This inspection was carried out on 23rd February 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home provides a warm and caring environment to service users who are supported to lead as normal a life as possible, by a committed management and staff team. Record keeping was of a good standard and service users were being involved in developments concerning their home.

What has improved since the last inspection?

The homes staff recruitment procedures had been strengthened and the recording of service user`s finances developed; in order protect the residents from abuse. The upstairs bathroom was now in operation to give the service users a greater choice of facilities and the homes systems for checking the quality of its service had been developed. The Manager had updated the home`s statement of purpose and service user guide.

What the care home could do better:

Some recommendations were made concerning practices to ensure that the service users` health and safety continue to be ensured.

The home is on a quiet country lane and there is not immediate access to public transport. The home has been registered since 1990 and provides personal care and accommodation for 23 older people who may have dementia. The home consists of a traditional two-storey country house, which has been extended providing further bedrooms and a laundry. The home has six double bedrooms and 11 single rooms over the two storeys, all of which are en-suite. The upstairs is accessed by a chair lift. There are 2 communal bathrooms; both have assisted baths available. There is a garden area to the front of the building, which is accessible to service users and includes chairs, tables and benches. This inspection was unannounced and took 4 hours including preparation time. During the inspection, a tour of the premises was undertaken, and the inspector spent time talking with residents and observing their daily lives and checking on the requirements and recommendations from the last inspection. Further time was spent reading care plans and files and talking to the home’s management and staff. The home was clean and furnished to a good standard and had a welcoming atmosphere. Staff were helpful and polite and all of the residents spoken to confirmed that it was very good. One resident described it as a “Home from Home” whilst another stated that she “Couldn’t ask for better”. Relatives of a further resident, who had been coming to the home for day and respite visits, and was due to move in the following day, commented that they “Wouldn’t want him to go anywhere else”. Good progress had been made in implementing the findings from the previous inspection. What the service does well: What has improved since the last inspection? No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 Service users had been provided with information about the home to help them decide whether to move in to the home. EVIDENCE: A combined Statement of Purpose for the home and Service User guide had been developed to assist prospective residents make an informed choice about whether to move into the home, and inspection of these materials confirmed that these were of a satisfactory nature to meet requirements of the Care Home Regulations 2001. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 21 The service users lived in a clean and well-maintained environment. EVIDENCE: The home was warm, clean and comfy which residents described as being “A home from home”. Those residents spoken with said they were able to bring items of their own to personalise their rooms and inspection of the building confirmed this. The upstairs bathroom had been made operational since the last inspection, in order to maximise the residents’ choice of bathing facilities as previously recommended. A new kitchen was in the process of being installed. Discussion with the manager indicated that as part of the home’s ongoing plan of maintenance, the upstairs areas in the annexe were to be upgraded over the coming year. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29 The home’s recruitment policies and procedures were being appropriately followed in order to safeguard the service users. EVIDENCE: The home had recruitment policies and procedures; in order to support and protect the residents and inspection of a random sample of staff files indicated that these were being followed appropriately. Staff files examined now contained all of the required documentation outlined in the Care Home Regulations 2001, together with up to date supervision records. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, 37, 38 The home’s management and administration systems were of a good standard in order to benefit the welfare of the service users. EVIDENCE: Residents indicated that the home was well run and said they “Couldn’t ask for better” and inspection of the records confirmed that they were being well maintained. Since the last inspection, the manager has become registered with the Commission for Social Care Inspection and discussion with her indicated that she had 2 units to complete of her Registered Managers Award and that she was also undertaking her NVQ 4 in care, together with the deputy manager of the home. Residents said that they were consulted about the home and that they regularly had meetings with the management and staff about this. The manager had developed a Quality Assurance for the home since the last Ryehill Country Lodge DS0000019721.V283875.R01.S.doc Version 5.1 Page 15 inspection, and inspection of this confirmed it was of a satisfactory standard. Questionnaires had been issued to residents, relatives and other relevant professionals to obtain their views, and an action plan developed following a quality audit that had been undertaken of the standard of food served. The home had policies and procedures relating to residents’ finances, and a random check of service users individual records confirmed that the homes practices concerning the maintenance of these had been strengthened as previously recommended. Records inspected were up to date, accurate and well maintained. Discussion with the residents and staff indicated that their health and safety were being met. However the temperature of the hot water in the upstairs bathroom was unsafe and posed a risk of scalding to residents. This was immediately rectified and made safe and records inspected indicated that regular audits were being carried out with regard to this. Similarly, a recent visit from the Environmental Health Department had resulted in a requirement being met that some of the kitchen tiles be replaced for ease of cleaning. As previously identified in this report (standard 19), work was underway to upgrade and improve the kitchen.
