Conditions of registration: To include two named people under 65 years of age 19 beds to be used flexibly between both categories Date of last inspection 7 February 2005 Brief Description of the Service: Rosewood Lodge is a care home providing personal care and accommodation for 19 older people. It was first registered in 1995. The home is situated in a residential area in the London Borough of Redbridge, overlooking a park. It is in within easy reach of the town centre and other local facilities, such as small shops, pubs, a post office and other amenities. All bedrooms are situated on the ground and first floors, which are served by a lift and stairs. There are three lounges and a separate dining area downstairs plus a garden, for the service users enjoyment. All the bedrooms occupied by service users are single and four have en-suite facilities. Various in-house activities are provided, such as exercise class, art and craft, animal shows, floor basket ball, bingo and outings, including cinema trips, pub lunches, walks in the park, theatre trips, shopping and weekly visits to church. The service users are able to have their meals flexibly, or in their rooms if they are unwell. A hairdresser visits the home weekly, as well as a reflexologist and a manicurist. Staff showed a caring and sensitive atttitude towards the service users, which was confirmed by the service users spoken to on the day of the inspection. This is an overview of what the inspector found during the inspection. The inspection was unannounced and started at 12.30pm. It took place over three and a half hours, during the afternoon. The inspector spoke to several residents, the cook, the hairdresser and three members of staff plus the manager. A tour of the ground floor of the home took place. A variety of records, including care plans, staff files and health & safety documents were looked at. What the service does well:
Residents feel at home and relaxed at Rosewood Lodge because staff pay close attention to meeting their individual needs. Residents said that they enjoyed living at Rosewood Lodge and were very happy with the quality of care they received. One of the resident said, “The staff do everything for you, you don’t need to worry about anything in here.” The residents said that they felt able to talk to the manager or the staff if they had any concerns or worries. They also said that they are informed about issues affecting the home through residents’ meetings and individual consultations. All necessary healthcare services are accessed for the residents in order to meet their assessed and specialist needs. A member of staff takes responsibility for arranging a weekly programme of activities, which is posted on the notice board in the hallway. One resident said that, “There is always something to do here and I like going to the hairdresser.” The inspector saw the activities and outings leaflet, which listed a wide variety of activities the service users can participate in. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3, 4 and 5 When people move into the home, the manager works out what they want and need, so that the home is sure it can meet these wishes and needs. This enables residents to be confident that Rosewood Lodge is an appropriate home for them. Residents and their representatives are invited to visit the home, so that they know the home they enter will have the knowledge to meet their needs. EVIDENCE: Evidence was seen that prospective residents and their relatives or representatives are offered the opportunity to visit the home to assess its suitability. The admissions procedure seen for the most recently admitted residents showed that it is sufficient to guide staff on the actions to be taken to make sure that the new residents’ needs will be properly assessed and planned for. The residents spoken to said that they enjoyed living at the home and thought that their needs were being met. One resident, who had recently moved in

Her case file contained an assessment by her social worker as well as the manager, of her needs and wishes. The manager had put together a plan of care, setting out how this person’s needs and wishes would be met by the home. She had also completed a risk assessment to ensure that any risks identified could be managed by the home. Daily records show that staff follow the care plans and how they meet the service users’ needs. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9 and 10 Residents benefit from the close attention paid by staff at Rosewood to meeting their health needs. The individual plans of care for each resident would be improved by increasing further the involvement of residents and their relatives, so that these plans reflect as accurately as possible the needs and wishes of the residents themselves. The medication policy and procedure is followed by staff and none of the residents are responsible for their own medication. EVIDENCE: Feedback from the residents was positive about the commitment of the home to keep residents as well as possible. One resident said that the doctor is called whenever she feels unwell and records confirmed that residents are seen by dentists, opticians, chiropodists, district nurses and doctors. At the time of the inspection, a specialist medical practitioner had been called to the home who was visiting a service user and the manager was closely involved in dealing with this.

