Conditions of registration: Date of last inspection 7th June 2006 Brief Description of the Service: Ridgemede is located in a quiet residential area of Bishops Waltham, with local amenities close by. The home is set in pleasant well maintained grounds and accommodation is arranged on two levels, in twenty-eight single and four shared rooms, all with en suite facilities. Communal areas include a large entrance foyer with comfortable seating, a quiet lounge overlooking the rear gardens, a comfortable sitting room, and separate dining room overlooking the gardens. Additional seating is provided in the grounds. The home is registered to accommodate 36 older people, including those experiencing dementia. Up to four people with a physical disability can be accommodated. The manager reported during the site visit that the weekly fees for a place in the home range from £400 to £550. This is an overview of what the inspector found during the inspection. The evidence used to write this report was gained from a review of the information the provider sent to the Commission for Social Care Inspection (CSCI) since the last visit. This information included an annual quality assurance assessment, incident reports and comment cards from thirteen people who live in the home and five relatives. A site visit to the home was made on 28 August 2007. During the site visit the inspector spoke with several residents and a visiting relative and observed the interactions between residents and staff. The inspector also spoke with the manager and members of staff on duty. The communal areas of the building were viewed and documents relating to the running of the home were inspected during the visit. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 3 and 6. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has good systems to assess the needs of potential residents, which provides assurance to people that their needs can be met. EVIDENCE: The records of six people who live in the home were inspected during the visit. All six people had a needs assessment that was completed prior to them moving into the home. This assessment included their physical, psychological and spiritual needs and was completed by the manager. An additional nutritional assessment has also been completed for residents to assess dietary needs. Residents and relatives spoken with said that they were reassured that the home would be able to meet their needs prior to moving in. All thirteen people who completed a survey for people who live in the home said they

The manager reported that the home does not provide intermediate care, therefore standard six is not applicable. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 7, 8, 9 and 10. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The details recorded in care plans, support to access health services and the way staff support residents ensures that they are treated with dignity and respect and their needs are met. EVIDENCE: The care plans of six people who live in the home were inspected during the visit. These documents set out how their assessed needs should be met. Since the last inspection the manager has ensured that care plans are reviewed every month. Some of the plans seen have been amended where the person’s needs have changed. All of the care plans contained risk assessments, which include actions that should be taken to minimise the identified hazards. All residents are registered with a local GP practice. GPs visit the home when requested by the staff. People’s records showed that they were supported to

People spoken with said they were able to see their doctor when they need to. Medication is securely stored in cabinets and a locked trolley and is supplied in a monitored dosage system. The administration records had been fully completed. One of the people living in the home currently administers their own medication, following an assessment of their ability to do this and consultation with their GP. All staff administering medication have received training. Records were available of medication that had been returned to the pharmacist to be destroyed. People spoken with said that staff treat them well and provide support in a manner that maintains their dignity. All thirteen people who completed a service users’ survey said they receive the care, support and medical assistance they need. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12, 13, 14 and 15. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home provides good support for people to take part in social activities and visitors are made to feel welcome. A choice of good food is provided and meal times are a relaxed, social occasion. EVIDENCE: The home organises a range of entertainment including shows, choirs, bands, games and trips out. There is also a range of books and games that people can use in the home and there is a well maintained garden that people were using during the visit. People with dementia are supported to take part in activities to meet their needs, for example one to one discussions and additional support for trips out. All thirteen people who completed a service users’ survey said there were activities arranged by the home that they could take part in. People spoken with said recent activities had been very successful, including a strawberry cream tea and a trip to Marwell Zoo. People’s interests are recorded as part of their initial assessment before moving into the home, including any spiritual or cultural needs. People are

