The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Residents told the inspector that the home is "great" and "very nice" and were clearly very happy and comfortable in the friendly and homely environment that the home provides. Residents are actively supported by the manager and staff to be as independent as possible and all of the residents have a busy weekly timetable of work, occupational activities and social and leisure activities. The home is very comfortably furnished and well decorated and all of the residents have their own bedroom and lots of communal space and a garden. The home is well run to provide a safe and comfortable home for the residents with a good team of staff who are experienced, skilled and qualified.

What has improved since the last inspection?

Since the last inspection the home has increased the number of bedrooms to six and these have been decorated and carpeted as chosen by the residents.

The home is jointly owned and managed, with one of the owners managing the home on a daily basis. The owners formerly lived on the premises themselves but have moved out and increased the number of bedrooms from four to six. This was an unannounced inspection during the day, and the report is based on discussion with 4 residents, 2 members of staff, the owner/managers, and observations from a tour of the premises and inspection of some of the home’s records. Inspection comment cards were left for residents and visitors to complete, if they wished to, and two have been received from relatives with very positive comments about the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,&4 Residents care needs are assessed before moving into the home to ensure that the home can fully meet all of their care needs. New residents have the opportunity to visit the home and meet the other residents and staff before deciding to move in. EVIDENCE: Records show that all residents have a thorough assessment of their physical, emotional and social care needs before moving into the home. The assessment process includes all relevant information from the residents, families, care managers and all people involved in the care of the residents. The assessment process continues after moving in, in order to draw up an individual care plan for each resident. New residents are able to visit the home, meet the other residents, stay for meals and take part in some of the activities, before making the decision to move in. The home has a Statement of Purpose and Service User Guide and this is being updated to reflect the increase in the number of residents and the increase in staffing numbers. JUDGEMENT – we looked at outcomes for standard(s) 6,7 & 9 Individual care plans are agreed and reviewed with the residents, and staff respect residents’ right to make decisions as part of this process. Residents are supported to take risks as part of an independent lifestyle and making informed choices about their daily living activities. EVIDENCE: Individual care plans show that all care is reviewed weekly with the residents and their key workers, and care plans are reviewed annually with residents and everyone involved in their care, including family, care managers and other care professionals. A ‘settling in’ period of 6 weeks is agreed initially when a resident moves into the home and the care plan reviewed and updated. The manager is very clear that the role of the care staff is to support residents to make informed choices about all aspects of daily living activities, and for the residents to take responsible risks within the home’s risk assessment and risk management strategies and as agreed within the individual care plans. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11 - 17 Residents are supported to be involved, as much as possible, in valued and fulfilling activities, both within the home and in the local community. EVIDENCE: The managers and the staff are pro-active in supporting the residents to find work opportunities, and social and leisure activities, and are very knowledgeable about local resources and services that are available for the residents. It is clear that individual needs and aspirations are always considered in the day-to-day organisation of the home and residents have lots of support to take part in the activities of their choice. The policy of the home is “open house” and visitors are always welcome. Residents are supported to maintain and develop family and peer group relationships. Residents are involved in the shopping and meal preparation for the home, with the staff, and told the inspector that the food is good and there is always lots of choice. Each resident is supported to take responsibility for the cleaning and tidying of their own bedroom. It was evident that the routines in the home have a flexible and informal structure and meet the needs of the residents. Staff are able to produce information for residents about events and activities using both a pictorial and written format as appropriate. