What the care home does well

Rosemary Mount continues to provide a warm, homely environment that is central to resident`s collective and individual needs. Residents are encouraged to own and propose any changes that would improve their lives there. From resident`s comments and observing staff at work, it was clear that the outcomes for residents are high with staff committed to treat residents with respect and dignity.

What has improved since the last inspection?

The appointment of the deputy manager to manager has been extremely positive. Outcomes for residents are very good and their views are taken into account when providing activities and changing menus. The appointment of the cook has proved to be an exceptional choice and the new catering assistant has increased the prised received from residents. New residents or those on respite care are well supported and comments made showed that every aspect of care has been considered.

The establishment is situated in a residential area of Worthing approximately two miles from the centre of the town and all its amenities and ½ mile from the sea front. Local transport provides access to the town and main line railway station. Accommodation is provided in thirty-three single rooms. The rooms are arranged over two main and one mezzanine floors with a lift giving access to all rooms. A lounge, quiet lounge and separate dining room provide the main communal space. The responsible person is Mr Tony Matthews who represents West Sussex Housing Society Limited, a voluntary organisation who owns the service. The newly appointed registered manager is Mrs Denise Waller. This unannounced inspection, one of two required under the Commission for Social Care Inspection was planned to observe the morning activities and lunch provided for the residents. The newly appointed manager was present and a number of care and ancillary members of staff. The manager was also providing support to a volunteer from a day centre. A visiting professional told the inspector of the good support he had received when visiting. On arrival some residents were being supported to go out whilst a number of residents were in the two lounges socialising with each other having just finished breakfast in the dining room. The residents seemed generally happy and relaxed and provided very positive comments about the care and services provided. All residents were able to give a clear account of their life at Rosemary Mount and all comments were clearly freely given and enthusiastic. The Statement of Purpose and Service Users Guide have been reviewed and have been distributed to all interested parties. These documents include information on how the home is run and how changes can be made to improve residents lives there. During the inspection, six residents were spoken with in the main lounge following a musical event and seven residents spoken with privately in their rooms. Four residents’ records were examined to see if the assessed care needs were provided and two staff records examined to ensure a consistent and high recruitment was maintained. The transition of the deputy to manager has been very smooth and it was clear that the support and leadership always present has continued. From the resident’s comments and from observing the staff at work, it was clear that there is a high commitment to treating residents with respect and dignity and that outcomes for residents are the main focus of care and services. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,4,5. All residents had been assessed before moving into the home. The staff at the home are meeting the residents identified needs. Relatives were given enough information to help them decide the home would be suitable. EVIDENCE: The Statement of Purpose and Service Users Guide has been updated and provided to all residents, representatives and the Commission for Social Care Inspection for their records. Four care plans, newly presented to provide clarity of assessed needs, were examined and it was clear residents are being assessed prior to admittance to ensure the home would be able to meet their needs. Relevant risk assessments had been reviewed and staff are well informed about the care required for each resident. Records were being updated accordingly. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 All residents had an individual care plan set out for staff to follow and staff members are meeting the health care needs of the residents in an appropriate and respectful manner. EVIDENCE: Four care plans gave clear information of the care needed with good risk assessments to inform staff how to minimise risks. The Accident Forms showed comprehensive information and outcomes were being monitored according the “Slips, Trips and Falls” guidelines. The inspector observed that this is providing good factual information for prevention or awareness. Medication sheets were not fully completed and a recent incident involving medication has prompted the inspector to arrange for the pharmacy inspector to examined training and related records more fully and provide advice. A visiting professional confirmed that full support had been given to assist him to provide a good service. Referrals to the Primary Health Care Team appeared appropriate with good care being provided under their direction. Residents comments included “staff are wonderful”, “kind”, “good” and “we couldn’t do better”. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15. Activities are suited to the conditions and dependency levels of each of the current residents. Visiting is positively encouraged. Residents are served meals that are nutritious and appetising and these were highly praised. EVIDENCE: Activities are based on ability with staff assisting with these. The number and choice of activities is high and was well complimented on by the residents. The inspector observed a well attended musical event being provided in the morning. Although Rosemary Mount accommodates up to thirty-three residents, staff still seemed able to spend individual time with them. It was clear from the visitor’s book that visitors come every day at all times of the day and evening. The residents told the inspector that they really enjoyed their food. The new cook has recently had a new catering assistant and from resident’s comments, meals continue to be of a high standard. The breakfast ranges from cooked to continental and residents can choose from a hot supper dish or a variety of cold options. The inspector noted the lunch provided an excellent choice and was well presented to residents on tables for four to six persons to maintain a homely atmosphere. The food is all home cooked with fresh vegetables. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,17,18 Residents are confident that any complaints they may have are taken seriously and acted upon appropriately. Staff training in adult protection procedures is up-to-date so staff are equipped to protect residents from abuse. EVIDENCE: The home has a complaints procedure displayed and included in the Statement of Purpose and Service Users Guide. One resident said she knew who to complain to, but had no occasion to do so. The West Sussex Multi Agency guideline was available in the home and updated training has been provided to ensure staff members are aware of how to protect residents from abuse. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,22,23,24,25,26. The indoor areas used by residents are clean, safe and homely with good access to the front, rear and side gardens. The resident’s rooms are suitable for their needs and are homely. EVIDENCE: Residents are encouraged to move around the home as they wish and have access to the all the communal areas which are comfortably furnished. The dining room tables accommodate a maximum of six residents giving it a homely atmosphere. There is a passenger lift for those residents whose rooms are upstairs. The garden areas have been designed to assist residents to walk independently or use a wheelchair and are furnished with occasional garden furniture. There are enough toilets and assisted baths to meet the needs of residents and thermostatic valves protect residents from scalding water temperatures. Radiators are guarded and the home was clean and hygienic. Resident’s rooms were visited and were homely and comfortably furnished with their own Rosemary Mount Residential Home DS0000014691.V263927.R01.S.doc Version 5.0 Page 14 possessions around them. Training records showed that staff members have received training in fire safety procedures and fire risk assessments were in place. One empty resident’s room was being redecorated at the time of the inspection and all individual and communal areas rooms were clean and smelt fresh. However, in particular, the first floor corridor floor covering required attention. It is badly stained and was raised in the previous report. A recommendation has been made for this to be attended to. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,30. The duty rotas did indicate that enough staff are on duty over the 24 hours period to meet needs and that recruitment processes are robust and ensure residents are protected. EVIDENCE: The inspector observed day staff as they assisted residents during the morning and before lunch. The rota confirmed the staffing arrangements for the home. The numbers and skill mix of staff was appropriate to meet their needs. Three staff spoken with said they were happy working at the home and felt well supported by the manager. From the two staffing records seen, recruitment records were consistent and staff received good induction, supervision and training. Staff have access to all job descriptions giving good information about the structure of working roles throughout the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,36,38. The newly appointed registered manager is well qualified and experienced to manage the home. EVIDENCE: Since the previous inspection, Mrs Denise Waller has been appointed as manager. Mrs Waller is well qualified having obtained the Registered Managers Award and has been employed at Rosemary Mount for a number of years, the latter years as deputy. As such, Mrs Waller is well experienced to manage the home. Staff are clearly delighted at her appointment and were unanimous in their praise of her leadership style. They said that they felt supported to carry out their roles. Recruitment, induction and supervision records confirm that the resident’s best interests were safeguarded. All communal rooms and rooms meet the National Minimum Standards giving resident’s sufficient space for them to have personal possessions or necessary equipment to support their care needs and move around their rooms safely.
