Conditions of registration: Date of last inspection 12th July 2005 Brief Description of the Service: Rosedene Residential Home is a bright, clean and welcoming environment that provides support for 21 older people. The Home is situated in the quiet residential area of Westbrook and is overlooking the esplanade providing some sea views. The Home is adjacent to the seaside town of Margate, within walking distance of all local amenities and has ample on road parking to the front of the premises. The Home provides 24 hour staff cover, including waking night staff. There is always an identified senior member of staff on duty and /or on call. The Manager is available on call when not on duty in the Home. The health care needs of service users are met by the local primary health care team. This is an overview of what the inspector found during the inspection. This was an unannounced inspection undertaken with a focus on the well being, safety and quality of life of service users living in the home. The methods of inspecting the home included speaking to service users, management and staff, a sampling of records and a tour of the building. The first part of this inspection was conducted with the senior member of staff on duty, who displayed a thorough knowledge of service user needs and health care developments. The manager later arrived on duty and participated in this inspection. The home was found to be clean and bright with a warm and welcoming environment. Service users spoken to at this time stated their needs are met and the staff are very caring. One new service user expressed her delight at the environment and the care being delivered and explained that any needs, particularly being taken to church when required, are fully met. There is a varied menu with special diets, such as diabetes and high cholesterol, being catered for. Service users stated they enjoy the food and some eat in their rooms if they choose to do so. While staff were dealing with service users with due consideration for personal needs and in an appropriate manner, attention must be paid to ensure staffing levels are sufficient to meet needs, particularly at busy times of the day. It is noted that the proprietor would usually be in the home, but was on annual leave at this time and the manager is on call and was due on duty later in the morning. Independence and mobility are encouraged and hobbies or activities that are enjoyed are fully supported. The home continues to provide support and care in a safe, relaxed and well maintained environment. What the service does well:
The home continues to ensure service users are taken out whenever possible. When staff go shopping or use the car, service users are invited to go along.

Independence is encouraged through continued support to become mobile and also to achieve personal goals. Service users spoken to at this time, all confirmed they are supported and well cared for by the staff group. While any incidents are notified regularly, this information needs to be fuller to support both service users and staff. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3, 4 & 5 All new admissions are undertaken following a pre admission assessment to ensure the placement is appropriate and visits are encouraged and supported prior to admission into the home. EVIDENCE: Files for new admissions contain clear assessments of needs and how staff are to ensure full needs are met by the home. Visits to the home are encouraged prior to admission and this was confirmed through discussions with service users at this time. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8, 9 & 10 All healthcare needs are met and the senior member of staff expressed a good knowledge of current health matters. There is a policy in place for the dispensing of medication and medication is stored appropriately. Service users are treated appropriately with due consideration for privacy and respect. EVIDENCE: Visits by healthcare professionals are clearly recorded and specialist organisations are contacted to support service users. The senior carer on duty at this time expressed a thorough knowledge of health needs and current matters concerning individual service users. Service users are treated with respect and the right to privacy is upheld.

This was confirmed through observations and discussions with service users at this time. One service user explained that a low cholesterol diet has been advised and the staff were ensuring the appropriate diet is being served. Staff were also taking time to explain the reasoning behind specific foods being served and the way the low cholesterol diet works. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 Choice and autonomy are supported and encouraged, with choices being undertaken with support if required, providing a good quality of life. All aspects of service user needs are met by the home and family, friends or advocates are encouraged to maintain contact. Any food request is met for individuals at any mealtime. EVIDENCE: Choice and autonomy is supported and encouraged through regular meetings with service users and also through informal discussions each day. Minutes of all formal meetings are recorded. One person who has moved to the home since the last inspection stated she has been welcomed into the home and supported with all aspects of her move. Visits to church have been supported with suitable travel arrangements being made when needed. Other activities that are regularly undertaken include trips to the shops, theatre, the sea front and walks. Keep fit is offered as are bingo sessions, manicure and therapy plus car rides out if anyone would like to have a trip out. One service user explained that she enjoys typing the minutes of the service user meetings, providing valuable information and continuing to use skills that are very important to maintain. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 17 Service users know their complaints will be listened to and acted on. EVIDENCE: There is a complaint policy and procedure in place in the home. Service users spoken to at this time stated they are confident and comfortable to discuss any matters with care staff, management or the proprietor. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 23,26 The home is well maintained, clean and hygienic. EVIDENCE: The home continues to offer comfortable, well-furnished and bright rooms. Service users confirmed they are able to bring person items with them and rooms are colour co-ordinated and comfortable. Hand wash dispensers with antiseptic liquid have been installed along corridors to encourage service users to clean their hands and to ensure the control of infection. While no structural alterations have been undertaken since the last inspection, a new conservatory is planned and corridors are to be redecorated and recarpeted. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28 Staff are caring and consider the dignity of service users but staffing levels must be appropriate, particularly at busier times of the day. Training is monitored throughout the year and all training is approved and appropriate. EVIDENCE: Discussions with service users confirm that service user needs are met in an appropriate way and the inspector was told that staff are very caring. While staff were dealing with service users with due consideration for personal needs and in an appropriate manner, attention must be paid to ensure staffing levels are sufficient to meet needs, particularly at busy times of the day. There was 1 senior carer, 1 carer and a cook on duty at the time of this inspection and the manager later joined the inspection. The manager explained that the cook would answer any phone calls if the staff were busy, but there were a few service users who needed to use the bathroom and with only 2 carers on duty, it would obviously mean that some service users would have to wait for support at this time. Discussions were undertaken with the senior carer and later with the manager about staffing numbers. While staffing
Rosedene Residential Home DS0000023524.V271694.R01.S.doc Version 5.0 Page 18 is always increased when activities are planned or appointments are undertaken, any busy periods in the home must also be covered with appropriate staffing levels. The proprietor was on annual leave at the time of this inspection and would normally be in the home during the daytime and the manager was due on duty at 11am. Staff stated they feel they do manage and while this may be the case, staffing levels must be sufficient to meet all needs. Training is monitored and refreshed as required, some training that is currently booked for this year includes Adult Protection, Challenging Behaviour, First Aid, Health & Safety, Loss & Bereavement and Infection Control. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,38 The registered manager is a suitable person and service users benefit from a safe and well run home.
