Conditions of registration: Date of last inspection 9th August 2005 Brief Description of the Service: Parklands Residential Home is a two storey detached building located on one of the main thoroughfares of Preston. The home is close to local amenities and has a garden area to one side and parking to the rear. Parklands are registered to provide personal care to fourteen older persons of both sexes. There is a clear commitment to provide a high quality of individualised care and promote the independence of service users, within a homely atmosphere. Social activities both within and outside the home are arranged in consultation with service users and visitors are made welcome. This is an overview of what the inspector found during the inspection. The inspection was unannounced and concentrated on the standards not assessed at the last inspection. As well as the registered manager, staff and service users were spoken with and a tour of the premises took place. What the service does well: What has improved since the last inspection? The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1&2 Written information is available about the services provided by the home and each service user has a written contract that details the terms and conditions of residency. EVIDENCE: A Statement of Purpose and Service User Guide is available and is due to be reviewed and up-dated with regard to the staff and their qualifications and other changes that have taken place within the home. The service user documentation seen includes a statement of Terms and Conditions/contract for all service users. The documentation was signed by the service user or their representative and they are provided with a copy. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 & 11 Appropriate medication policies and procedures are in place to ensure the protection of the service users. Procedures are in place with regard to the death of a service user. EVIDENCE: There are appropriate medication policies and procedures and the medication is now in a secure cabinet with a facility to store any controlled drugs; currently there are no service users who require a controlled drug. A lockable box is now available within the fridge for any medicine that requires to be refrigerated. Three senior staff are completing certificated medication training, ‘Managing and Safe Handling of Medicines level 2’; the registered manager has already completed the training and is now helping the staff. Each service user now has an individual medication file that includes a photo and has proved useful for visiting health professionals. The manager regularly checks the record of medication provided for accuracy. Friends and relatives are supported and can offer comfort according to the needs and wishes of the service user. At the initial assessment important details are recorded about individual service users; there is a space on the form for the wishes of service users with regard to their death. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15 A varied menu is planned and provided that provides service users with nourishing meals according to their wishes. EVIDENCE: Meals are served in an attractive and well-appointed dining room overlooking the garden area. Discussion with individual service users indicated that meals are varied and appealing, and the menus show that a nutritious and balanced diet is provided. Service users confirmed that mealtimes are flexible and unhurried and are offered individual choices aside from the main menu. The home employs the services of a full time cook and one of the carers covers for her at other times. A Dietician has been contacted over to support one of the service users who currently have a poor appetite and there is also liaison with General Practitioners where necessary. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 23, 24, 25 & 26 The home provides a comfortable safe environment that meets the needs of the service users. EVIDENCE: The home is well maintained and meets service users’ needs in a comfortable and homely way. A programme of maintenance and renewal is ongoing. The parking and garden areas are kept safe and tidy with service users having full access to these external areas. The internal environment is attractive and domestic in character. Since the last inspection one of the corridor walls has been painted and certain bedrooms have been decorated with new carpets and curtains and one of the bedrooms has a new wardrobe. The home has ample communal space for the number of service users. The lounge areas are furnished in a homely style and provide facilities for a range of social activities. The dining room is bright and spacious with service users accessing this area other than at meal times e.g. for board games and
Parklands Residential Home DS0000009847.V256011.R01.S.doc Version 5.1 Page 14 dominoes. A conservatory has been built off the dining room and provides access to the garden. The home now only accommodates non-smokers. There are sufficient toilet, washing and bathing facilities for the number of service users. One of the single bedrooms and one double bedroom has ensuite facilities; there are hand basins in each of the bedrooms. The home has a variety of aids and equipment to meet the needs of service users. A lift provides access to the first floor and ramps ensure that service user have access to the outside areas. Specialist assessments and the provision of further aids would be instigated in response to specific individual needs as required. The home has grab rails in key locations and storage areas for equipment. The bedrooms are furnished to a comfortable standard and meet the needs of the individual service users, who are also able to bring their own furniture into the home and personalise their rooms according to the space available. All the bedrooms have locks on the doors and lockable drawers for the storage of medication or valuables within the room. Thermostatic devices are fitted to all hot water outlets in service user accommodation to control the temperature of the hot water delivered. Emergency lighting is in place and all equipment within the home is routinely maintained. All the radiators and pipe work have now been guarded to provide safe surface temperatures. The home has appropriate policies in place regarding hygiene and control of infection. Guidance on hand washing is displayed in the laundry and a copy of the policy on infection control is also sited there. Laundry facilities are appropriately sited and equipped and easily cleanable. JUDGEMENT – we looked at outcomes for the following standard(s): 28 Training continues to be a high priority for staff. EVIDENCE: All staff are encouraged to attend training as well as medication training, since the last inspection, all staff have been provided with training in the ‘Protection of Vulnerable Adults’. Other training has included Moving and Handling, First Aid, Fire Safety and Health and Safety. All staff are completing training in Adult Literacy to assist them with report writing.
