Conditions of registration: Date of last inspection 10th August 2005 Brief Description of the Service: Oliver House is a care home run by City of York Council and registered to provide a service for 30 older people of either gender aged over 65 years who do not have any specialist requirements. Oliver House was purpose-built approximately 40 years ago and is located within a short walk of local facilities in Micklegate. The centre of York is within 1 mile. The accommodation is provided in single rooms on three floors. The upper floors are accessible via passenger lift. There is an enclosed rear garden. There are limited parking spaces. This is an overview of what the inspector found during the inspection. This unannounced inspection was carried out on 2nd February 2006;the inspection took three hours to complete along with four hours preparation. A tour of the home was made and discussions were held with the care leader on duty, with service users and their relatives. The staff were seen carrying out general care duties, and their interaction with the service users was observed. A check was made as to whether the home had complied with the requirements and recommendations from the last inspection. Feedback was given at the end of the inspection to the care leader. What the service does well: What has improved since the last inspection?
The policy for recruiting staff policy has been improved, and no staff have been allowed to start working at the home without the necessary checks, this is necessary in order to protect the service user. Fire doors are no longer held open by unauthorised means, enabling them to function correctly and offer protection to the service users in the event of a fire. Out of date medication is disposed of promptly, and the supplying pharmacist has been invited to the home to inspect the medication system, this should ensure that a safe and robust system for the administration of medicines is maintained. Risk assessments that had been stuck onto items of furniture have been removed, helping the home to maintain a domestic, not institutional décor. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 Service users are protected by the homes policies and procedures for dealing with medicines. EVIDENCE: The staff follow the homes procedures for the receipt, recording, storage and disposal of medicines. The medication records for all service users were inspected and these were found to be accurate and up to date. A check of the controlled drugs was made against the records, and this was found to be in order. There are no service users that self medicate at present. The supplying pharmacist has been invited to the home, to inspect the medication systems. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. JUDGEMENT – we looked at outcomes for the following standard(s): 19 Service users live in a home that on the whole provides a safe and wellmaintained environment, however there are some safety issues that need to be addressed. EVIDENCE: Service users bedrooms are comfortable and well decorated. Most of the communal areas are pleasant and furnished to a good standard but there are some areas that need attention. Of the three long corridors in the home only the ground floor corridor had the lights on, the second floor and third floor were not illuminated, and with little natural lighting, this presents as a risk to the service users safety. The smokers lounge is situated on the ground floor next to the non-smokers lounge the smell of cigarette smoke continues to be evident in the home, and the non-smokers health may be put at risk through passive smoking. This room is shabby and needs decorating, a notice that was for staff was stuck onto the window, this should be removed to minimise institutional practice. Several aerosol cans of air freshener and deodoriser spray were lined up on the window ledge by the sitting room; these should be removed for safety purposes and because of the unpleasant association. JUDGEMENT – we looked at outcomes for the following standard(s): 29 Service users are supported and protected by the homes recruitment policy and procedures. EVIDENCE: Ten staff files were inspected including the newest recruitments; all of the files had the documentation required by regulation, including two references and the criminal record bureau check. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 38 The health, safety and welfare of staff could be better promoted and protected. EVIDENCE: Whilst a full inspection of this standard was not carried out, the staff toilet facilities were checked and found in need of redecoration and a thorough clean particularly to the back of the door. The manager should ensure that the lighting to the second and third floor corridors is kept on throughout the day to ensure the safety of the service users. The smokers lounge should be provided with adequate air extraction equipment, the manager must draw up the necessary plan to provide this, and also make a review of this area The manager should ensure that aerosol cans of air freshener and deodoriser are stored safely and discreetly.
