One specific service user under the age of 65, named on variation dated 9th May 2005, may reside at the home 26th July 2005 Date of last inspection Brief Description of the Service: Sandygate Care Home is a purpose built home offering personal care and accommodation for 54 older people. The home is built on two floors with access via a passenger lift or stairs. There are two units within the home; the upstairs unit offering residential care for older people and the downstairs unit provides care for older people with Dementia type illnesses. The home is located just off the main road of Sandygate in the town of Wath Upon Dearne. There are local shops and facilities close by and other community amenities. All the bedrooms are single and have en-suite facilities; there are assisted baths and showers on each floor for service users whom require them. Outside there is a safe enclosed garden on the lower floor and a sun terrace on the upper floor for service users to use. There is a private visitors lounge on the lower floor. This is an overview of what the inspector found during the inspection. One Inspector from the Commission for Social Care Inspection carried out this unannounced inspection at Sandygate Care Home, on the 20th of December 2005, commencing at 12:50 and finished at 17:25,this was the homes second Inspection since April 2005,any standards not covered in this inspection were covered in the unannounced inspection that was conducted early in the year. It may be the case that some standards will be covered twice in the inspection year 2005/2006, which is considered good practice, and consistent with a professional approach to regulation. During the Inspection we looked at chosen number of documents, sampling of records, tour of the premises and direct and indirect observation of staff interaction with residents, this Inspection also included individual and group discussions with staff, residents, and comments from visitors on the day. Any issues or concerns that were raised were discussed with the Manager during and at the end of the Inspection. What the service does well: What has improved since the last inspection?

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 356 Residents are fully assessed prior to moving into the home, with other professionals involved if needed, this ensures that residents care needs will be met. EVIDENCE: The home offers residential and personal care for older people and the downstairs unit provides care for older people with Dementia type illnesses. The home does not offer intermediate care, but residents can have trail visits either day visit or overnight stay if this is appropriate. Residents that were spoken to during the Inspection, one of them had stay prior to admission to the home. All residents are assessed prior to admission, and it was also evident that when residents are admitted to hospital they are re-assessed to ensure the home still meets their care needs. Evidence that other professionals are involved in assessments and care including regular reviews.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 89 Residents are treated with respect, and their health care needs met. Staff must pay attention to the issues that as been highlighted regarding medication, and ensure that those residents who self medicate are not put at risk. EVIDENCE: Resident’s health care needs were discussed with the manager, the staff on duty on both units and records examined. All residents are registered with a GP and have access to health care facilities, for example hearing and sight test, chiropody is available. Records show that residents had been assessed for nutrition, pressure care, and risk of falls. District nurses are involved in residents care, and records show that a number of other professionals come into the home to see residents; Consultant Psychiatrist visits the EMI unit at the home every three months, and this is very beneficial to both residents and staff, as this not only monitors mental health and medication, but also gives staff the chance to discuss any concerns they have. Sandygate Residential Home DS0000055221.V268993.R01.S.doc Version 5.0 Page 10 Evidence seen that residents had been referred to the dietician when required and dietary advice had been adhered to in care plans. Medicines in the custody of the home must be handled according to the requirements of the Medicines Act; therefore a number of records were checked on both units. Random MAR sheets were examined and found satisfactory, with the exception of; Mar sheets that are hand written must have two signatures, other issues raised residents that self medicate must have up to date risk assessments in place. Stocks of medication must be rotated, this ensure that new stock is not being used before older stock. From observation on the day and speaking to members of staff it was evident that staff treat residents with respect at all times, it was also noted that staff responded to residents call buzzers promptly and gave any personal care in private, this was confirmed when speaking to those residents that could give their opinions. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 13 Staff treat residents with respect at all times, it was also noted that staff responded to residents call buzzers promptly and were aware of promoting the interests of residents needs. Atmosphere within the home appeared good and lots of verbal communication between staff, residents and visiting relatives, EVIDENCE: Routines of the home appear varied on both units on the residential unit it was very evident after speaking to a number of residents that residents are capable of making choices and have the opportunity to exercise their rights in relation to all aspects of life within the home. On the EMI unit not many of the residents were able to give their opinions, therefore observation of staff interaction and interviewing of staff members. All demonstrated good knowledge of care, and could speak of differing approaches of care they may use depending on the resident. All residents were noted to look well cared for and were clean and tidy on the day; with any aids they require e.g. glasses, hearing aids and walking sticks or frames.