The records for three residents were looked at and indicated that for each one there was a current plan of care which set out their needs and how they were to be met by the home, with evidence that these were being reviewed regularly. In addition, each file contained an assessment of any area where there was any risk to the resident and how these risks were to be dealt with and reduced as far as possible by staff. The records for the most recently admitted resident were yet to be fully completed. In order that care plans reflect the views and wishes of residents and their relatives/representatives, it is important that they are involved as much as possible in this process. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 and 15 Social activities and meals are both well managed, creative and provide daily variation and interest for the service users living in the home. Rosewood Lodge is particularly good at being able to meet the cultural and religious needs of people from various ethnic backgrounds. Residents have nice, well-balanced meals in comfortable surroundings with their individual preferences catered for. EVIDENCE: A staff member takes responsibility for arranging activities and outings and there is a weekly programme of activities posted on a notice board in the hallway at the entrance. Several service users said that they appreciated and enjoyed the activities offered, to stop them getting bored and give them something to do. Many residents said that if they did not want to join in some of the activities, then staff respected this. Two residents have joined an art club at a local church and are taken by a member of staff to the church. The ladies said they enjoyed the activities very much. There were several pictures on the wall completed by them. The residents also enjoy the animal show, which includes

The residents said that they look forward to this visit. Photographic evidence was seen on the walls in the corridors of various outings, dinner, pub lunches and celebrations enjoyed by the residents in the home. Visiting times are very flexible and residents confirmed that they are able to receive visitors in one of the lounges or in their own bedrooms, as they wish. The inspector had an opportunity to speak to the cook, who said he knows what each resident likes to eat and knows which residents need a special diet; for example, if they are diabetic. Many of the residents said how much they enjoyed the food in the home. A couple of Caribbean residents also confirmed that they enjoy West Indian dishes and this is often provided and enjoyed by them as well as other service users. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 17 and 18 The home works hard at sorting out problems before they become complaints and makes sure that residents and relatives feel confident that their complaints and concerns will be listened to and acted upon. Staff in the home confirmed that they have received training in Adult Protection/abuse awareness, to ensure a proper response for reporting any suspected or witnessed abuse. EVIDENCE: The home has policies and procedures in place in relation to reporting and investigating complaints. Residents said that they felt able to make complaints and raise issues if they needed to. The complaints’ log was checked, which showed that any issues raised by residents are promptly and appropriately dealt with by the manager. The book also lists compliments and the home has received several compliments from relatives and friends, as well as residents about the quality of care provided/received. A policy and procedure for responding to allegations of abuse is available and staff members spoken to confirmed that they have received training regarding this. Staff spoken to confirmed that they have an awareness of the procedure to follow if an incident is reported to them. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 22, 23, 24, 25 and 26. The standard of the environment within the home is good, providing service users with a clean, attractive, safe and comfortable place in which to live. Specialist equipment is obtained for service users who require it. The home is clean, pleasant and hygienic. EVIDENCE: The completion of a major extension to the home in the past year has resulted in a more spacious and attractive lounge area and a separate hairdressing room, which has improved facilities for residents. The standard of décor, furnishings and fittings in the home are maintained to a comfortable standard. There is an ongoing programme of refurbishment and redecoration. On the day of the inspection, some of the residents were sitting outside as it was a warm day. The residents spoken to said that they were very pleased with their individual bedrooms. There were no unpleasant odours in the home. Throughout the inspection, the home was found to be clean and hygienic. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29 and 30 Residents benefit from a committed and experienced team of staff at the home, who have the skills and training to meet their needs. Residents are protected by the home’s recruitment policy and practices. Staffing levels are satisfactory and there are sufficient staff on duty to meet the individual assessed needs of the service users. EVIDENCE: A good deal of positive feedback about the staff at the home was received from the residents. A typical comment being “The staff are very kind.” The home has a relatively stable workforce and, in discussion with the staff, it was evident that they fully support the main aims and values of the home. A number of staff have worked there for many years and have built up a good knowledge and understanding of the needs of the residents. Staffing levels for both care and domestic staff were adequate to ensure that residents’ needs continue to be met. Staff files showed that staff had done training in essential areas, such as food hygiene, Health & Safety, administering medication, dementia awareness and first aid. More than 50 of the staff have achieved NVQ level 2, with some having completed NVQ level 3. A comprehensive training programme is in

Newly appointed staff do receive induction training, however, the manager needs to start providing this training to staff using the induction training packs acquired. See recommendation no. 2. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33 and 38 Residents benefit from living at Rosewood Lodge because the home is run in the best interests of the residents. The home is good at making sure the residents are kept safe and secure whilst living at Rosewood Lodge. EVIDENCE: The manager and staff work together to achieve high standards for the home. Feedback from residents was positive about the way in which the home is run. Some of the compliments received by the manager indicate the level of satisfaction that relatives have expressed, congratulating the manager and staff for providing good levels of care. The recently completed extension represents a significant commitment by the proprietor to improve the quality of the facilities for residents at the home.
