The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home is clean and provides a comfortable and homely environment. Residents are helped to lead as normal a life as possible and enjoy going out into the community independently. Staff and residents also go out as a group using public transport for day trips which are planned together often on the spur of the moment. Residents said that they enjoyed the food and have a mixture of home cooked food, takeaways and meals out as people would who live in their own homes. Routines in the home are flexible and centred round the needs of the residents.

What has improved since the last inspection?

The statement of purpose has been updated. The home has a written complaints procedure.

What the care home could do better:

Care planning is poor and must be improved to make sure that staff know the action they need to take to meet the residents` needs. All care records should be dated and signed by the staff completing them. Any restrictions placed on residents must be recorded. Radiator guards must be fitted to protect the residents from hot surfaces. A robust recruitment procedure must be put in place for all staff employed at the home. This will make sure that staff are suitable and safe to work in the home and safeguards the residents.Duty rotas must be recorded and show all staff working in the home.

Date of last inspection 25/03/05 Brief Description of the Service: Parkfield Rest Home is a large Victorian house in a residential area near Valley parade football ground in Bradford. The home is registered to provide personal care for up to nine older people. The accommodation consists of three single and three double bedrooms. There is a lounge and separate dining room. There are gardens to the front and a large paved area at the back of the home. This announced inspection was carried out between 10am and 3pm. The provider said that she did not receive notification of the inspection and explained that she had problems with post not being delivered. During the visit I spent time with the three residents who are living at the home. I looked around the home and had lunch with the residents. I looked at some records including care plans, duty rotas, staff recruitment files, maintenance records, accident records and food records. The manager and senior care assistant were on duty and both were present for feedback at the end of the visit. What the service does well: What has improved since the last inspection? No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9 and 10. Care planning is poor and must be improved so that staff know what action to take to meet the needs of the residents. Suitable arrangements are in place for residents to access health care services. Residents receive their medication as prescribed. Residents’ privacy and dignity is maintained. EVIDENCE: I looked at the care records for all three residents. There was some good information recorded about the resident’s previous life and their interests and lifestyles. However care planning was poor with little information to show what help and support staff need to provide to meet the resident’s needs. Many of the records were not dated or signed. Some monthly reviews of care were recorded but these were not linked to the care planning process. Day and night reports for all the residents are recorded in one book. It was clear from discussions with the manager and staff that they have a good understanding and knowledge about the individual needs of all the residents. They are committed to providing good care and welcomed the suggestions I made about improving the care records. It is recommended that an individual file is used to store all the care records for each resident including their daily reports. This will make it easier for staff to access any information. Care plans must show how the resident’s health, social and personal care needs are met. They should be drawn up in consultation with the resident and reviewed at least once a month. Arrangements are in place for the residents to access health care services as required. The chiropodist and optician visit the home. The manager said that all three residents refuse to see a dentist although if emergency treatment were needed this could be accessed through the GP. A community psychiatric nurse provides specialist support to one of the residents. Medication administration charts showed that residents are given their medicines as prescribed. Storage and ordering arrangements were not discussed at this visit. The resident’s privacy and dignity is maintained. Screening is provided in shared rooms. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,14 and 15. Residents are supported to keep their chosen lifestyles by staying independent and going out in the community. Residents enjoy their meals. EVIDENCE: Residents are encouraged and supported to maintain their independence and enjoy the same lifestyles as living in their own homes. Residents regularly go out independently using local transport to go shopping or just out for a walk. The staff and residents also all go out together as a group for the day, again using local transport and enjoy shopping in local markets and having meals out. Residents said how much they’d enjoyed a recent day out when they’d gone on the train to Skipton. Planning trips is flexible with residents and staff agreeing what they want to do according to the weather and how everyone is feeling. The residents have regularly holidays and said how much they’d enjoyed a four day break to St Annes in November. Residents are given choice in their lives in relation to how they spend their days. This includes what times they get up and go to bed, when and what they have for their meals and how they chose to spend their time. There are some restrictions in place to safeguard the residents, which have been agreed Parkfield Rest Home J52 S1217 Parkfield V231098 210705 Stage 4.doc Version 1.30 Page 12 with the individuals and their social workers. These restrictions must be clearly recorded in the care records. Residents and staff had lunch together and enjoyed a takeaway from Kentucky Fried Chicken. The food diary showed that a variety of meals are provided which includes takeaway meals and eating out. Residents said that they enjoyed their meals. Environmental Health officers have carried out a recent inspection of the kitchen and their report showed everything to be satisfactory. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 24, 25 and 26. The home is comfortably furnished and homely. Residents’ rooms are clean and communal areas are well used by the residents. EVIDENCE: All parts of the home were inspected. Only two bedrooms are currently occupied. Two residents share one room. Bedrooms are comfortably furnished and were clean. The residents have few personal possessions due to the lives they have lived before moving into the home. Communal areas are comfortably furnished and well maintained providing a homely environment. There are some pictures and ornaments as well as photographs of residents on outings and celebrating special occasions. Some radiators are unguarded. Radiator guards must be fitted to make sure that the residents are kept safe from the risk of hot surfaces. The manager agreed to produce a plan to fit radiator guards throughout the home. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. Duty rotas must be maintained. Recruitment procedures are very poor and must be improved to safeguard the residents. EVIDENCE: There are only three residents living at the home. The manager lives on the premises and works as part of a small staff team. The manager said that she had experienced difficulties in recruiting suitable staff. Recruitment records for five staff were examined. Recruitment practices are very poor with written references on file for only one employee. There were Criminal Record Bureau (CRB) checks for only two of the staff. There were no application forms completed for any of the staff, just some personal details included on a form. A relative of the manager who is 16 years old is working in the home. There was no employment information available for this person. The manager advised that this person is not involved in any personal care of the residents. All staff working in the home must be subject to a robust recruitment procedure that establishes their fitness and suitability to work in the home. This protects the residents by ensuring that the staff who care for them are thoroughly checked before starting work at the home. The duty rota for the week of the inspection was seen but the rotas for previous weeks could not be found. Duty rotas must be maintained showing the hours all staff are working in the home.
