What the care home does well

Sharston House is well managed. Staff deliver consistently high standards of care and are well regarded by residents and relatives. Staffing levels are good, ensuring that any additional care needs are met. The standard of catering is good providing quality, quantity and choice. Sharston House provides a care pathway for the dying which is good practice. More than 50% of care staff have achieved NVQ level2 or above.

What has improved since the last inspection?

Fans used in the kitchen are clean. The owner has indicated that the bedpan washer is to be moved from a clean to a dirty area. Chiropody takes place in the privacy of the residents` bedroom. Hazardous materials are locked up.

What the care home could do better:

The owner should ensure that the draft terms and conditions for residents funded by social services is introduced as quickly as possible. This should include a written copy of the complaints procedure. The process of formal supervision needs to be fully documented and carried out at least six times a year.

This home is registered for a maximum of 48 service users to include: * Up to 48 service users in the category of OP (old age not falling within any other category * One named service user in the category of PD (physical disability under 65 years) The registered provider must, at all times, employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection 11th July 2005 2. Date of last inspection Brief Description of the Service: Sharston House is a Victorian detached two storey, converted property situated in its own grounds one mile from the centre of Knutsford. A large extension has been added to the existing premises to bring the total number of bedrooms to 48. The bedroom accommodation consists of 36 single bedrooms with ensuite toilet facilities and 7 without en suites. There are 3 double bedrooms and one has en suite facilities. These rooms are usually used as single rooms but could also be used as doubles, for instance for married couples. Bedroom accommodation is provided on both floors. There are two passenger lifts and two staircases providing access to all levels. Some ground floor rooms have patio doors providing access to the rear garden. A nurse call system is provided throughout the home. Wheelchair access is possible via the side entrance and to all parts of the home. Shared living space includes three lounges and a dining room. There are sitting areas in the grounds of the home. There is car parking space. This unannounced inspection took place over 6 hours. A partial tour of the building was undertaken. 6 residents, 4 relatives, the registered manager, the owner and 4 staff were spoken with. Care records for three residents were looked at as well as records on information about the home, complaints and fire safety. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 4 and 5 Sharston House provides information to residents on admission to the home. Residents funded by social services need to be given information about the terms and conditions of their occupancy. Staff have the knowledge and skills to deliver a good quality service. EVIDENCE: When residents are admitted to Sharston House they are given an information letter (Service User Guide). This includes details about the aims, objectives, philosophy of care and services and facilities available at Sharston House. Residents who are privately funded are given a contract. This details all information that they need to know such as their rights and obligations and those of the owner, rooms to be occupied and also the overall care and services covered by the fee. The owner has not yet provided a statement of terms and conditions to residents funded by social services informing them of their rights and the overall care and services. However, the owner has drawn up a draft terms and conditions for these residents which he intends to make available in the near future. Staff are well trained and were described by residents and relatives as “wonderful” and “very caring.” Where possible, residents are invited to visit Sharston House before moving in on a permanent basis. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 10 and 11 The standard of care planning at Sharston House is good. Further health care needs are identified and met. Residents’ rights to privacy and dignity are upheld. EVIDENCE: All residents have a care plan. Three care plans were read. These were all of a good standard and had addressed all care needs. Appropriate risk assessments were provided. The plans indicated that regular contact had been made with various health care professionals. Care plans had been reviewed monthly. Staff at Sharston House have introduced a care pathway for the dying which is good practice. The pathway is research based and involves close communication with the family and other health care professionals who may be involved in the resident`s care. It ensures that symptoms are identified promptly and are well managed. Residents and relatives commented that staff treated them with courtesy and always knocked on their door before entering. Comments in care plans also indicated that staff observe the privacy and dignity of residents. All staff are aware of the need for confidentiality. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 Individual and group social activities are provided. Visitors are welcomed at the home. Residents are given choice about various aspects of their daily lives. The standard of catering is good. EVIDENCE: Sharston House offers a variety of social activities on a one to one or group basis. The house coordinator records individual activities for each resident. There were on average three to four entries for each resident. Residents said that they enjoyed the activities and entertainers who visited. They also commented that they did not have to join in if they did not wish to do so. A religious service is held at the home at least monthly to give communion. Visitors said they were made to feel welcome and could visit at any reasonable time. Residents can express choice in their daily lives in many ways. Several residents prefer to stay in their own rooms or to eat there. A number of residents take a daily newspaper. Sharston House Nursing Home DS0000018784.V272446.R01.S.doc Version 5.0 Page 12 The standard of food was described as being “very good” with evidence of the use of fresh rather than frozen produce and home baking. There was enough to eat and provision was made for specialist diets. Carers assisted residents to eat in a manner that preserved the dignity of residents. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 17 Sharston House has a complaints procedure. Residents are able to vote if they wish. EVIDENCE: Sharston House provided a complaints procedure, which was displayed in the reception area. It also kept a record of all complaints together with any actions taken as a result to put matters right. Residents and relatives where aware of whom to speak to if they had any concerns. The complaints procedure was included within the letter sent to residents who are privately funded but was not incorporated within information given to residents funded by social services. The owner said that this matter would be rectified. Quite a few residents have a postal vote. Steps are taken to ensure that all residents who wish to can participate in the electoral process. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 22, 24, and 26 Sharston House provides a pleasant living environment. The premises are well maintained. EVIDENCE: The owner provided details of environmental improvements since the last inspection. These included new carpets in three bedrooms, new beds and pressure relieving mattresses, new cutlery and crockery and a new sluice washing machine. The standard of the décor and furnishings was good. During a tour of the building no major issues were noted in relation to maintenance. No malodours were detected in any part of the home. A number of rooms were viewed and these had been personalised by residents. Residents said that they were satisfied with the standard of décor of their rooms. A range of aids and adaptations at the home were provided to assist residents with varying degrees of disability. The owner said that the bedpan washer was to be moved away from the laundry to a more suitable location. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 and 28 Staffing levels at Sharston House are satisfactory. Staff are well regarded by residents and relatives. Staff are supported to undertake training relevant to their role. EVIDENCE: The registered manager said that staffing levels remained unchanged. There are adequate numbers of nurses and care staff on duty at all times. The care home does not use agency staff. Residents and relatives said that they were very happy with standards of care at the home. A number of staff have worked at the home for many years, which provides continuity of care for residents. Staff said that they are supported to undertake training. More than 50 of care staff have achieved NVQ level 2 or above. A new member of staff indicated that she had been given a full induction into the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 35, 36 and 38 Sharston House has a strong management team and is well managed. There is a safe system for the management of resident finances. Fire safety is maintained. EVIDENCE: The registered manager has been in post since 1989 and is well respected by residents, staff and care professionals. Relatives and residents commented that the manager is very approachable and helpful. Two able and experienced deputy managers support the manager. All three managers are first level Registered Nurses. The registered manager has not undertaken the NVQ level 4 in management qualification. The owner is very involved in the running of the home and visits about twice a week for 5 hours on each day. He supports the managers to undertake further training relevant to their role. Sharston House only keeps small amounts of cash for several residents. In most cases the families of residents manage their finances and bills are invoiced directly to families. Records were looked at and found to be satisfactory. The process of formal supervision of care staff has started. The deputy matron said that the staff member keeps a copy of the content of supervisions. A copy should also be retained by the home in case originals are lost and to promote continuity. Supervisions need to take place at least six times a year. Sharston House employs a full time maintenance man and part time gardener. The home is well maintained. The testing of portable electrical appliances was taking place and also fire alarm testing. Fire extinguishers have been recently serviced. Staff undertook a fire training update in June 2005. Fire drills have been held usually once a month during the last six months.