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 - 20 Residents receive personal care and support in the way they prefer. Medication is administered by staff according the home’s policies and procedures. EVIDENCE: Whatever support that residents need with health and personal care is agreed with the key worker and recorded in the care plan and regularly reviewed. Residents are encouraged to develop independent living skills and support and assistance is individually assessed and agreed. The manager and staff have good working relationships with community support services and specialist support is arranged for residents as required. The manager confirmed that medication is dispensed using a blister pack system and there is access to specialist advice and review when required. The manager would seek advice and assessment from an occupational therapist if the need for any specialist aids or equipment were identified for the residents. The home would not be suitable for any residents who were not independently mobile as the bedrooms and bathrooms are located on the first and second floors with access via stairs. JUDGEMENT – we looked at outcomes for standard(s) 23 The health, safety and welfare of the residents is protected at all times. EVIDENCE: All staff follow an initial induction training that includes reading and understanding the policies and procedures for the safeguarding of the residents from the risk of harm or abuse. All of the senior staff have done training in adult protection and all staff have a contract and receive a copy of the Code of Practice for care workers. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24 - 30 Residents live in a homely, comfortable and safe environment with their own bedrooms that suit their needs and lifestyles. The home is very well maintained and decorated and furnished to a very high standard. EVIDENCE: The residents have their own bedrooms, all with en-suite facilities and there is also an additional bathroom and separate toilet. No bedroom is identical and residents are involved in choosing the colour scheme and furnishings. All of the bedrooms reflected the residents’ individual and personal taste and choice. The home has a spacious lounge/dining room, an additional sitting room, kitchen with dining area, and a garden providing a patio, lawn and flower beds and residents have access to all communal areas of the home. Residents clearly felt comfortable and at home with their surroundings and take great pleasure and pride in the way that bedrooms are decorated and personalised. One of the residents showed the inspector the garden where some re-planting has taken place recently and where residents keep two pet rabbits. There is also a cat. The home is very clean and tidy and well maintained and the atmosphere is welcoming and friendly. Staff who sleep in have their own room and there is also an office. Two of the bedrooms have recently been re-furbished and there was an amount of rubbish in the front garden as a consequence. However, the manager explained that this was being taken away within the next day or two. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32.33.34 There are sufficient staff to meet the needs of the residents and there is an ongoing programme of staff training and development. EVIDENCE: The inspector spoke to two members of staff who said how much they enjoy working in the home and with the residents. Staff were very positive about the training and development opportunities that they have and as well as doing NVQ level 2 and 3 in care, staff are also working to achieve training awards with LDAF (Learning Disability Award Framework). Where residents have particular care needs, specialist training has been arranged for staff. Staff feel confident and well supported by the management and supervision structure. With the increase in the number of residents, new staff are in the process of being appointed or are going through induction training. The manager confirmed that all new staff receive the required checks and do not start work until these are confirmed. Staff were confident that the hours provided on the staff rota were sufficient to meet the needs of the residents and to provide 1:1 support for residents where this has been identified as a requirement. JUDGEMENT – we looked at outcomes for standard(s) 37,38,39,40,42 Residents benefit from a well organised and well run home that protects the rights and best interests of the residents. EVIDENCE: The home is jointly owned and managed and one of the owner/managers has day-to-day responsibility for managing Sea Gables. The manager explained that she is in the process of finishing the last units of the NVQ level 4 in care and will then have some additional units to achieve the NVQ level 4 Registered Manager Award and anticipates that this will be completed by January 2006. Although this standard is not quite met, a requirement has not been made as it is clear that this will be met in the very near future. Both owners/managers demonstrated a positive and open management approach and clearly have a good working relationship with the residents and staff in the home and all comments from residents, staff and relatives were very positive. The owners/managers have many years of experience of working in the care sector and considerable management experience. They Sea Gables H55-H04 S12587 Sea Gables V218471 140605 Stage 4.doc Version 1.30 Page 18 demonstrate a high level of knowledge and awareness of good practice in the area of providing care for adults with a learning disability and have a good knowledge of the various resources and services that are available in the local area. The managers subscribe to a number of systems that are designed to support care providers with the organisation and management of the home and these systems regularly update policies and procedures to ensure that providers are meeting their statutory requirements. Records show that weekly checks of the fire alarm and emergency lighting are carried out and the staff and residents have regular fire practice drills and these are recorded. Staff receive regular training in all aspects of safe working practice including food hygiene, infection control and first aid. The manager explained that as a small home, day-to-day issues are discussed as they arise on a one to one basis and there are no formal residents’ meetings or quality assurance systems. All records were up to date and securely stored to maintain confidentiality.
