This inspection was carried out on 9th March 2006.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home is welcoming with a warm and pleasant environment. A number of residents spoken to were very complimentary about the staff; "they are kind and respectful" "they look after me well". Family, friends and other visitors are made welcome by the staff.

What has improved since the last inspection?

Care plans for the service users have been reviewed regularly since the last inspection, and have more detail; meaning that the paperwork describes the care and support needs of the individual. Communal toilet areas have been improved by the fitting of appropriate locks to the doors. A robust training programme is underway for all staff, with statutory training being carried out presently, and other training being planned for the near future. Changes to the dining area have brought about improvements for the service users, with better access to the tables, meaning the area is more spacious and comfortable. Recruitment procedures are now robust with new employees having two references and a supervised induction programme. Four staff files were inspected and found to be in order. Regular supervision is taking place for all staff. The hot water regulation identified as a problem at the last inspection has been corrected and has been working efficiently.

What the care home could do better:

The home needs to continue in its efforts with the staff-training programme and ensure that appropriate refresher courses are arranged for all staff. The home needs a registered manager, and should seek to recruit an appropriate person as soon as possible.

Age Range 60 years upwards Date of last inspection 4th November 2005 Brief Description of the Service: Sowerby House is a care home registered to provide nursing care for up to 51 older people aged 60 and over. The premises comprise of a large, Victorian two-storey house with a passenger lift to the first floor. The service is owned by Ashbourne ( Eton) Limited. The home is in the village of Sowerby, close to local shops. Sowerby is located within 10 minutes walking distance of the market town of Thirsk, which benefits from larger shops and facilities. This unannounced inspection took place on the 9th of March 2006.The inspection took 5 hours, including 3 hours preparation time. This was a limited inspection and focused on the requirements and recommendations from the previous inspection, which had taken place on 4th November 2005. An inspection of the premises was made, including some of the service users rooms. The care records of five service users were looked at, and discussions were held with these service users about the care that they receive. The general manager and senior nurse were available during the inspection and feedback was left with the general manager at the end. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 8 and 10 The service users health care needs are reflected in the care plans, and a caring respectful staff group meets those needs. EVIDENCE: Four care plans were inspected, and evidence of improvements could be found; regular reviews had taken place and the care plans were more individualised which will be of benefit to the service users. Staff were observed speaking to service users in a kind, courteous manner. The lunchtime meal was delivered in a calm, unhurried fashion, and the service users spoken with commented that the staff showed a “great deal of respect” to them. The downstairs toilets (2 cubicles) have now been fitted with appropriate locks enabling the service users to maintain their dignity and privacy. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Service users are protected from abuse by appropriate policies and procedures. EVIDENCE: A comprehensive programme of training is in place with all staff currently attending the statutory training, ensuring that they know enough about abuse and how to identify and report it. There are regular supervision sessions taking place for all staff, which means they have access to further support. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 21 Service users have sufficient and suitable lavatories and washing facilities within an attractive, clean environment. EVIDENCE: There are sufficient en suite and communal washing and lavatory areas for service users, and these have been improved by the addition of new locks. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29 and 30 Service users are protected and supported by the homes recruitment policy, and staff are undertaking statutory training. EVIDENCE: Four staff files were inspected to check for the documentation required by regulation; these were found to be satisfactory, ensuring the service users are protected by the recruitment of appropriate staff. Whilst a number of staff have out of date statutory refresher training such as fire and safety/moving and handling, a comprehensive programme is underway and this should be concluded soon; providing a more up to date, competent compliment of staff. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 36 and 38 The health and safety of the staff and service users is promoted, although the registered managers post remains vacant. EVIDENCE: The position of registered manager remains vacant, and it would be in the best interests of the service users to have an appropriate person in post, however the home does have both a competent general manager, who oversees the financial employment and environmental issues, and an experienced clinical nurse manager. Staff are having adequate continuous supervision, which helps to ensure that the policies and procedures of the home are put into practice; and this is of benefit to the service users. The hot water regulation was in order, preventing any risk of scalding to the service user.
