To allow one named individual under the 65 years of age to reside at the home. 20th July 2005 Date of last inspection Brief Description of the Service: Roseleigh is a 50-bedded purpose built home that provides personal care for 24 older people and 26 people suffering with dementia. All of the rooms are single with an en-suite comprising of a WC and hand basin. There are spacious bathrooms in the home with equipment suitable for immobile people or for those residents’ who need assistance. The home is situated close to Middlesbrough centre and is accessible by bus or car. It is in a built up area and overlooks a greenbelt. This is an overview of what the inspector found during the inspection. This was an unannounced inspection and nearly of the National Minimum Standards inspected were met. The inspection lasted four and a half hours with one inspector. The residents and their relatives were given the opportunity to talk to the inspector. The inspector talked with six residents and three relatives about life in the home and the service they received. During this inspection discussion with residents took place, the inspector walked around the home, looked at medication policies and practises, activities, the homes complaint policy, staff records and supervision records were viewed, personal allowance records and the quality assurance programme. A tour of the home showed that it was clean, tidy and well decorated throughout. What the service does well: What has improved since the last inspection?

There is a budget available for the furniture and decoration to be on going. The manager has requested in her budget for new armchairs, as they are showing signs of wear. Residents’ views are considered when redecoration is planned particularly in their bedrooms. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9,10 Medication administration procedures ensures that the resident’s health care needs are addressed. Staff at the home successfully support service users with their social, health and personal care needs in a way that treats them with respect and promotes their rights and privacy. EVIDENCE: Currently there are no resident’s that self medicate, though residents who use inhalers and sprays keep them at hand. Lloyds pharmacy collects the prescriptions from the GP surgeries and delivers each persons medication in the dispensing boxes. Some medicines are delivered weekly and others monthly. The system meets the residents needs and is safe, it also prevents over ordering and waste. There are policies and procedures in place for the safe handling, storage and administration of medication. The storage facilities are safe and suitable for medication and the staff who administer medication have received training in the safe handling of medication with Middlesbrough College.

One resident said “the girls will do anything for you” another resident said “nothing is any trouble”. The staff were seen with the residents’ to be kind and considerate and they spoke to the residents kindly and with respect. Some residents who engaged in conversation said their privacy was always maintained and respected. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13 A planned structured and well delivered activities programme can contribute to a more interesting and stimulating lifestyle for service users. The residents’ make positive choices about how they spend their day and friends and families are encouraged and involved too. EVIDENCE: The manager is currently advertising for an activities co-ordinator to be employed in the home. The care staff participate in activities during the afternoon, however records do not reflect the activity that takes place. Modern apprentices are employed in the home and they spend time with the residents playing bingo, dominoes and manicures. The visitors can visit at any reasonable time one relative said that she is always made welcome when she visits. As a token of appreciation to the staff a visitor brought them a tin of chocolates to share between them. Three relatives I met with spoke highly of the staff. JUDGEMENT – we looked at outcomes for the following standard(s): 16 Complaints in the home are handled objectively and openly with the manager and staff encouraging service users, their friends and families to offer comment on the services that are offered. Service users are confident that any complaints made would be acted upon. EVIDENCE: Relatives confirmed that if they had any concerns they would be dealt with appropriately. The policy for handling complaints is clear and concise; it has timely responses included to inform a complainant of the process that is followed. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29 The deployment and number of staff on all shifts is appropriate to ensure that at all times service users are supported by an experienced group of staff. Record keeping in respect of new staff employed in the home is robust and provides relevant and sufficient information to ensure that service users are protected. EVIDENCE: The home is divided into 2 units “Cook” unit is on the first floor cares for people with dementia and the “Endeavour” unit provides personal care for older people. There are 4 carers during the day and evening and 2 night duty carers on Cook unit and 3 carers during the day and evening and 2 night duty carers on Endeavour unit. A senior carer is on duty at all times and a day duty carer starts the duty earlier to help at a busy time in the morning. The manager has a flexible staff team that will change duties to enhance the residents’ care. 55 of the care staff have achieved a National Vocational Qualification in care and the remainder of the staff are working towards this. Staff in housekeeping and catering also have National Vocational Qualifications. Each staff member has a personnel file that is kept in accordance with data protection. To ensure the residents are safe each employee completes an application form and explains any gaps in employment. Two references are requested by the manager, one of the references is from the previous employer. Photographic identification and birth certificates are copied and a criminal records check is sought at an enhanced level. All staff have a company handbook and a contract of employment. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35,38 Residents’ financial matters are managed appropriately. The resident’s health and safety is promoted by a well managed home. EVIDENCE: Where the home handles personal allowances for the resident, each resident has his or her own balance sheet. All transactions have 2 signatures and receipts are kept. Residents’ money is appropriately stored and accurate records are in place. The manager and the administrator check the balance of the money and the records monthly. There are also secure facilities in the home for money and valuables. The environmental health officers’ report shows they are satisfied with the standards in the kitchen. Service records for electrical systems are in place and up to date. The water is chlorinated annually to reduce the risk of Legionella bacteria growing in the water system. Hot water temperatures are thermostatically controlled and records show they are checked regularly to prevent accidental injury to the residents’. Relevant legislation relating to health & safety is adhered to. Supporting policies are in place to ensure safe working practises are adopted. Accident books are kept and are in keeping with the Data Protection Act 1998. A health and safety poster is displayed in the home and the homes induction programme is in keeping with the skills for care specifications.
