The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Spring Cottage is a real home from home. Service users say they feel part of a family atmosphere and the proprietors work to promote this. Each service users said that they felt understood and cared for as an individual and that this had made a great difference to their settling in and remaining content with their care. The staff and proprietors show they care both personally and professionally for those living at the home. Medication is safely stored and handled, and the home is well furnished. The surroundings are very pleasant with well kept gardens. The home is well staffed.

What has improved since the last inspection?

The home has not placed a great emphasis upon the development of recording systems in the past, but work has been done since the last inspection to improve this and all records seen were satisfactory.

What the care home could do better:

This home is best suited to the individual who is looking for a very caring family atmosphere where care staff are kind and thoughtful. Those who prefer a service more along the lines of a hotel where a professional distance is maintained will not find it here. The home provides just the care it hopes to and provides it well.

The home does not provide nursing care. The general ethos of the home is one of homeliness and all staff endeavour to create an informal and family like environment. If it has the capacity, the registered persons will admit people on a temporary or respite care basis. Invariably, however, the home has a waiting list of pending admissions.The home has a stair lift in lieu of a passenger lift and consequently service users need to be reasonably ambulant on admission. The home has the advantage, however, that the ground rises to the rear of the building and consequently the first floor bedrooms can, if necessary, be accessed on foot without the need for a lift. Spring Cottage consists of the main building and a detached bungalow. It is located on the fringes of the market town of Norton and is reasonably conveniently situated for the public transport network. It is set in large landscaped gardens that have appropriate seating for the service users. The home has its own vegetable and fruit garden. It is adjacent to a racing stable that tends to be of considerable interest to the service users. It has its own car parking area.The accommodation in the main building is on two floors. The communal space consists of two lounges, one of which is designated a smokers’ lounge although there are no smokers resident at present. This unannounced inspection took place on June 28th and took 11 hours to complete which includes time for preparation and for writing the report. Four service users were spoken to and two members of staff. Whilst particular comments by service users are described in more detail in the main body of the report, all were complimentary about the way in which care was offered at Spring Cottage. Only seven of the standards were assessed at this inspection. Others may be assessed at the next inspection. This report has generated no requirements. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 9 Service users are protected by the homes’ medication policy and procedure. EVIDENCE: The home uses a monitored dosage system for the administration of the service users’ medication. The medication packs are secured in a locked filing cabinet located in the office. No controlled drugs are prescribed at present. The staff responsible for the administration of medication had received approved training that included the safe handling of medication. A policy and procedure on the medication process was available. Staff spoken to were confident they knew how to handle medication and a description of the process confirmed that this is appropriate. The medication records were complete and up to date. None of the current service users self medicated. The guidance issued by the Royal Pharmaceutical Society was available. Service users said they were happy for the staff to manage medication and that they felt safe in their care. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Service users are protected by the homes’ complaints and abuse policy and procedure. EVIDENCE: The home has a suitable complaints policy and procedure. Service users said they were encouraged to speak up of there was a problem and all said they found the staff and manager very approachable. The abuse policy must be amended to include that all cases of alleged abuse must be referred to social services as the lead agency. Staff have received abuse awareness training in induction and said they knew how to respond if they suspected any abuse was taking place. Service users said they felt the staff were all kind and that they felt safe in the home. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 26 EVIDENCE: The home is safe and well maintained. It complies with the local environmental health department. The last fire service report required that the rear fire exit be attended to. This has now been done. The home is decorated to a good standard and all service users spoken to said it was like being at home. The gardens provide a very pleasant environment to sit or walk in during the warmer months and feed tables have been set up near windows so service users can watch birds and squirrels feeding from them whatever the weather. All said the setting of the home was particularly pleasant, and enjoyed the home -grown fruit and vegetables produced in the extensive plot and green house nearby. The laundry is suitable with a washing machine complying with water regulations and a dryer. Service users said their clothes were always clean and carefully handled. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for standard(s) 27 and 30 Service users’ welfare is protected by staffing levels and staff training. EVIDENCE: The staffing rota was examined and there are sufficient staff on duty at all times to meet the needs of service users. Mr and Mrs Marucci live on site and Mrs Marucci is on call if required at night and spends much of her time on site during the day. Staff receive induction and foundation training through ‘learn direct’ and staff training is now in line with current requirements. Service users said there were always sufficient staff on duty to attend to them when needed.
