Conditions of registration: 1. 2. 3. This home is registered for a maximum of 40 service users in the category of OP (old age, not falling within any other category). Room 24 must not be used for any service user who requires nursing care. The registered provider must, at all times, employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection. Staffing must be provided to meet the dependency needs of the service users at all times and will comply with any guidance which may be issued through the Commission for Social Care Inspection. The matter detailed in the attached schedule of requirements must be completed by the stated timescales. 13th September 2005 4. 5. Date of last inspection Brief Description of the Service: Redwalls is a care home providing nursing and personal care and accommodation for up to 40 older people. Redwalls is a large converted property the original being a grade 11 listed building and standing in its own grounds of 1.5 acres. It is situated close to the local amenities and the village of Sandiway, which is approximately five miles from Northwich town centre. The home is a three-storey building with the ground floor and first floor accommodating residents. Residents’ accommodation currently consists of 34 single and 3 double bedrooms. 31 single and 1 double bedroom have en-suite facilities. The remaining bedrooms have hand-wash basins fitted. There are two lounges and a conservatory on the ground floor and a third lounge on the first floor. The dining room is situated on the ground floor next to the kitchen area. There are separate toilets and a variety of bathrooms. There is a hairdressing salon situated on the ground floor. A nurse call system is provided in all resident areas. The residents’ primary clinical needs are due to a physical frailty and infirmity or chronic physical disability in old age. In accordance with statutory regulations the home is staffed with registered nurses, carers, and ancillary staff at all times.

This is an overview of what the inspector found during the inspection. This unannounced inspection took place on 16th February 2006. A partial tour of the building took place. The inspector spoke with five residents, members of staff and the manager. What the service does well: What has improved since the last inspection? What they could do better:
The care records of the residents need to be improved by plans of care being devised for the management of all of their identified needs/problems. There were signs of wear and tear on the paintwork within the home, which needs to be addressed. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 & 10 There were care plans in place to address most of the residents needs; however there is a risk that their needs will not be met properly because not all of them showed what the individual needs were and how these should be met. EVIDENCE: Residents said that staff look after them well. The documentation relating to residents care plans has changed since the last inspection. A standard pre-printed care plan is used, however care must be taken to ensure that individual plans of care are in place, which address the residents specific needs. In one of the care files examined, the daily record noted that the resident suffered from a skin beak to her sacral area, however there was no plan of care in place to address this issue. The nutritional risk assessment identified that the resident was at risk and required assistance at all times with eating and drinking, there was no plan of care in place to address this issue.

Although there was a risk assessment identifying that residents family wished bedside rails to be used there was no risk assessment to establish whether rails were suitable for the resident. There was evidence in the care records to indicate that staff monitor residents health and where necessary seek the advice of a specialist for example the GP or dietician. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13,14, &15 Visiting can take place at any reasonable time so that residents can keep in touch with their relatives and friends. Meals are varied and balanced providing adequate nutrition for residents to keep them healthy. EVIDENCE: Residents spoken with said that they can receive visitors at any reasonable time. They can suit themselves and choose what they want to do in the home. The priest visits weekly and the vicar monthly. Mixed opinions were given about the food. Some residents said that they didn’t like the food, however they confirmed that there is a choice of meals at lunch and teatime. One resident said she liked the food and was offered a choice. Three of the residents said they were not offered any snack with the morning cup of tea. Staff giving out morning teas said that biscuits or fruit was not offered at that time, as residents may not eat their lunch. Following discussion the manager agreed to address this issue immediately and the provider

The menus were varied and contained a good balanced diet. The chef and manager said that the menus are due to be changed shortly. Residents’ views on the menus are not formally obtained. Fresh meat was available in the fridge. This is obtained from the local butcher and was of a good quality. There was plenty of fresh fruit stored in the fridge. Lunch on the day of the inspection consisted of home made tomato soup or melon, cottage pie or grilled salmon, cauliflower, green beans, new potatoes and mash. Rice pudding was offered for dessert. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Information about the complaints process for the home is readily available so residents and their relatives know how to make complaints and who to make them to. EVIDENCE: There is a clear complaints procedure, which identifies the action to be taken in the event of a complaint. Residents knew who to go to if they wished to make a complaint. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 26. Some of the paintwork needs to be attended to so that residents are living in a pleasant and safe environment. The kitchen floor has been renewed since the last inspection so improving the facilities. EVIDENCE: Residents said they were happy with the environment and found it a pleasant place to live. They said they were looking forward to better weather when they could use the outside facilities more. There were signs of wear and tear on the paintwork within the home. This needs to be addressed. The standard of furnishings was satisfactory. One of the bathrooms and several bedrooms have been redecorated since the last inspection. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 There were plenty of staff on duty so that the needs of the residents could be met. EVIDENCE: All of the residents said that there were enough staff on duty to meet their needs. Comments such as ‘staff are delightful’ ‘good staff ‘ ‘all of the staff are great’ were made. Staff were seen to interact well with residents, a happy and relaxed atmosphere appeared to exist. There have been no new staff recruited from abroad since the last inspection, however the manager said that should they recruit someone from abroad the company will ensure that a police check from their home country will be obtained. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35 & 38 The manager is experienced and qualified so the home is run well. Residents or their relatives manage their own money. EVIDENCE: The manager has a lot of experience of managing care homes. She is registered with the Commission for Social Care Inspection. Residents meetings are currently not held in the home. These could provide a forum for residents to influence what happens in the home. Staff at the home do not manage any residents money. Residents’ health and safety is maintained. Wheelchairs are stored by the wall in the corridor leading to the side door. Care needs to be taken to ensure that

No STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. RECOMMENDATIONS These recommendations relate to National Minimum Standards and are seen as good practice for the Registered Provider/s to consider carrying out.
