This inspection was carried out on 9th November 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Padgate House provides short term care for older people leaving hospital or requiring a period of additional support. This enables many service users to regain their independence and return to the community. The home also provides respite care. The service is flexible and constantly evolving to respond to changing needs. The service brings together the local authority, the primary care trust and the acute trust in collaboration. A multi-disciplinary team is based within the home, which ensures that service users have access to specialist services on a daily basis. Two residents, who have lived at Padgate House for several years, continue to be supported in their chosen environment. A range of social activities is provided on a regular planned basis. A good choice of meals is offered and the standard of catering is good. The building is clean and well maintained throughout. The local authority provides regular training for staff.

What has improved since the last inspection?

More staff have been given the opportunity to achieve NVQ qualification. The care coordinator has arranged for telephone points to be installed in all bedrooms.

What the care home could do better:

Provide a main entrance that is more easily accessible for service users and visitors who use wheelchairs. Fit automatic door closers to the bedroom doors so that if service users prefer to have their doors open they can do so safely. Achieve the Department of Health target for 50% of care staff to have an NVQ qualification.

Date of last inspection Brief Description of the Service: Padgate House is a modern single storey building located in a residential area of Warrington. It is within easy reach of local shops and other amenities and is served by public transport. The home is owned and managed by Warrington Borough Council. Padgate House DS0000036225.V266970.R01.S.doc Version 5.0 Page 5 A total of 36 places are registered for older people, however a maximum of 35 people are usually accommodated. These are made up of two long stay places, intermediate care for 24 people, step up care for five people and short stay respite care for four people. Service users in receipt of intermediate care are admitted under the care of a medical consultant, other service users receive medical services from local GP practices. Nursing input is provided by nursing staff employed by the local Primary Care Trust. An office in the centre is used as the base for the evening and night nursing service, and the nurses are available for advice and support as needed. Input from physiotherapy and occupational therapy is provided by staff based within the home. Two social workers are also based at the home. The premises was purpose built and designed to meet the needs of older people. There are four lounge/dining areas, each of which has a kitchenette. The bedrooms are single, with the exception of one twin room which is available for service users who may wish to share. Toilets and assisted bathrooms are located near to the bedrooms and living areas. This unannounced inspection was carried out on 9th December 2005 as part of the Commission for Social Care Inspection annual inspection programme. Most of the key standards were inspected at the announced inspection carried out on 27th July 2005 and were found to be met to a good standard. A tour of the communal areas of the building was conducted. Time was spent in conversation with the service manager, the coordinator, the principal officer and other members of staff. Eight service users and two visitors were spoken with. Staff training records, the arrangements for handling service users’ money, and medicines procedures were inspected. Two long stay residents were living at the home and a further 26 people were receiving short term care. What the service does well: What has improved since the last inspection? No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8, 9 and 10. Service users’ health needs are met in full. Medicines are well managed and service users are supported to handle their own medicines. The privacy and dignity of service users are respected. EVIDENCE: Three nurse are employed by the primary care trust to provide nursing care, including administration of medicines, to service users in ‘step up’ places. District nurses also provide services for service users receiving intermediate care, respite care, and for the two permanent residents if needed. Two medical consultants visit the home weekly and a general practitioner visits on most days. Physiotherapy and occupational therapy are provided on a daily basis. Other services such as optician, chiropodist, dentist, are contacted as and when needed. All medicines received into the home are recorded in detail in a medicines book. Most service users receive their medicines in a NOMAD dispenser, and senior care staff demonstrated that they are familiar with all of the tablets contained within the NOMAD’s and contact the pharmacy immediately with any queries. Some service users are able to self-medicate and a full assessment is recorded. Whenever possible, service users are familiarised with their medicines prior to discharge. All of the service users spoken with said that staff treat them with respect and are always polite and kind. Bedroom doors are fitted with locks and service users are provided with a key for their door. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 The social needs of service users are provided for. Visitors are welcomed at the home at any reasonable time. Service users are able to exercise choices in daily living. Service users receive a good diet. EVIDENCE: An activities organiser is employed and a programme of social activities is available for all who wish to participate. Service users spoken with said that they enjoy the social activities that are organised at the home. Several service users were knitting squares to make a blanket. There were several visitors to the home during this inspection. Service users spoken with said that their relatives and friends can visit at any reasonable time. One service user was enjoying a visit from her dog. Service users may take visitors to their bedrooms for a private conversation. They have a choice of sitting areas, and some people prefer to stay in their own room. The home has a payphone which is situated in the smoking lounge, however the coordinator has arranged for each bedroom to be fitted with a telephone point so that service users will be able to make and receive calls in privacy. Recommendation Breakfast is served on an individual basis at any time during the morning. Cooked breakfast is available between 8am and 9:30am. Lunch is at 12, tea at 4pm, and supper, which includes sandwiches and snacks, is at 7:30pm. Night staff have access to all food supplies. There is a choice of three main courses at lunchtime. Hot and cold drinks are made in the kitchenette areas whenever service users would like them and biscuits are always available. Service users expressed their satisfaction with the standard of the meals provided. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 The home is clean and well maintained. EVIDENCE: A tour of the building showed all areas to be clean and well maintained. The home has recently recruited a new maintenance person who is shared with a similar service. There are outdoor sitting areas planted with flowers and shrubs, and service users were enjoying watching birds in the garden where there are bird tables. A range of mobility equipment is provided by the therapy staff employed at the home. Fire doors in the corridors are fitted with magnetic catches that close the doors automatically when the fire alarm is activated, but bedroom doors are not fitted with these devices, so must be kept closed. The coordinator said that she had been obtaining estimated for a new fire alarm system for the home that will include automatic closers for the bedroom doors. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28, 29 and 30. The home is working towards the Department of Health target for 50 of care staff to have a qualification in care. Recruitment is carried out by the local authority’s human resources department and personnel records for recently recruited staff were not available for inspection. Staff receive training to enable them to carry out their work. EVIDENCE: Warrington Borough Council has an NVQ centre. Six members of care staff have achieved NVQ level 2 in care and two have level 3. Eight staff are working towards NVQ level 2 and seven towards NVQ level 3. When these staff attain their qualification, this standard will be met in full. The home’s coordinator is working towards a Registered Manager Award. Two new members of staff had been recruited since the last inspection. There were no personnel records on the premises to demonstrate that the required pre-employment checks had been carried out. These are held centrally by Warrington Borough Council, and following the inspection the local authority provided evidence that the appropriate recruitment checks had been carried out prior to the two new staff commencing work at the home. The home’s training records demonstrate what training each member of staff has received and when. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35. Service users’ personal money is handled appropriately. EVIDENCE: The home employs administration staff who look after the day to day finances of the service. There is a safe where service users can deposit small amounts of personal money. The home’s staff assist one of the long stay residents to manager his finances and good records of this are maintained. On the day of the inspection two short stay service users had a very small amount of money in safekeeping and records were available for inspection. If service users bring a significant amount of money with them on admission, this is passed to their social worker for safekeeping.
