Conditions of registration: 1. 2. The maximum number of service users to be accommodated is sixteen (16). Service users must be older people aged sixty-five (65) years or over on admission. 23rd August 2005 Date of last inspection Brief Description of the Service: Southfields House is a care home registered for sixteen (16) older people. The home is situated in a residential area of Eastbourne, within a short walk to Eastbourne Town Centre, the library and the Railway Station. The home provides a dining room that looks out to a large, well-maintained garden. There is a lounge on the ground floor and a dining/lounge room on the first floor. All communal and private areas are accessible to service users and a stair/chair lift is provided for easy access. There are three bathrooms, two with assisted bath seats. This is an overview of what the inspector found during the inspection. This was an unannounced inspection, which was undertaken over three hours during the day. Parts of the environment were inspected, including all communal areas, the kitchen and some bedrooms. Three care plans were examined. Several people were spoken to on the day of the inspection, including three members of staff and six residents. What the service does well: What has improved since the last inspection? All resident’s care plans need to contain information about their social, spiritual and occupational needs and clear guidelines for staff to ensure these needs are met. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 A comprehensive assessment is carried out on each prospective resident prior to them moving into the home, as part of the admission procedure to ensure that each individual’s needs can be met. EVIDENCE: Since the last inspection one person has moved into Southfields House and a pre-admission assessment had been carried out to ensure that the home could meet his physical, emotional and social needs. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10 The health needs of residents are well met and resident’s privacy and dignity is respected. EVIDENCE: Care plans are comprehensive and regularly monitored and indicated that there is a range of multi disciplinary working taking place to assist staff in ensuring that the health care needs of residents are continually identified and met. However, it was noted that in one resident’s care plan there was little information about his social, spiritual and occupational needs or any clear guidelines for the management of his alcohol consumption. Southfield House uses a Monitored Dosage system for the administration of prescribed medicines and a pharmacist carries out quarterly monitoring visits. The home has a policy in place with regard to the procedures for the administration of medication, recording, storage and disposal of medicines. All staff that administer medication receive training in the safe handling and administering of medication from a pharmacist. Where service users have been assessed as capable or not capable of self-administering their own medication the agreed strategy is documented in their care plan.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15 There is a well-balanced and varied menu that offers choice and meets residents’ dietary needs. EVIDENCE: Residents spoken to said that Southfields House is providing a satisfactory standard of homemade and freshly prepared meals. The previous cook has ceased working at Southfields and a replacement is being sort. The menu is varied and flexible and takes account of resident’s likes and dislikes. Residents confirmed that they are offered a choice of main meals and supper dishes. There is a menu book to record the choice made by each resident. Residents are encouraged to eat lunch and supper in the two dining rooms (one on each floor) and residents confirmed that meals could be eaten in their bedroom if requested. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 18 The homes’ policy and procedures for dealing with concerns or complaints ensures that residents and relatives are listened to and that complaints will be acted upon. The home has satisfactory arrangements to ensure staff have knowledge and understanding of Adult Protection issues to prevent residents being placed at possible risk of harm or abuse. EVIDENCE: Southfields House has a clear policy on concerns and complaints. The homes’ complaints procedure is contained within their Statement of Purpose and Service User Guide. The home has a book to record all complaints and the action taken and the outcome of the complaint investigation. The home has a policy and procedures document on adult protection and prevention of abuse. A copy of the Brighton & Hove and East Sussex Multi-agency Policy and Procedures for the Protection of Vulnerable Adults is available to staff. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 30 Southfields House struggles to provide a wholly stable staff team, however resident’s needs are being met by competent staff. EVIDENCE: The Provider/Manager has endeavoured to develop a stable staff team and has achieved this at a senior carer level but as yet has not achieved a full and stable staff team. However, residents spoken with consider they are being ‘looked after well’. Staff spoken confirmed that they are provided with a range of external and inhouse training such as manual handling, fire safety, infection control, food hygiene, medication and adult abuse. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33, 35 Resident’s are wants and needs are protected by the home’s quality assurance systems but their financial interests could better protected. EVIDENCE: Southfields House has several quality monitoring systems in place, such as satisfaction questionnaires, residents meetings, staff supervision and staff meetings and the home was reassessed as having achieved Investors in People status in December 2005. Resident’s relatives or representatives receive itemised bills for services such as hairdressing, chiropody, newspapers and toiletries. The home maintains a written record of all extra amounts payable for these additional services and this is documented in resident’s terms and conditions of residency and a record is maintained of money and valuables kept on their behalf. However, it was noted that one resident was not provided with lockable facilities in his bedroom to keep his money.
