This inspection was carried out on 2nd February 2006.

What the care home does well

Salroyd Villa has a homely and informal atmosphere and the staff are kind and caring. Staff in the home understand the residents needs and preferences and daily routines are flexible to take account of these. Residents are encouraged to exercise control over their lives and are helped to keep in contact with family and friends. Residents said they were happy in the home and enjoyed the food. Medicines are managed safely in the home.

What has improved since the last inspection?

All the requirements from the last inspection have been dealt with. The bathroom on the ground floor has been refurbished and now provides a nicer environment for residents when they are bathing. The home has implemented a form to record the risk of developing pressure sores; any residents identified as being at risk are referred to the District Nurse. Staff have been provided with new uniforms and looked very smart.

What the care home could do better:

The owner is considering further improvements to the environment such as the addition of more en-suite facilities. However while some improvements have been made to the environment the pace of the refurbishment programme is slow. If this is not addressed the home is likely to find it more difficult to meet the expectations of prospective residents and their families. Some aspects of the way in which health and safety is managed in the home need to be improved to make sure that residents are living in a safe environment.

Car parking is permitted on the road in front of the property. The home is close to local bus routes. There is disabled access to the home at the front of the building. There are gardens and a patio area to the front of the property that residents can use. The home has been converted from two semi-detached buildings and is registered as a care home providing personal care for sixteen older people. Accommodation is provided on the ground and first floor and there is a stair lift linking both floors. There are two bedrooms and a toilet on the first floor that are not accessed by the stair lift. There are fourteen single bedrooms, one of which has en suite facilities, and one double room. The home has two communal bathrooms and four toilets. There are two communal rooms on the ground floor, one is a large lounge/dining area and the smaller room is used as a dining room and as a quiet area for visitors. Over an inspection year, from April until March, care homes have a minimum of two inspections; these may be announced or unannounced. This was the second inspection of this home and it was unannounced; the first inspection was also unannounced and took place in September 2005. One inspector carried out the inspection and spent approximately four hours in the home. There have been no additional visits to the home since September 2005. The methods used during the inspection included talking to residents, staff and management. I looked at care records and carried out a partial tour of the home. The home prefers the term “resident” to “service user” therefore that is the term that will be used throughout this report. Comment cards were left at the home for residents and relatives; these cards provide an opportunity for people to share their views of the service with the CSCI. Comments received in this way are discussed with the provider without revealing the identity of those completing them. What the service does well: What has improved since the last inspection? The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 Residents are protected by the home’s systems for dealing with medicines. For information on the remaining standards please see the report dated 27 September 2005. EVIDENCE: All the staff that are involved in administering medicines have been trained. None of the residents in the home were managing their own medication. Medicines are stored safely and the records were up to date and accurate. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 Residents are helped to exercise choice and control over their lives. EVIDENCE: Residents can choose whether to spend their time in one of the communal areas or in their bedrooms. Daily routines are fairly flexible and adapted to fit in with residents’ preferences. Residents are offered a range of activities in the home and opportunities to go out either with staff or with family/friends. Residents who are able to are supported in managing their personal allowance money. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 22 Residents live in a clean and comfortable environment. EVIDENCE: All areas of the home were clean and reasonably well maintained. There were no unpleasant smells. Since the last inspection the ground floor bathroom has been refurbished, some new furniture and beds have been purchased and the home is in the process of replacing many of the existing commodes. The first floor bathroom needs to be improved and the floor covering replaced. Call bells are available in all areas used by residents. Residents’ bedrooms are comfortable and in the rooms seen residents’ had their personal belongings around them. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 & 29 Staff are supported in developing their skills and knowledge. Residents are protected by the home’s recruitment procedures. EVIDENCE: There is an ongoing NVQ (National Vocational Qualification) training programme, 33 of staff are qualified and a further three staff are doing NVQ training. The deputy manager said that the home is having difficulty reaching the target of 50 because of staff turnover. Training has been provided during the past 12 months covered subjects such as dementia care, health & safety, infection control, healthy eating and food hygiene and palliative care. The home has not employed any new staff since the last inspection. The deputy manager said that all the required checks are done before new staff start work in the home. The home uses an external organisation to get CRB (Criminal Record Bureau) checks and the deputy manager confirmed that POVCA (Protection of Vulnerable Adults) First checks are done. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 35, 37 & 38 Overall the home is well managed and residents benefit from living in a homely and informal setting. The financial interests of residents are safeguarded despite there being some shortfalls in the record keeping systems. Some improvements are needed to make sure that the health and safety of residents and staff are promoted and protected. EVIDENCE: The deputy manager is the owners’ daughter and she takes responsibility for the day-to-day management of the home. The deputy manager has the relevant experience for this role and has successfully completed the Registered Managers Award. The home now has a policy of not becoming involved in the management of residents’ finances, if necessary residents are helped to find independent support with managing their finances. The home holds small amounts of spending money on behalf of residents; receipts are kept for most transactions but are not provided by the hairdresser and chiropodist who visit the home. The home does not always get two signatures for cash transactions. While some improvement has been achieved there is still too much personal information about residents in the homes’ diary. Information provided by the home showed that most of the required maintenance checks were up to date, however the current gas safety certificate was not available. The deputy manager was not sure if Legionella checks had been done and was not able to confirm if all the fire officers’ recommendations had been implemented. Requirements have been made about these matters.