People spoken with said their friends and relatives were able to visit at any time and were made to feel welcome. During the visit a mealtime was observed. Staff were observed providing appropriate support to people who needed help to eat and there was a relaxed and friendly atmosphere. At the time of the visit the dining room was being decorated. The dining tables had been placed in the large central lobby. People spoken with said these arrangements were suitable, although they were looking forward to having meals back in the dining room. People spoken with said that the food was good and they could order something different from the planned meal if they wanted to. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 16 and 18. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has good systems to investigate complaints and protect people from abuse. This gives residents and their representatives confidence that any concerns will be taken seriously and acted upon. EVIDENCE: The home’s complaints procedure is supplied to residents and their relatives with the service users’ guide. People spoken with said that they were aware of the home’s complaints procedure and were confident that any complaints would be taken seriously and responded to appropriately. All thirteen people who completed a service user’s survey said they were aware of the home’s complaints procedures and they know who to speak to if they are not happy. Four of the five relatives who completed a CSCI comment card said they know how to make a complaint about the home if they need to No complaints have been received by the home or CSCI since the last inspection. Staff have received training in adult protection issues and those spoken with demonstrated a good understanding of issues of abuse and action they should take if they witness or suspect abuse. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 19 and 26. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is clean, safe and well maintained, which provides a comfortable and homely environment for people. EVIDENCE: All the communal areas of the home were viewed during the visit. The home was clean throughout and people spoken with said this was always the case. All thirteen people who completed a service user’s comment card said the home was always clean and fresh. At the time of the inspection the dining room was being decorated. The maintenance manager reported that there was an ongoing programme of re-decoration to ensure the environment was kept in good order. The home has a well-maintained garden that people can access. There are wide paths for those who use a wheelchair or walking frame and people spoken with said they enjoyed spending time in the garden.

There are suitable handwashing facilities throughout the home. The environmental health department inspected the kitchen in February 2007 and there were no actions required. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 27, 28, 29 and 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staff training, robust recruitment procedures and the deployment of staff in sufficient numbers helps to ensure that people are protected and their needs are met. EVIDENCE: People who live in the home and staff spoken with during the visit said they felt there are sufficient staff on each shift to meet people’s needs. The home has a rota, which showed that there are at least five carers between 8am and 1pm, four carers between 1pm and 8.30pm and two carers between 8.30pm and 8am. As well as the carers working overnight, there is a member of staff sleeping in the home and on-call and a member of management staff who is available by phone. In addition to care staff there are ancillary staff in the kitchen and cleaning the home. The manager reported in the annual quality assurance assessment for CSCI that six of the fifteen permanent care staff have completed the National Vocational Qualification (NVQ) in care at level two or above and four are currently completing the qualification.
Ridgemede Care Limited DS0000067273.V343006.R01.S.doc Version 5.2 Page 18 The manager reported in the annual quality assurance assessment for CSCI that everyone who has worked at the home in the last year has had satisfactory pre-employment checks. The recruitment records of five members of staff were inspected during the visit. These were found to contain all of the required checks, including a Criminal Records Bureau disclosure and written references. Staff undertake an induction when they start work and other courses, including fire safety, moving and handling, first aid, food hygiene, adult protection, dementia care, risk assessment, medication and infection control. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 31, 33, 35 and 38. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is well managed and has good systems to keep residents and staff safe, monitor the performance of the home and plan future improvements to the service. EVIDENCE: The registered manager is a general and mental health nurse and has been qualified for 30 years. She has undertaken further qualifications, including a teaching certificate and cognitive and behavioural studies and keeps her skills up to date with regular training and research. Staff and residents spoken with said the manager was good at her job.

Regular meetings are held for people who live in the home, which enables them to comment on how the home is running. The manager recently sent out questionnaires to people who live in the home, their relatives and other professionals involved in their care, such as GPs and district nurses. The manager has employed a member of staff to complete regular audits of the home’s performance. The findings from these checks and surveys are used to plan improvements to the home. The manager reported in the annual quality assurance assessment for CSCI that the home does not hold any money for residents or operate any bank accounts on their behalf. A record is kept of valuable items that people bring into the home. The manager reported in the annual quality assurance assessment for CSCI that that the home’s electric, gas, hoists, heating and fire systems have all been regularly serviced and maintained. Records relating to fire fighting equipment and alarms and servicing of the gas system were sampled during the visit and found to confirm the manager’s report.
