Conditions of registration: 1. The home is registered for a maximum of 27 service users to include: up to 10 service users in the category of OP and up to 17 service users in the category of DE. Date of last inspection 09/11/04 Brief Description of the Service: The Rossendale Nursing Home is registered with the Commission for Social Care Inspection to provide personal and nursing care for 27 service users of either sex aged 65 years and over. Care is offered to those with physical/medical needs and those with dementia (up to 17 service users). The Rossendale Nursing Home is a detached Victorian property, which retains many of its original features. Three en-suite bedrooms were added to the home in 2004.It is situated in a residential area of Ansdell Lytham St Anne’s, overlooking Grannies Bay. The home is within easy reach of shops and local amenities. It is situated on a bus route and within five minutes walking distance of Ansdell railway station. Service users are encouraged to retain their links in the community and every effort is given to ensuring that relationships, hobbies and interests are pursued. Relatives, friends and visitors are made welcome at the home at any time. Service users accommodation is provided in nineteen single rooms and four double rooms, nine of which have en-suite facilities. Communal areas are provided in two lounges, a dining room and a conservatory. The conservatory is designated a smoking area. There are attractive gardens to the front and side of the home which are accessible to service users, and there are car parking facilities at the rear of the home. This is an overview of what the inspector found during the inspection. This inspection was announced and started at 9.30am and took place over 5 hours. The Inspector spoke to six residents, two visitors and the manager. Comment cards were received from four residents, one relative; one GP and one Social Care professional were received back. Staff and care records were also examined. An activity was observed and a partial tour of the premises was undertaken. What the service does well: What has improved since the last inspection?
Individual care plan records have improved providing better information for staff. Training, monitoring and recording of health and safety issues in the home ensuring the health, safety and welfare of residents is promoted. Time is generally set aside each afternoon for staff to discuss issues which staff feel is ensuring a more cohesive approach. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 and 6 The admission procedure ensures an adequate assessment is carried out. Care needs are clearly documented and generally met. EVIDENCE: The records of three residents one of whom was recently admitted had full assessment information. Staff members confirmed they had access to this information and could describe in detail the care needs of residents. Catering staff had been informed about residents who had special dietary needs. One resident who had special dietary needs confirmed these were being met. Residents confirmed they had been involved in their assessment and were happy that their needs were being met by the home. Four residents returned comment cards and said they felt well cared for. People are not admitted to The Rossendale solely for intermediate care. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 8 Promotion of health is taken seriously. Resident’s welfare is closely monitored and health needs are met. EVIDENCE: Individual care records are kept for each resident with a plan of care setting out in detail the action that needs to be taken by care staff to ensure all aspects of health, personal and social care needs of the residents were met. Significant events had been recorded on appropriate documentation and daily entries made setting out the care given. Comment cards received from a GP and a Social Care Professional stated that they felt health care needs were being met. The care plan for one resident showed specific health care requirements and the action taken by the staff to ensure the best care was given which included a visit from a specialist nurse to treat the problem. When spoken to, the resident was very grateful to the staff at the home for the care and support they were providing. Her visitor said, “ She looks 100 better since she came in here”. Evidence of the residents and family involvement was seen. Residents who were being cared for in bed had been provided with a special mattress that was suitable for the relief of pressure and prevention of pressure sores. Discussion with staff confirmed they were aware of the needs of both residents and the level of care that needed to be provided. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14 and 15 Leisure and recreational activities are provided. Contact with relatives/friends and the local community is encouraged. Meals are varied and nutritionally well balanced. EVIDENCE: Residents spoken to confirmed they enjoyed the food provided by the home. Residents who required a special diet or had preferences confirmed these were being accommodated. Meal times were served in a relaxed and unhurried manner with staff seen providing discreet assistance to residents who required help with eating. The menus were inspected and found to provide a varied and balanced diet using fresh produce and home baking. The cook was able to confirm he had information about residents with special diets and personal preferences. Residents can exercise choice over their chosen lifestyle this was evident throughout the visit. Residents took part in a game of carpet skittles during the visit .One lady spoken to said, she enjoyed this and liked to win. The home organises other indoor activities, which seem to be enjoyed by residents. Consideration to the high percentage of residents with dementia is taken into consideration when activities are planned.

Staff try to take people out for a walk by the lake as regularly as possible. Four residents returned comment cards two felt activities were suitable, one felt activities were not suitable and one person felt they were sometimes suitable. On speaking to residents the inspector found that many of the residents like to spend time alone and liked their “own space”. The home has a visitor’s policy and encourages visitors to become involved in care in the home where possible. Clergy visit on a weekly basis to meet religious needs. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 29 Recruitment policies have not been consistently followed resulting in service users receiving care from staff members who have not been properly vetted. This potentially leaves people who use the service at risk. EVIDENCE: There has been moderate staff turnover of the staff in the last 12 months. The staff files indicated that the home had not carried out all the necessary recruitment checks to ensure protection of the residents. Criminal Record Checks had not been requested for two staff members. The manager was informed that all staff including oversees staff must be referred to The Criminal Records Bureau for clearance prior to employment. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31 and 38 The leadership from the manager is good and promotes a consistency of care. The home provides staff with a good awareness of health and safety via training which impacts on the care given to residents. EVIDENCE: The registered provider is the matron for the home and has completed her Registered Managers Award. Residents at the home made positive comments about the manager being available, approachable and willing to listen to their opinions. Staff spoken to confirmed that time is set-aside in the afternoon for staff to get together and discuss issues. Inspection of records indicated that regular fire drills take place and that fire equipment is regularly serviced. Fire training is covered within induction alongside other mandatory areas. Staff spoken to confirmed this. Information provided in the questionnaire completed by the manager stated that all safety equipment is regularly serviced.

Members of the management team have areas of responsibility for Health and Safety and complete monitoring documentation on a regular basis. Each employee is issued with a Health and Safety handbook.
