The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Verbal comments received from the four residents seen on the day of the inspection were limited by their learning disability and level of understanding. One resident when asked whether he liked living at the home grinned and smiled, whilst another danced to the music playing on his stereo at no time did residents show signs of distress or anxiety, which could suggest that no problems were evident and that the residents did in fact enjoy living at the home. The home is furnished to a high standard and staff members have made great efforts personalise individual bedrooms in a style specific to the resident. The care planning processes used within the home follow a person centred planning model, which are both thorough and comprehensive. Staff members were also observed to have a good rapport with the residents and encouraged them to do things for themselves thereby promoting their independence and self help skills.

What has improved since the last inspection?

Since the last inspection the home has undergone some refurbishment particularly in the kitchen areas. New units and appliances have been professionally installed. As a result there has been an increase in the amount of work surfaces and cupboard space.

What the care home could do better:

The homes` owners Community Integrated Care have developed a quality assurance system where feedback is reported about the organisation`s care provision. However there was no evidence to show that information is available specifically about the care provision within Rosedale/Rosewood.

It is a modern purpose built bugalow which provides single rooms for all residents, each bedroom has its own wash handbasin. The home is divided into two units, each with three bedrooms, kitchen, lounge, dining room and toilet with bathing facilities. The home has a good sized garden to the rear and is situated close to bus routes which allow easy access to the town centre of Hartlepool. There are also a range of shops near to the home which sell more convenience items. This un-announced inspection took place on Monday 20th June 2005 and was carried out as part of the annual inspection programme. The inspection lasted for approximately 4.5 hours during which, time was spent talking with four of the residents, three members of staff and the manager. The communal areas of the building were looked at in addition to four of the residents’ bedrooms. A number of records were also examined. No visitors were spoken with at the time of this inspection. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 and 5. Residents are only admitted to the home on the basis of having received a full assessment of their needs and aspirations carried out by a person trained to do so. This helps to identify needs prior to taking up residence within the home. Each resident has an up to date individual contract / statement of terms and conditions, which clearly states the roles and responsibilities of both the resident and the provider. This helps ensure that residents have clear information about the home and the costs of living there. EVIDENCE: The manager reported that the last admission to the home took place a number of years ago and she believed comprehensive assessments of need had been undertaken prior to any admissions taking place. However, individual service user files examined did not contain evidence of these assessments having been undertaken. The manager did however describe the process which would now be followed should a vacancy arise within the home. This process was appropriate considering the needs of this client group. Residents spoken to at the time of the inspection were unable to give any insight into the admission process followed due to their level of understanding and learning disability. All documentation seen included cost of the placement and the residents contribution in addition to the room occupied and the responsibilities of all parties. There was evidence of resident involvement in this process or that of their next of kin / family members. JUDGEMENT – we looked at outcomes for standard(s) 6, 7 and 9. Residents’ care plans are detailed, up to date and clearly state how their needs are to be met and their independence promoted this enables staff to meet the individual care needs of all residents. Residents are supported and enabled by staff to make decisions about their lives in order to have greater control over their lifestyles. Residents’ risk assessments are both detailed and regularly reviewed. Risk assessments are therefore appropriate to individual residents’ circumstances. EVIDENCE: Residents’ files examined contained detailed care plans indicating how best to meet the needs of individuals and how to promote their independence. The information present contained comprehensive information about each resident’s long-term plans and goals as well as the shorter-term aims and action plans to meet needs. There was evidence to suggest that the plans were subject to regular review and update and are used as a working tool for staff members. The manager indicated that family members of residents are consulted about the content of care plans wherever possible and that as far as their abilities allow residents are also involved in this process. Care plans and daily routines are based on individual needs and preferences and where appropriate risk assessments have been carried out. Residents’ files examined contained comprehensive risk assessments, which had a direct link to individual care plans. Risk assessments were signed and dated and there was evidence of periodic review having been undertaken. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 15, 16 and 17. Residents’ are encouraged and supported participate in activities, which are age, peer and culturally appropriate. Residents of the home utilise community resources with the support of staff members, which helps to promote their community presence. Residents have maintained close contact with friends, relatives and other family members who support them in their placements. Residents are encouraged to participate in activities of daily living within the home. This helps them to maintain their own independence. The meals in the home are good offering both choice and variety and promote a healthier balanced diet. Rosedale/Rosewood Bungalows DB54 S21756 Rosedale Rosewood V227077 160605 Stage 2.doc Version 1.30 Page 12 EVIDENCE: Discussions with staff members and an examination of individual case files indicated that residents are enabled to participate in a range of community activities on a daily basis. On the day of inspection four of the six residents were present as two were attending local day services. One of the residents was “timetabled” to go ten-pin bowling and was observed to use the local paper shop another was encourage to become involved in household activities whilst another was listening to music in his room. The manager and staff all showed an awareness of the needs of residents and the need for them to access community resources in order to promote their community presence. Residents themselves were unable to give any examples of the way they participate in community resources due to their level of learning disability. The manager reported that all residents retain some contact with friends, family and relatives. There were a number of photographs present within the building to indicate that family contact is both promoted and maintained. A good rapport was observed between staff members and residents and routines within the home, were described as flexible by both the manager and staff members. Menus and meals within the home are regularly reviewed and changed in partnership with residents and their individual choices. An examination of the menus indicated that a balanced diet is offered. The manager and staff also indicated that on occasion take away meals are also eaten usually at weekends. