This inspection was carried out on 21st March 2006.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

There is good information available about the home. The admission process is thorough and residents` needs are fully assessed before admission. Residents and relatives were unanimous in their praise of the staff describing them as "friendly", "good", "helpful and caring" and "full of smiles". Residents said that they felt well cared for and were treated with respect by staff. Residents said that they enjoyed the food and praised the cook. There is a good range of activities provided in the home. The home is kept clean and furnished and decorated to a good standard. The home appears to be well managed and staff feel supported and happy in their work.

What has improved since the last inspection?

Records of maintenance throughout the building are now kept e.g. electrical certificate. The manager has brought in a system, which provides evidence of documents checked when recruiting staff.

What the care home could do better:

All residents` care plans should be reviewed and amended, if necessary, at least once a month. Residents should be assured that at the time of their death their wishes will be respected. Formal supervision for care staff should take place for each individual every five to six weeks. Several of the bedrooms windows were stuck and could not be opened by residents. This does not allow natural ventilation into the rooms. The manager said this had happened when the outside of the building was painted and that she would get it sorted.

It was first established in 1985 by the owner and her late husband and has always been managed by the owner. The home is a large, stone-built detached property, which stands in its own grounds, situated in a quiet residential area of Morley, on the outskirts of Leeds. It is fairly close to shops and public transport links into Morley town centre and to Leeds. The original house had a large ground floor extension added in the late 1990s, constructed in stone to blend in with the original building. The home now provides 18 single bedrooms in total (9 with en-suite facilities) and 2 doubles (1 with en-suite). Communal living space is in four large rooms on the ground floor of the original house, providing flexible dining and sitting arrangements. Many of the service users are from the local area, which enables them to maintain contacts within the community. Over an inspection year from April until March, care homes have a minimum of two inspections a year; these may be announced or unannounced. The last inspection was announced and took place on the 14th September 2005. There have been no further visits until this inspection, which is unannounced. The purpose of this inspection was to monitor progress in meeting the requirements made at the last inspection and to look at the standard of care for people living in the home. One inspector between 10.15am – 4.00pm on the 21st March 2006, carried out this inspection. The people who live in the home prefer the term resident therefore this will be used throughout this report. During the inspection, I looked at records, saw staff carrying out their work and spoke with residents and staff. Comment cards/questionnaires are left for residents, visitors and other professionals at each inspection. These provide an opportunity for people to share their views of the home with the Commission for Social Care Inspection (CSCI). Comments received in this way are shared with the manager, without revealing the identity of those completing them. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 3, 4 and 5. The home provides sufficient information to allow prospective residents to make an informed choice about moving in. All residents are assessed before admission to make sure that the home can meet their needs. EVIDENCE: Residents spoken with during the inspection gave insight into their decision making in respect of admission to the home. This included the information they were provided with about the service in the form of service users guide and Statement of Purpose, which provides detailed information about the service and facilities. The manager assesses all residents before they move into the home and offers a trial visit. Residents I spoke with said that they had been happy for their relatives to look round on their behalf. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9,10 and 11. Care plans need to be reviewed regularly and residents’ wishes at time of death must be recorded and respected. Residents’ are treated with respect. EVIDENCE: The four care plans seen provided detailed and personalised information clearly showing how care needs are met. The care plans had been agreed by residents and relatives, however not all have been reviewed regularly. There is no recording of residents’ wishes at time of their death. The manager is aware of the shortfalls and said that she is working with the staff to make improvements. Arrangements are in place for residents to access health care services such as the dentist, chiropodist and optician. Staff were seen using the hoist in accordance with the moving and handling assessment. Residents said that they felt well cared for and said that staff treated them with respect. Residents said that they could get up and go to bed when they wanted and felt that staff respected the choices they made. One resident said, “staff are very good and all seem to get on well together which creates a nice, happy atmosphere”. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13 and 15. The level of recreation was good residents appear to have meaningful and fulfilling daily lives. Contact with relatives and friends is encouraged and supported. Residents enjoy the food. EVIDENCE: Residents were very complimentary of the home’s environment and the manner in which they supported in their daily lives. Residents said that they had control over their daily lives in relation to how they spent their days. Activities are planned on a weekly basis. Residents said, “There’s plenty going on if you want to join in”. One resident said how much she enjoyed the flower arranging session and another was looking forward to the entertainer who was visiting that afternoon. Some residents said they were content entertaining themselves by watching television, reading or listening to the radio. During the inspection residents received visitors. These were all comfortable entering the home and were offered refreshments during their stay. Residents praised the food and made the following comments “the food’s very good”, “lots of choice”. Residents confirmed that it was generally accepted that Springfield House DS0000001507.V283428.R01.S.doc Version 5.1 Page 11 the cook would respond if they did not like a meal. Residents said the manager and cook welcome constructive feedback on the quality of meal provision. On the day of the inspection the meal of the day was roast turkey, roast potatoes, mix vegetable, treacle sponge and custard, rice pudding and jam, juice, tea, or coffee. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 23, 24, 25 and 26. The home provides a clean and pleasant environment. Bedrooms and communal areas are decorated and furnished to a good standard. Some bedrooms windows need to be released so that they can be opened. EVIDENCE: Only some areas of the home were inspected at this visit. Communal areas are furnished and decorated to a good standard providing light and spacious accommodation. Bedrooms seen were decorated and furnished to a high standard. Residents said that they were pleased with their rooms and were glad to have many of their own personal belongings with them. Several of the bedrooms windows were stuck and could not be opened by residents to allow natural ventilation into the room. The manager said this was because they had recently been painted. She will ask the maintenance man to free the windows that are stuck. All areas of the home were clean with no unpleasant odours. Formal staff supervision does not always take place. Staff were confident, courteous and respectful towards residents. EVIDENCE: The comments made by residents were very positive about the staff and they complimented their care and kindness in meeting their personal needs. Staff spoken to and observed during the inspection appeared relaxed and confident. They displayed a commitment to the service and an understanding of the residents in their care, which demonstrated efforts they had made in responding to residents as individuals. Although staff supervision has commenced this is still not regularly done. The manager agreed to address this.
