Conditions of registration: none Date of last inspection 4 October 2005 Brief Description of the Service: Shoreline is a care home that provides nursing care to older people aged 65 plus. However there is a condition of their registration that allows the home to admit up to 20 service users age 55 and above who have a physical disability.The Responsible Individual is Dr Lal who is a GP in the Redcar area. The home is located on the sea front and is close to central Redcar shopping mall, where there are pubs, shops, and other amenities. Weather permitting the carers take the residents to the shops and along the seafront in wheelchairs if the service user feels it is too much to walk.The home was built in 1993 and is a 2-storey building. There are 44 rooms in the home of which 30 single rooms have en-suite facilities and there is a passenger lift. A lounge and dining room are available on both floors and there are Registered Nurses on duty 24 hours per day. There is a paved area around the home where plants and shrubs are in pots and a veranda on the first floor where service users can sit out if they choose. This is an overview of what the inspector found during the inspection. A very positive inspection with nearly all of the National Minimum Standards inspected being met. The inspection lasted four hours and four residents were interviewed. A tour of the home showed that it was clean, tidy and well decorated throughout the home. The proprietor must consider replacing the corridor carpets as they are badly stained. It is acknowledged the carpets are less than two years old and the directors may consider further investigation into the quality of the carpet they purchased. Four of the residents interviewed were very satisfied with the care they received. They said they felt happy and well cared for. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3,6 Each resident has a pre-admission assessment from the care manager and the home manager. Intermediate care is not provided. EVIDENCE: Residents care records include a detailed pre assessment that demonstrates the homes ability to provide the appropriate level of care for the resident. Intermediate care is not provided at Shoreline, however some residents receive regular respite care. Shoreline provides nursing and personal care for residents from 55 years. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10 Each resident has an individual care plan that includes the health and social care needs. The records show the home meets the residents’ health care needs. Further development of the medication policy is needed. Service users said they were happy with the care provided. EVIDENCE: Care plans are generated from the problems that are identified at the initial assessment. To ensure all health care needs are met various assessments are used to identify risks of nutritional problems, mobility issues, pressure sores and other risks that may affect the person’s well-being. The care records have sections in them for the appropriate health care professional such as GP’s district nurses, optician, dentist and podiatrist to record their findings. The medication policy needs amending to reflect the changing legislation with regard to the disposal of medication. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 15 Each resident is assessed on admission to the home to ensure his or her nutritional intake is appropriate. The menus are well balanced and appealing. EVIDENCE: The home has an excellent policy for nutritional guidance. Meals are catered for those residents who require “special diets” for medical conditions. Menus are displayed in the dining areas and show a choice of appealing meals. The cook makes her own pastries and cakes rather than purchase pre packed ones. Each day the cook will visit the residents to discuss their menu preferences. Mealtimes are at specific times, however, alternative arrangements can be made if a resident is going out of the home. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 Residents are protected from abuse as far as what is reasonably practicable. EVIDENCE: All staff has a CRB check undertaken prior to their employment. These checks are not portable from a previous employer. Two references are requested and gaps in the employment history are explored. During the induction period Adult Protection is discussed and a policy is available for all staff to read. However the policy is not in keeping with the Tees wide interagency policy. The protocol for reporting suspicions of abuse should be made clear to all staff employed in the home. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,24 26 The standard of the environment within this home is fairly good, with exception of the corridor carpets. The home is safe and regular maintenance and servicing of equipment is undertaken. It was noted that the home is clean and tidy throughout. EVIDENCE: The home was bright and cheerful throughout and homely. Resident’s bedrooms had some of their own furniture from home, photographs and ornaments. Bathrooms and toilets were spacious and disabled people could easily access them. The home has suitable equipment to assist with mobility and bathing. It is a clean and pleasant home. There is evidence that the home has a specific budget to allow furnishings to be replaced as needed. The company plans to investigate the matter further and decide what the plan is with the current carpet as it is unsightly. Regular maintenance of heating and electrical systems takes place annually and certificates were seen to evidence this. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,30 There are sufficient staff on duty to reflect the current needs of the residents living in the home. Staff receive training in health and safety and care practise issues. EVIDENCE: The duty rota showed there were 2 registered nurses at all times supported with five carers. The manager is supernumerary to the care rota to monitor and supervise the staff accordingly. The manager ensures that staff are working within the European Union Working Times Directive. The Responsible Individual for the company Dr Lal allows the manager to staff the home, as she deems necessary. This is based on the dependency of the residents in the home. The dependency of the residents who are currently living in the home is medium to high dependency needs. This means two members of staff are required to attend to the residents at most times of the day and night. Regular training takes place to keep the staff abreast of current issues and care practises. The nurses are registered with the Nursing & Midwifery Council and have up to date personal identification numbers and are eligible to practise. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 38 The home has systems in place to ensure health and safety of the staff and residents. EVIDENCE: There were certificates of maintenance in place and up to date that showed the equipment used in the home was safe and in good working order. Policies are also available to advise staff of how to deal with a safety issue at any time. Regular fire drills take place that include staff and residents.