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19 and 20. Personal support is offered in the home in such a way as to promote and protect residents’ privacy, dignity and independence. The physical and emotional health needs of residents are well met with evidence of good multi-disciplinary working taking place on a regular basis. The systems for the administration of medication are good with clear and comprehensive arrangements in place to ensure residents’ medication needs are met. EVIDENCE: Staff members were able to demonstrate their knowledge and understanding of residents and how the home provides care to them. Some of the residents are able to attend to their personal care needs with only the minimum of support whilst others need more assistance and help. Care plans examined contained detailed information about the needs of individuals and the manner in which these needs are to be met. A positive rapport was observed to exist between residents, staff and the homes’ manager. Residents are registered with a local GP and Dentist. The level of learning disability, which the residents have, precludes them from safely managing their own medication. Prescribed medication is retained within the home in the “Boots” blister pack system. The manager reported that she re-assesses staff members at least every six months to ensure they still follow the agreed procedures. All medication is retained within a locked cabinet within a locked cupboard. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 and 23. The systems for residents’ consultation in the home are good with a variety of evidence that indicates residents’ views are both sought and acted upon. The home has comprehensive policies and procedures for dealing with any issues of adult abuse of exploitation in order to help safeguard residents. EVIDENCE: Regular key worker meetings are held with residents, during which their views and opinions are sought. Regular Regulation 26 visits are also undertaken which also include the views of residents. Residents were unable to confirm whether they felt their views and opinions were acted upon due to the level of their learning disability. The home has good policies and procedures for dealing with any issues relating to the protection of vulnerable adults (POVA). Training records examined confirm that staff members receive appropriate training in relation to this. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24 and 30. The standard of the environment within this home is very good providing residents with an attractive and homely place to live. Cleanliness within the home is of a high standard and provides residents with a clean and hygienic place to live. EVIDENCE: The home is decorated and furnished in a domestic style and appears well maintained. Both bungalows have recently had their kitchen areas updated with the installation of a range of new units and appliances. Residents’ bedrooms have been highly personalised with individual paint schemes and furnishings based on their personal interests. The decoration has, been undertaken by staff members. The attention paid to creating an environment that meets the residents’ needs and which is personal is worth commending. The home was found to be clean and hygienic and free from any offensive odours. Service users benefit from clarity of staff roles and responsibilities. Service users are supported by competent and qualified staff. Service users are supported by an effective staff team. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32, 33, 34 and 35. Staff within the home are competent and posses a range of suitable qualifications. Residents within the home are therefore well supported. Staffing levels within the home are sufficient to meet the individual needs of all residents. The home has comprehensive policies and procedures for the recruitment of staff, which helps to maintain the safety and well being of residents. Staff within the home, receive regular and appropriate training, which helps them to meet the individual needs of residents. EVIDENCE: It was obvious from direct observation that staff members relate well to the residents of the home and work well to meet their individual needs. Staff members spoken to had a good knowledge of the needs of adults with a learning disability and specifically the needs of residents. Rosters examined indicated that three staff members are on duty at key times during the day (mornings) with four staff members on duty at other times (afternoon’s). During the evening two members of staff are on duty, one waking night the other sleeps in. The home has no specific domestic, catering or laundry staff. Care staff, are required to undertake some ancillary duties out of their allocated care hours. There is no suggestion that direct resident care is compromised as a result of this arrangement. The home has comprehensive policies and procedures for the selection and recruitment of staff, which includes the involvement of the residents themselves. Staff members’ files examined contained copies of their job descriptions and staff members spoken to were clear about their individual roles and responsibilities. All staff members follow a thorough induction process and have the opportunity to participate in on-going training. Seven staff members have achieved NVQ level 2, three are currently undertaking the course and one has yet to start this training. Staff members all have personal development plans in place and a new training matrix has recently been introduced. Service users benefit from a well run home. Service users benefit from the ethos, leadership and management approach of the home. Service users are confident their views underpin all self-monitoring, review and development by the home. Service users’ rights and best interests are safeguarded by the home’s policies and procedures. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 37, 39 and 42. The homes’ manager is committed to self-development, staff training and also has the experience necessary to ensure that the home is well run for the benefit residents. Residents’ views are sought through regular key worker meetings and where possible their views are borne in mind by the manager when considering how to improve the running of the home. Records in general including Health and Safety were up to date. The Health and Safety of visitors, residents and staff are maintained. EVIDENCE: The manager has a number of years of appropriate experience working within the care sector and has also held management positions. The manager has commenced the Registered Manager’s award and is also undertaking the NVQ level 4. It is anticipated that this course of study will be completed in the Spring of 2006. It was not possible due to the level of learning disability of residents to determine whether they were confident that their views and opinions were used to develop the home. Community Integrated Care have a quality monitoring system which reports on the organisations overall care provision however, this does not provide information specific to this home. Monthly visits and reports by the responsible individual for the home are being carried out. Records examined confirmed that as far as is reasonably practicable the health and safety of visitors, residents and staff is maintained. On the day of inspection a number of doors within the home were propped or wedged open, in particular the bedroom doors of some of the residents. The day of inspection was particularly hot and the opening of doors in this manner did assist in creating a cooling draft throughout the building for the benefit of both residents and staff members, in addition it was noted that residents themselves preferred their bedroom doors to be retained rather that swinging shut as this enabled them to observe what was happening within the home and to have interaction with staff members.
