This inspection was carried out on 13th February 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

One of the major strengths of Roundstone is that it is situated in the centre of the local community. This means that the service users are able to continue with their interests and meetings without too much disruption. It also means that they can walk to the local shops and meet their friends for coffee, as they would have done when at home. The proprietors work alongside the staff and have a good working knowledge of the service users. This enables them to provide the appropriate support. They have also developed good relationships with the families and one relative said `I have confidence in the care provided in the home and don`t know what I would do without it`.

The proprietors must ensure their recruitment process is thorough and for all staff they must obtain two references and a POVAFIRST and/or a Criminal Records Bureau disclosure prior to their deployment in the home. It is owned by Mr Ray and Mrs Margaret Koralewski and managed by Mrs Koralewski. The houses have been adapted to provide for a maximum of 13 service users who were accommodated by virtue of age or infirmity. The registered providers can also offer accommodation on a short stay basis. The premises occupy three floors, the top one being the private accommodation of the registered providers and their family. The home has 11 single and one shared bedrooms located on its two lower floors. Two single and the shared bedroom have the advantage of en-suite facilities. There is no passenger lift but a stair lift is fitted to one staircase. The registered providers and their staff provide personal care, an in-house catering service, an in-house laundry service and a cleaning and domestic service. Staffing cover to the home is maintained throughout the 24-hour period each day. Leisure and recreational facilities are offered by the staff in-house and are also available at a number of outside locations. Many of the service users are able to go out with the assistance of staff or relatives. This inspection including preparation time took place over 8 hours. A tour of the building was conducted. A number of the service users records, staff records and records about the management of the home were inspected. Service users, staff on duty and a relative were spoken with. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8, 9, and 10 The service users health needs are met and they are treated with respect and dignity. EVIDENCE: The service users files contained information about their health needs and details of GP visits and district nurses are recorded. The service users spoken with said that they could see their GP on request and that any visits made either to the home or the surgery were in private. The files also contained information about other health services accessed by the service users such as chiropody, dental, optician, and district nursing services. One service user was on pressure relieving equipment and the home is fully supported by the local surgery. Staff were observed treating the service users with respect at all times. The service users said that staff always treat them with respect and try to preserve their dignity at all times. There is a medication policy in place and all staff that administer the medication have received appropriate training. The storage, dispensing and documentation of the medication was in line with the policy and procedure of the home. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15. The service users are able to keep their own routine within the home and join in with social activities. The service users have a varied and balanced diet. EVIDENCE: The service users are encouraged to continue to attend their external activities and several still go out to the local church, Mothers Union or into town. One service user said that if she were unable to get out to any of her events, the staff would assist her as much as they were able. All of the service users spoken with said that they could dictate their own routine in the home if they wanted to and were able to make their own decisions about whether or not to join in with any activities available. A visitor spoken with said that they are always made welcome and there are no restrictions as to when they visit. The service users confirmed this was the case. The visitor also said that the managers were good at keeping them informed about events affecting their relative or generally what’s going on in the home. A visitors’ policy is in place and all visitors sign a visitor’s book on entering the home. They confirmed that choices are available at each meal. The meal observed was relaxed and the staff assisted service users appropriately where necessary. The menu was examined and appeared to provide a balanced and nutritious diet. The cook explained that local fresh produce is used and they are able to vary the menu to include seasonal foods. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Experienced staff that receive regular training support the service users. Staff are employed following a thorough recruitment process. EVIDENCE: The staffing levels provided are sufficient to meet the needs of the service users. There are always 2 staff and sometimes 3 on during the daytime, 1 night staff and the proprietors live on the premises and provide an on-call service. The service users spoken to said that the staff were ‘first class’ and ‘very nice’. They said that the staff were always helpful and they appreciated the fact that they have time to spend with them. All prospective staff complete an application form and this provides an employment history and the names of two referees. Three staff files were seen and whilst two of them contained all the information as required by the Care Homes Regulations 2001 one did not contain any references or evidence of a POVAFIRST and/or a Criminal Records Bureau disclosure. Any member of staff deployed in the home must have a POVAFIRST and/or a Criminal Records Bureau disclosure prior to the commencement of their employment. The proprietors are working towards the 50 ratio of staff with a National Vocational Qualification level 2 in care. All staff are provided with the statutory required training. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 and 38. The service users live in a well managed home. Policies and procedures are in place to ensure the service users financial interests are safeguarded. The health and safety of the service users and staff is promoted. EVIDENCE: Mrs Koralewski is the registered manager and will complete her National Vocational Qualification level 4 in management and care this year. She also has a variety of experiences as a manager in caring environments. Service users spoken with said that she was always approachable and they felt they could talk to her if they had any problems. A staff member also said that she had been very supportive and helped her settle in to a new job. A quality assurance programme has been set up but not implemented. The personal allowance for several service users is held in the office and the records were checked and found to tally with the cash held. Receipts are obtained for any purchases made. Roundstone Care Residential Home DS0000055727.V276764.R01.S.doc Version 5.1 Page 16 The staff spoken with during the inspection said that they had received training in, back care, first aid, food hygiene, COSHH, and fire training. The equipment used in the home is serviced at the prescribed intervals. Health and safety notices were displayed around the building. Accidents are properly recorded and where necessary, reported to the Commission for Social Care Inspection. Information from the accident records is used in the care planning process. All staff can access Induction and Foundation training, this provides them with basic skills required to undertake the role of a care assistant.
