The home is registered to take 33 residents and provides nursing and residential care for older people. It is situated close to all the local amenities and shops in Harrogate. This is an overview of what the inspector found during the inspection. The unannounced inspection took place over one hour and fifteen minutes. The deputy manager was available to assist in this process. Service users and staff were spoken with. A tour of the building was undertaken; this included communal areas, service users bedrooms, laundry, kitchen and staff room. Records inspected included the controlled drugs register, medication policy and kitchen records of fridge, freezer and cooked meat temperatures. At the end of the inspection a feedback session was held with the deputy manager and the regulatory inspector. What the service does well: What has improved since the last inspection?
Controlled medication was now being witnessed when dispensed and given to service users. Both staff then sign the controlled medications register. The ordering of medication has been reviewed; the system has been made more efficient. An infection control audit system has been introduced. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 9 Service users are protected by the homes practices, policies and procedures regarding medications. EVIDENCE: This standard was not fully inspected. However the highlighted shortfall from the last unannounced inspection was examined. The controlled medication register was observed, It was found that two staff now witness the dispensing and giving of all controlled medication. This complies with current law and legislation. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 15 Service users receive a wholesome and appealing diet in pleasing surroundings, However external transportation of food to the dining areas may pose a risk to service users. EVIDENCE: Service users could eat within their own rooms or in the two dining areas. Tables were laid with linen cloths and napkins; there were flowers on the tables. Service users requiring assistance and encouragement were seen to be helped in an unhurried manner by attentive staff. Conversations were occurring between service users at the dining tables, and it was apparent that mealtime was a very sociable and enjoyable event. A service user stated, “The food is good “. The kitchen was inspected it was clean and tidy. Food being prepared looked appetizing and wholesome; it was well presented upon the plate. Special diets and puree food was also available. Records of fridge, freezer and hot meat temperatures were examined. Food storage areas were adequate and clean. Stock was rotated upon delivery. A birthday cake for a service user had been made and decorated to a high standard. During the inspection lunch was being served, it was witnessed that the deserts for the downstairs dining area were delivered via the back kitchen door. Food was carried on a tray, exposed to the open air, approximately 3 meters before staff entered the conservatory. This exposed the uncovered deserts to possible risk of contamination. This was discussed with the chef and the deputy manager at the time; Staff was informed that all food must be delivered to the dining room by an internal route, via the dumb waiter.
