Conditions of registration: 1. The maximum number of service users to be accommodated is sixteen (16). 2. Service users must be older people aged sixty-five (65) years or over on admission. Date of last inspection 5 January 2005 Brief Description of the Service: Southfields House is a care home registered for sixteen (16) older people. The home is situated in a residential area of Eastbourne, within a short walk to Eastbourne Town Centre, the library and the Railway Station. The home provides a dining room that looks out to a large, well-maintained garden. There is a lounge on the ground floor and a dining/lounge room on the first floor. All communal and private areas are accessible to service users and a stair/chair lift is provided for easy access. There are three bathrooms, two with assited bath seats. This is an overview of what the inspector found during the inspection. This was an unannounced inspection, which was undertaken over four hours during the day. Parts of the environment were inspected, including all communal areas, the kitchen and some bedrooms. Three care plans were examined. Several people were spoken to on the day of the inspection, including three members of staff, seven residents and two sets of relatives. Residents care plans should record any changes in care needs and how staff are to care for their current needs. Staff are to be reminded of the homes policy on entering residents bedrooms. Staff need to check that bedroom doors shut when the hold-open devise is released. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 4, 3 The homes Statement of Purpose and Service User Guide contains extensive information to enable people to make an informed choice about where to live and how their care needs will be met. The admission procedure ensures that there is an adequate assessment carried out prior to people moving into Southfield House. EVIDENCE: Since the last inspection three people have moved into Southfields House and a pre-admission assessment had been carried out to ensure that the home could meet their physical, emotional and social needs. The Inspector talked to one resident and his relatives, who confirmed that he was staying on a trial basis for four weeks. This person had been given a Service User Guide to the home, which contains extensive information about the services and facilities provided at Southfields House, although it was not easy for this person or his relatives to read. Six comment cards confirmed that residents ‘feel well cared for’. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 10 The health needs of residents are well met with evidence of some multi disciplinary working taking place but improved monitoring of the care planning is needed to ensure that the health care needs of residents are continually identified and met. Generally residents privacy and dignity is respected. EVIDENCE: Each resident has a care plan and a monthly reassessment of their care needs, although, it was noted that one resident’s care needs had changed considerably over the past two months, but the changes to his needs and how staff were to care for him had not been appropriately documented. However, this resident said “they are looking after me”. Residents care plans contained evidence that various healthcare professionals are contacted as and when necessary. The need to seek advice and assistance for one resident’s continence needs was discussed with a senior member of staff. Staff were observed to interact and speak to residents in a friendly and respectful manner and five residents confirmed that their ‘privacy is respected’. However, one resident spoken to said that “staff do not wait after knocking, to enter his room”. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 The home encourages and supports residents to maintain links with the community and contact with family and friends, thus ensuring their independence and social well-being is promoted. Good and varied homemade meals are provided and dietary needs, likes and dislikes are catered for. EVIDENCE: Six residents confirmed that Southfields House ‘provides suitable activities’ such as, arm chair exercises, sing along and dancing sessions, indoor bowls and bingo in the lounge. The home also provides a visiting church service once a month and offers day trips out in the home’s easy access vehicle. Staff encourage residents to maintain their hobbies and interests and residents are supported to access the local community. A discussion about the possibility of assisting one resident to visit his sister was had with a senior staff member and his relatives. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 22, 25, 26 The environment provides residents with an attractive and homely place to live. EVIDENCE: Southfields House was seen to be kept clean and tidy, with all communal and private rooms decorated and furnished to a good standard. The home provides a comfortable and homely lounge and dining room on the ground floor and a small dining/lounge on the first floor, enabling residents to spend time in different communal rooms as they wish. There is a large rear garden, which the residents were enjoying on the day of the inspection. There is a stair lift to access the first floor accommodation, which was in working order. The home provides three bathrooms, two with assisted bath seats and a call bell system throughout the home. JUDGEMENT – we looked at outcomes for standard(s) 27, 29 Southfields House is developing a stable staff team to ensure residents’ needs are met. The recruitment of staff offers protection to the residents living at the home. EVIDENCE: The staff rota indicated that there are two care staff and the manager on duty during the day and one carer working a waking night duty. One resident spoken to during the inspection said that “the staff are very kind” and another commented that staff treat her ‘very well’. Two new members of staff are currently being recruited and the necessary recruitment information is being sought prior to their employment. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for standard(s) 38 Improvements to the fire safety arrangements are being carried out to ensure the safety of residents and staff is maintained at all times. EVIDENCE: A fire officer has recently carried out an inspection of the premises and several matters requiring attention were highlighted. Hold open safety devices have been fitted to doors in the communal areas and are gradually being fitted to residents bedroom doors, where they request these to be left open at night time. However, it was noted that one residents door did not shut when the hold-open devise was released.
