Conditions of registration: Date of last inspection 10th June 2005 Brief Description of the Service: Sandrock House is a care home providing accommodation and personal care for 20 service users in a converted building with an extension at the rear. All the bedrooms are single with en-suite toilet facilities except one. There are two floors the upper floor is accessed by a stair lift. Nursing care is not provided. A small independent company owns the home with one of the owners being on site frequently overseeing the business. The home is in Bessacarr, Doncaster and has amenities nearby. This is an overview of what the inspector found during the inspection. This was the homes second inspection in the year 2005/2006 and took place over one day the inspection commenced at 11.20 and finished at 14.15. The previous requirements were looked at with the manager, a selection of the standards were assessed and this year all the key standards have been looked at. 8 Service users 1 visitors and 2 staff were spoken to. A tour of the building took place, observing staff and practices including lunch. What the service does well: What has improved since the last inspection?
The staff training has improved since the last inspection most staff are up to date with mandatory training and this is ongoing to ensure staff are kept up to date with yearly updates. This ensures the staff are appropriately trained to meet the needs of the service users. The home now has a dedicated activity co-ordinator working at least an hour every day and then care staff continue to provide activities when they are able this ensures there is some stimulation for service users every day. Service users likes and dislikes regarding activities have been obtained and recorded to ensure the appropriate activities are provided. The home continues to also have many outings and entertainment in the home in addition to this. Service users commented that the activities and entertainment were very good. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8, 9, 10 & 11. Service users health care needs are met, they are treated with dignity and privacy is upheld. EVIDENCE: Health care needs of service users are met these were documented in plans of care with all health care professionals accessed when appropriate ensuring needs of all service users are met. Service users confirmed they saw the chiropodist, dentist, GP’s and opticians when they were required. Service users wishes regarding illness and death were clearly documented in the plans of care for staff to follow ensuring their needs will be met at the time of their death. Staff spoken to were aware of the need to follow service users wishes and treat them and their families with sensitivity and respect. During the inspection staff were observed with service users they treated them with respect, gave choices and maintained their privacy. Service users spoken to also confirm that staff treated them with respect at all times and that they were always given choices.

Controlled drugs were also audited and these were all in order with robust checking procedures in place to protect service users and staff. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 & 15. Social, religious and recreational needs of service users are met, contact with family and friends is encouraged. The meals are appealing, wholesome and balanced. EVIDENCE: The home now has a dedicated activity co-ordinator working at least an hour every day and then care staff continue to provide activities when they are able this ensures there is some stimulation for service users every day. Service users likes and dislikes regarding activities have been obtained and recorded to ensure the appropriate activities are provided. The home continues to also have outings and entertainment in the home in addition to this. Service users commented that the activities and entertainment were very good. Contact with family and friends are actively encouraged with the home arranging various activities that family and friend are invited to attend. They are to have their annual Christmas party on Saturday for service users and all families are invited. They also maintain contact with the local community with the local church visiting regularly to ensure service users don’t loose touch with the outside community.

Supper is now offered to service users, as this had been a previous requirement. Service users spoken to said the food was good. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 21 The environment is safe and well maintained. The washing facilities are limited as one bath is not accessible to service users. EVIDENCE: The home is well maintained with a programme of routine maintenance. The grounds are kept tidy and safe and accessible to service users. One bathroom has a very small bath that is not accessible to service users this needs to be addressed as this has been a previous requirement and the home does not provide the required number of assisted bathing facilities. It would be ideal to provide a walk in shower in this room where the bath is situated which would make the facility accessible to all service users. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 29 & 30. Service users needs are met by skill mix of staff but would benefit from a structured staff team. The homes recruitment process protects service users. EVIDENCE: The skill mix of staff is very good with a low staff turn over, staff are able to identify and meet the needs of service users. The staff complement needs more senior staff, at present there is the Manager and one senior carer this leaves some shifts in the evenings and weekends without a senior on shift and lack of leadership which could put service users at risk. A selection of staff personal files were looked at by the inspector to determine that robust recruitment procedures are followed, the files were very good well organised and contained all the required information ensuring the service users were protected by the procedures followed. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33, 35, 36 & 38. The best interests of service users are upheld and the health, safety and welfare of service users are promoted and protected. EVIDENCE: The registered provider visits the home and carries out the requirements of regulation 26, and these are sent to CSCI office although these have not been received the last couple of months. The manager carries out regular quality monitoring results of this have been seen for food and activities. There are regular residents and relatives meetings gaining views and feedback to ensure the home is run in the best interests of service users. Finances are managed either by the service users if they are able or their families. The home manages pocket money for some service users. A selection was checked and found to be correct. Written records are kept of all transactions and receipts are kept and the money is stored in a secure facility.

The manager and senior carer are the only two members of staff able to carry out supervision so this is why some staff that they do not work with are getting missed and therefore the manager was unable to ensure that all staff were competent to carry out their jobs. The home has a health and safety policy. Safe working practices are ensured through regular staff training on all health and safety issues. Staff confirmed that training was carried out.