The home ensures that residents have links with the local community, and they often visit the local shops and town. The local church provides Holy Communion on a monthly basis, and their own vicar or priest visits some residents. Visitors are welcome to visit at reasonable hours and a pleasant private sitting room is provided for visitors who wish to use it, and lots information was available for visitors including the last Inspection report in the reception area of the home. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 24 26 The home is clean and tidy and communal facilities within the home look well maintained and homely but the EMI unit as some issues that require addressing, although this does not affect residents wellbeing, it would provide a pleasant living environment for people that are to vulnerable to complain themselves. EVIDENCE: The home was purpose built and the location and layout of the home is suitable for its stated purpose, tour of the premises found the reception area is very welcoming and very attractive; the home was clean and tidy and free from odours and looking very festive for Christmas. Routine maintenance takes place and the home is mostly well maintained, but some issue was raised regarding the difference between the units. EMI dining room, the carpet was very stained and the walls were covered in spills from food, no menu was highlighted on the board, on the residential unit the dining

This raised issue regarding the residents on the EMI unit cannot complain about their environment, where as residents on the upper floor would have made comments and this would have been addressed. The Inspector examined a random sample of residents bedrooms on both units during the inspection, these were found to be personalised and very comfortable being furnished to a high standard. One issue was raised regarding toiletries being left in ensuites that could potential put some residents at risk, all resident must be risk assessed t state they have the capacity to be able to identify that toiletries will not be consumed. Laundry facilities were seen, the home have a large laundry room that is well organised, two washing machines that has the specified programming ability to meet requirements, and two dryers. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 28 29 30 The procedures for recruitment are robust which ensures protection for residents living in the home. Appropriate staffing to meet residents care needs, and a wide range of appropriate training is available for staff, with on going development of staff by achieving NVQ qualifications, this ensures that residents are in safe hands at all times. EVIDENCE: Staffing numbers and skill mix was discussed with the manager; she is aware of the Residential Forum Staffing formula and states that resident’s dependency levels are taken into account for calculating staffing requirements. From observation on the day staffing was adequate to meet residents care needs. The home has a training and development plan, and the company is committed to providing staff training to ensure that the needs of residents are met, and a wide range of appropriate training is available for staff, on going development of staff was evident by some staff achieving NVQ qualifications, staff are encouraged to complete NVQ level 2 or 3 training. All new care staff undertakes the TOPSS induction training, this was evident when speaking to new members of staff who confirmed they had received induction training and are being encouraged to do NVQ training when appropriate, all staff have their own training file.

All had thorough recruitment checks completed, and the necessary proof of identity of each member of staff. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 35 36 38 Residents live in a home that is run and managed by a manager that tries to run the home in the best interest of those residents, and ensures so far as is reasonably practicable the health, safety and welfare of residents and staff are promoted. EVIDENCE: The manager is fully aware of her responsibilities within the home, and as the necessary experience to run the home; she continues to work towards her NVQ registered managers award. Health and safety was fully assessed on the last Inspection and fully met, therefore on this inspection observation of staff using equipment and discussion with the manager and staff all of which was found satisfactory, with staff being aware of health and safety policy and procedures. Supervision of staff was discussed with the manager and records examined, which show that most staff receive formal supervision on a regular basis.

One resident control their own finances, but relatives manage most other resident’s saving accounts. Alternative appointees have not been available for three residents and the homes operations manager is acting as appointee for these residents.
