What the care home could do better:

Assessments of need must be completed for residents living at Shandon House. This will enable the service to identify needs and demonstrate it is able to meet the needs of people living at Shandon House. Furthermore, assessments of need should be updated to ensure they are holistic and clearly identify each resident`s needs. This will help staff to understand the needs of the people they care for. Action should be taken to ensure all outstanding staff personnel files contain a photograph to ensure compliance with the Care Home Regulations 2001.

Whose primary care needs on admission to the home are within the following categories: - Old age not falling within any other category - Code OP The maximum number of service users who can be accommodated is: 20 Date of last inspection Brief description of the care home Shandon House is a care home that is currently registered to provide personal care and support for up to 20 older people. 0 6 0 5 2 0 0 9 Care Homes for Older People Page 2 of 8 Brief description of the care home The home is a large detached property situated close to the town centre of Birkdale village and is within easy reach of Southport town centre and all its amenities. Local services such as shops, a post office, bank and public transport are in close proximity to the home. All the 20 bedrooms are for single occupancy and thirteen have en-suite toilets. The home has a pleasant garden and communal areas including a lounge, dining room and conservatory to the side of the building. The building has three floors which are accessible by a passenger lift and the main and side entrance are equipped with ramps to provide disabled access. A small area is provided for off-road parking. The care home fees range from £379.90 to £410.00 per week. Care Homes for Older People Page 3 of 8 What we found: This random inspection was undertaken on 30th September 2009 and lasted approximately 3.5 hours. The purpose of the random inspection was to review progress and monitor compliance following a key inspection on 6th May 2009, during which some breaches in the Care Home Regulations 2001 were noted. Outcome Area - Choice of Home. The files of two permanent residents which were checked at the last inspection and found not to contain an assessment of need were checked again during the visit. An assessment of need had been completed for only one resident. The acting manager made arrangements to complete one outstanding assessment during the visit. Furthermore, the files of four people who had previously used the service for respite care and which did not have an assessment of need on file at the time of the last key inspection were checked. Only two of the files were available to view and these did not contain an assessment. The acting manager reported that none of the people had returned to Shandon House for respite care and that an assessment would be completed prior to admission, should they wish to use the service again. Examination of the admission register confirmed the people had not returned to Shandon House for respite care since the last visit. Assessment records pertaining to two people who had recently moved into Shandon House for periods of respite care were also viewed. Both files contained a pre-admission assessment of need however assessments viewed were brief and vague and some sections had not been completed. These issues were brought to the attention of the acting manager. Discussion with the acting manager and examination of records revealed that the manager had completed care plans and risk assessments for all of the permanent residents however five residents did not have an assessment of need on file. Outcome Area - Health and Personal Care. The files of two people who had moved into Shandon House for periods of respite care since the last inspection were viewed. Each file was found to contain a care plan that was linked to areas of assessed need and risk assessment documentation for a range of areas including: moving and handling; pressure sore assessment; falls and person centred risks. Evidence was also available to confirm care plans had been kept under monthly review. Advice was given to the manager on how to improve the evaluation and review process. Outcome Area - Staffing. Recruitment records for three staff who had commenced employment since the last inspection were viewed during the visit and found to contain evidence of references, protection of vulnerable adults / criminal record bureau checks and other records Care Homes for Older People Page 4 of 8 required under the Care Home Regulations 2001. Two staff files did not contain a photograph and this should be addressed. The content of one reference was discussed with the acting manager. Advice was given to consider undertaking an additional protection of vulnerable adult check and to review the outcome of a criminal record bureau check once received, to safeguard the welfare of the people using the service. Examination of additional staff recruitment records confirmed action had been taken by the acting manager to address shortfalls noted at the last inspection for staff who remained in employment at Shandon House. Since the last inspection the acting manager had developed a training matrix and a training file had also been established for each member of staff together with a record of training completed and documentary evidence. Furthermore, evidence of the Skills for Care - Common Induction Standards was on file for new staff in the form of progress logs. The acting manager was reminded that new staff should complete induction training within the first twelve weeks of their employment to ensure compliance with national training targets. Outcome Area - Management and Administration. Since the last inspection the acting manager had taken appropriate steps to ensure the Care Quality Commission was informed on significant notificable events in accordance with Regulation 37 of the Care Home Regulations 2001.
