Conditions of registration: Date of last inspection 4 August 2005 Brief Description of the Service: Ryde Cottage is a residential home providing care and accommodation for up to seven younger adults with learning disabilities. It is managed by Mr David Robinson on behalf of the proprietors Mr and Mrs Clewley. The home is approached via an extended private drive off the main Binstead to Ryde road, which provides easy access to Ryde and bus routes to other parts of the Island. The house is located within the extensive private grounds of Ryde House with access to a private beach. Accommodation within the cottage is arranged on two floors providing seven single bedrooms and a range of communal facilities and bathrooms. There is a patio area at the rear with seating available for use by the residents. The home does not have a lift. Ryde Cottage DS0000012529.V249076.R01.S.doc Version 5.0 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This was the second of two inspections for the year at Ryde Cottage and took place unannounced over 3 hrs. Core standards not assessed on this occasion had been assessed at the last inspection. During the middle and late afternoon residents returned from day services. There was an opportunity to speak with them and staff while the evening meal was being prepared. Additionally, the inspector viewed a selection of records and toured the building with the manager. The atmosphere at all times was warm and friendly; residents presented as being relaxed and happy; one in particular was looking forward to visiting her family later that evening and another was planning to do the same the following day. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 The home ensures that residents’ needs are met with the skills and experience of the staff, effective communication and access to specialist services. EVIDENCE: There have been no new service users admitted to Ryde Cottage for almost three years and all current residents are long term. They all have cognitive impairments and one has no verbal communication. The home provides longterm care/support and not dedicated intermediate care. Staff skills are developed with a wide range of statutory and care related training, which is ongoing. The home uses a mix of permanent and regular bank staff. Currently 89 of the staff group are qualified at NVQ level 2 or above. The home offers a place only to people whose needs can be met. There was evidence in personal plans that specialist services and clinical guidance are sought as and when required, examples being for a resident who occasionally self harms and for two with sight impairments. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 Residents are enabled to take control of their own lives within the limits of their cognitive ability. Management and staff support those with intellectual impairment and/or limited communication skills to make decisions. EVIDENCE: It was clear during the inspection that the manager and staff assist residents who find decision making difficult. Staff interactions with residents showed a good level of understanding of needs and wishes. Staff confirmed that experience helped them to recognise the signs exhibited by those with limited speech, and this was evident from observations during the inspection. The manager said that all but two residents have family to support them. He was very clear about how to contact the advocacy service where appropriate and described occasions where contact had already been made. The inspector looked at an appropriate risk assessment, which provided guidelines for staff in the prevention of self-harm. No residents are being assessed as being able to manage their own finances. The home provides a system for administration and safekeeping of their

Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 and 13 All but one of the residents at Ryde Cottage lack the capacity to take up employment opportunities. However, support is provided for two who benefit from life-skills learning. Generally they lead active lives and maintain regular links with the community. EVIDENCE: One resident has undertaken voluntary work for some time but mental health needs have meant the work has been sporadic. This issue was discussed with the manager. Personal plans provide information on a range of activities for residents, which are set out in weekly routines. One resident attends the Isle of Wight College; others attend day services, including the Ryde House Resource Centre where a variety of activities are provided for residents and other people with learning disabilities. The home provides opportunities for leisure activities, e.g., clubs, pubs, the cinema, bowling and cafes etc, either using public transport or Ryde House vehicles. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20 Residents are assessed as being unable to retain, administer and control their own medication. Medication for residents is held under secure conditions and appropriate records maintained. EVIDENCE: Medication is dispensed to residents by means of a monitored dosage system by staff deemed competent following training. At the time of the inspection medication was held in appropriate, secure conditions. Records relating to the safekeeping and administration of medicines were checked and found to be in good order. JUDGEMENT – we looked at outcomes for the following standard(s): 22 The home treats residents’ complaints seriously and responds appropriately. EVIDENCE: The manager confirmed there had been no complaints from residents or representatives in the last year. He said that the home maintains good liaison with families to ensure any issues are picked up and addressed. In the event of a complaint being made staff would make a record of the issue together with details of the action taken and outcome. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 30 All areas of the home are generally clean, hygienic and free from unpleasant odours. EVIDENCE: The inspector toured the building with the manager. It was seen generally to be clean and hygienic with no unpleasant odours. The home has a separate laundry room, away from areas where food is prepared, stored, cooked or eaten. The washing machine enables washing to be carried out at appropriate temperatures. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32 and 34 Staff at Ryde Cottage have the necessary skills and experience to meet the needs of the people who live there. Staff turnover is low and a robust recruitment procedure ensures residents are protected. EVIDENCE: All staff are scheduled to receive training and refreshers in first aid, food hygiene, manual handling and infection control. 89 of staff have achieved the NVQ at level 2 or above. Only the newest recruit to the home is currently undergoing induction/foundation training. An evidence book was available to demonstrate progress made. The Ryde House Group ensures staff in all their homes undertake awareness training in service related subjects, e.g., autism and challenging behaviour. Courses are accessed as and when they become available. One support worker had been recruited since the last inspection. All recruitment records were found to be in good order. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37 and 39 The registered manager has the experience and qualifications to run the home and meet its stated purpose, aims and objectives. The Ryde House group has an annual development plan for its homes. Ryde Cottage has a process of establishing residents/representatives’ views about the service. EVIDENCE: The manager of Ryde Cottage Mr Robinson has been in post for nearly four years and has achieved The NVQ at level 4 in care and the Registered Managers Award. He has completed all statutory training and as with the rest of the staff group keeps up to date with service related awareness training, which is accessed as and when it is available. He confirmed that some refresher training in the management of challenging behaviour is planned for 2006. The home has achieved the Investors in People Award. Each resident has a named key-worker who gets to know him or her well. This person is involved

Residents, relatives and care managers are included in the yearly care reviews. The manager said that such occasions were most valuable in terms of seeking service users’ views about the service, as they provided opportunities to bring people together with the residents’ personal plans. He felt that written service user’s surveys had been very hit and miss with responses. A representative of the group monitors the conduct of the home by carrying out monthly regulation 26 visits. This same individual is involved in regular reviews of policies and procedures. Ryde Cottage has recently had a complete review of all policies and procedures.
