This inspection was carried out on 21st February 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The service users are supported to and encouraged to maintain their independence. Service users gave positive comments about the care they received including " very settled" and "really happy" and "could not be better". Service users views are regularly sought about their care and how the home can improve the service and what it does well, through questionnaires, meetings and reviews.

What has improved since the last inspection?

The home has had a new stair lift since the previous inspection. The fire risk assessment has been updated. The manager ensures that all records are now clearly signed and dated.

What the care home could do better:

The home should have a more formalised system for seeking stakeholders and family and friends views about how the home can improve and what it does well. The home needs to ensure that risk assessments are undertaken on the use of bed rails and also the grounds of the home.One door was opened by unauthorised means an alternative needed to be in place.

One (1) named service user under 65 years of age may be accommodated within the maximum number. The condition is to cease when the named Individual ceases to live at the home. 8th June 2005 Date of last inspection Brief Description of the Service: The Promenade Hotel is a residential home that caters for the needs of 24 elderly service users. The home is owned by Continuing Care Services Limited and was first registered on the 25th January 1994. Personal care is provided to all service users along with meals and a laundry service. The home is located on the sea front at Hornsea, close to all town amenities and public transport. The home comprises of three houses that were originally converted into a hotel and restaurant. Accommodation is provided on three floors - the first and second floors are accessed via a stair lift. The environment consists of 2 lounges, 14 single rooms and 5 shared rooms. The home has an extensive garden at the rear and car parking is available at the front and rear of home. The inspection was completed as part of the programme of inspections for the year. It was an unannounced inspection undertaken by one inspector. The inspection day lasted from 9.15 am to 2.40 pm with a previous 2 hours preparation also being completed. During the inspection a tour of the premises was completed, and a review of the documentation including care plans and recording systems. Time was also spent with residents in the communal areas. The inspector had lunch with the service users. The inspector spoke to eight of the twenty-two residents and discussions were held with staff and the manager. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6 EVIDENCE: The home does not offer intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 Services users are health and medication needs are fully met. EVIDENCE: The home has a policy and procedures on medications. The medication for three service users was checked. The home uses a NOMAD system and a local pharmacist supplies this. The home had no controlled drugs at present but had appropriate storage and recording system in place. There are presently no service users who self medicate but this is offered to all service users when they are accommodated. Staff monitor the condition of service users on medication and call a GP if concerned. The staff that administer medication have not completed accredited training but are enrolled on a course, which starts in the near future. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14,15 Service users have choices and control over their lives that helps them maintain their independence. Service users have a well balanced diet and meals are well presented. EVIDENCE: Service users can make choices regarding their daily living and the staff promote their independence. The service users are encouraged to handle their own financial affairs. Service users are aware that they can access their own records. During the tour of the home it was noted that service users could bring in their own personal possessions when they are accommodated. The home have recently appointed two new cooks and the present menus are to be revised. All meals prepared offer fresh vegetables and are well presented. The inspector had lunch with the service users and the comments received about the food were positive. Service users can choose to have their meals in their rooms or in the dining room. It was noted that mealtimes are unhurried and service users are given time to eat. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 25 The service users live in a safe and comfortable ‘homely’ environment. EVIDENCE: The home offers a safe and ‘homely’ environment. The pipe work and radiators have been guarded with the exception of one in the downstairs communal toilets. The manager envisaged that proposed work would have been carried out on renewing these facilities but this has still not been completed so this radiator requires guarding. All the rooms are light and naturally ventilated. From the previous inspection the water temperatures in the washbasins has been controlled by pre-set valves in half of the building. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28,29,30 The procedures for the recruitment and selection of staff are robust and offer protection to service users. Staff are employed and trained in sufficient numbers to meet the service users needs. EVIDENCE: Three staff files were checked all have the appropriate checks, with the exception of one staff that had brought the Criminal Records Bureau (CRB) from the previous employer. This was discussed with the manager who was not aware that CRB’s are not portable. The home continues to promote training for staff. All new staff have an induction that is to TOPSS specifications. There are staff that have completed NVQ Level 2 and 3 and others are working towards this. The home have a training plan and staff have regular training on safe working practices. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33,37 38 The home is well run and provides an environment that is safe and the welfare of staff and service users is promoted. EVIDENCE: The home has a quality assurance system in place, which seeks the views of the service users. It presently obtains verbal feedback from family and friends and other stakeholders. A more formalised system is recommended. The home produce an annual review and development plan that is made available to the service users and CSCI. The home have been awarded Parts 1 and 2 of the East Riding Social Services Departments Quality Development Scheme. Service users have access to their records and information held about them by the home and service users have signed a document confirming this. Records are held securely. Since the previous inspection all records are now clearly signed and dated. Promenade Hotel DS0000019709.V262285.R01.S.doc Version 5.1 Page 15 Staff and management are aware of their responsibilities under health and safety legislation, and posters are displayed in appropriate staff areas. Fire drills and equipment testing was carried out appropriately and there is a fire risk assessment in place that has been updated. The inspector noted that one door was wedged open between the two lounges. Doors must not be kept open by unauthorised means. Staff regularly receives up to date training in fire awareness. Maintenance certification was available during the inspection. The electrical installations certificate is due for renewal. In the lounge a new fire has been installed it was recommended this be guarded which was done immediately. In discussions with the manager she advised that they had some problems with a service user going into the kitchen unsupervised putting themselves at risk. The manager advised a locking system is to be put in place. Risk assessments are in place but require reviewing, however, there was no risk assessment for bed rails or the homes grounds. No STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1 Standard OP25 Regulation 16,23 Requirement The radiator in the downstairs bathroom must be guarded or have a low temperature surface. The registered provider must ensure that risk assessments are in place to test water tempertures in washbasins. Where required pre-set valves which have fail safe devices are fitted locally and are regularly maintained to provide water close to 43C.(Previous requirement – not met – target date 31/07/05). The practice of holding doors open by unauthorised means must cease.
