents spoken to. Food was satisfactory to resident’s tastes. Staff were well trained and had the knowledge and experience to look after the residents competently. Plans of care were good and provided staff with sufficient knowledge to care for residents. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 The assessment of residents prior to admission ensured the home gained sufficient knowledge to help meet their needs. EVIDENCE: Two plans of care examined during the case tracking process contained assessment documentation from Blackburn with Darwen NHS Trust. The manager of the home had visited residents prior to admission and assessed them prior to admission. One resident newly admitted remembered a member of staff visiting him, explaining the service and discussing care. The resident found the member of staff to be “very friendly”. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 and 8 Residents had their health, social and personal needs set out in a plan of care. Resident’s health care needs were met. EVIDENCE: Two residents were case tracked during the inspection and their plans of care examined. Plans had been developed with residents or a family member and reviewed on a regular basis. Both residents were questioned about their care and plans. Two staff members were questioned about the care they gave to the two residents. The care given by staff matched the care written in the plans and met residents expectations. Plans were accurate and up to date to ensure resident’s needs could be met. Plans of care contained evidence residents attended specialist clinics (such as the diabetic clinic) and received attention from opticians and chiropodists. Both residents case tracked were able to confirm they attended health care specialists. Residents had been assessed for their nutritional and pressure area needs. The tissue viability nurse and dietician had visited the home to offer advice or treatment. Residents had been risk assessed for falls. Residents received treatment and advice from health care specialists to try to maintain or improve their condition. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 - 26 Residents lived in a secure environment. Communal and private space was homely and met current requirements. Parts of the home would benefit from improvement. The home was clean and hygienic. Aids and equipment met resident’s needs. Laundry services were provided for residents. EVIDENCE: The inspector conducted a tour of the building and visited all communal rooms and some bedrooms. Rooms visited had been personalised to resident’s tastes. Residents case tracked were satisfied with their rooms and communal space. There was a garden, which was accessible and safe for residents. Residents were observed sitting together and socialising in various lounges of the home. Toilets and bathrooms had disability equipment suitable for the resident group accommodated. Equipment such as hoists and grab rails was observed during the tour. Bedrooms had a lockable door and lockable facility within the room. The general décor was domestic in character but would benefit from upgrading in some areas. The general manager produced a copy of Northwood’s business plan dated January 2006 to December 2008. This highlights work and timescales for upgrading the home over the three-year period. It was

The facilities and décor was satisfactory to residents. There was an adequately equipped laundry. There were no hand washing facilities in the laundry. Floors and walls needed attention to be more easily cleaned. Staff said they had taken training in infection control. Two areas recommended at the last inspection had not received attention but was to be completed as part of the overall plan. Policies and procedures were in place for the control of infection and helped protect the health and welfare of residents and staff. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33 and 38 The registered manager had completed NVQ training to meet the requirements of the CSCI. Quality assurance systems had been developed but not practiced for some time and therefore the views of residents, family members and stakeholders were not up to date. Health and safety systems were in place for the protection of staff and residents. EVIDENCE: The general manager said, “the registered manager has now qualified in NVQ4 management and is a registered nurse I have also qualified in NVQ4 but need to complete another unit to attain the Registered Managers Award”. The registered manager and general manager updated their knowledge on a regular basis. The management of the home provided staff and residents with the direction needed to provide a good service. Quality assurance systems had been developed over time but the views of residents, families and stakeholders had not been obtained for two years. Recorded meetings were held with staff and residents. The views of all

There was a health and safety policy and procedure. Health and safety posters were placed within the building. Staff has undertaken training in health and safety related issues. Not all certification of gas and electrical appliances or equipment was available at the inspection or was out of date. Work was being carried out on the day of the inspection to remedy this.
