What the care home does well

Pre-admission assessments are well completed from which the basis of good care planning can be developed therefore providing the residents with good standards of care. The recruitment procedures followed by the home are of a very good standard this facilitates good staff selection with the abilities to meet residents needs. The system of staff supervision is well structured and carried out effectively staff are supported in their work in meeting the care needs of the residents.

Date of last inspection Brief Description of the Service: Springfield Park is a care home which provides residential care for 30 older people. Residents can access all areas of the home through level access or via a passenger lift. All rooms are currently used as singular occupancy. Residents have access to comfortable lounges and dining areas on both floors. Landscaped gardens surround the home and there are sitting areas outside for residents to use. The home is centrally located for shops and local transport links. The fees for the home range from £361 to £420. More information about the home can be obtained from the service users guide, which contains the previous inspection reports. This can be found in the main entrance of the home. The Visit: An unannounced visit was made on 9/5/07 During the visit we: • • • • • Talked with people who use the service, relatives, staff, the manager & visitors. Looked at information about the people who use the service & how well their needs are met, Looked at other records which must be kept, Checked that staff had the knowledge, skills & training to meet the needs of the people they care for, Looked around the building/parts of the building to make sure it was clean, safe & comfortable. We told the manager/provider what we found. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3 & 6. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Comprehensive pre-admission assessments are carried out to ensure peoples needs can be planned and properly met. Detailed information is available to help people make choices about the service before moving into the home. The home does not provide intermediate care. EVIDENCE: Four residents pre-admission assessments were checked these were completed to a good standard. Information in the files was detailed and comprehensive. Relatives spoken to said they had been given good information about the home before deciding about admission. This included the service users guide the philosophy of care and the homes complaints policy. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, & 10. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The standard of care planning generally gives a good level of information to staff to support meeting residents needs. Healthcare needs are well met by using a multi-agency approach. This helps residents to stay healthy. Adequate medication systems are in place to make sure that residents are not put at risk. Residents are encouraged to retain their independence and are well supported to make choices within a risk assessment framework. All of these had a good range of assessments completed to a good standard. Information from assessments formed the basis for good care plans being written with one exception social care assessment information is not always being used in developing good social care plans. Ongoing records there is evidence of these being linked to the care plans. Reviews of care needs are properly structured with relatives in attendance, relatives sign records to state they have been consulted about the residents care needs. Records of visiting professionals were read and are well completed there is evidence of residents healthcare needs being well met in the records. The systems for the receipt, storage, administration and disposal of medicines were checked. Records for these were being adequately maintained. A residents controlled medication stock balance was checked and found to be correct. However some entries in this residents records did not have two staff signatures. Resident records show that food likes, dislikes and what clothes they like to wear are identified and are being met. Gender of staff for personal care tasks is not currently included in the documentation. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 & 15. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Residents have some opportunities to take part in a variety of leisure pursuits and interests which helps them maintain links with the local community and keep and develop social skills. Mealtimes are flexible to suit individual preferences and lifestyles. Residents are mainly given plenty of choice and are sensitively supported to eat meals where they have specific needs. EVIDENCE: The home has an open visiting policy. Relatives spoken to said they could visit at any time and that the staff always made them feel welcome. The homes weekly activity programme records showed a good range of activities were structured. However the activity coordinator has recently resigned and activities for the residents has declined. Records showed the following events had occurred in April, Sing-along with a visiting singer/entertainer, crafts, bingo and a film show. A weekly visitor from St Bartholomew church visits the Springfield Park Residential Care Home DS0000069657.V338130.R01.S.doc Version 5.2 Page 13 home to see some of the residents. One resident goes out to church each week with her daughter. Menus generally show a good range and choice of food being provided for the residents. However cooked breakfast meals are not currently stated each day and sandwich ingredients are also not stated in the menus. The lunchtime meal was observed the meal was ham risotto or stewed steak with turnip, peas and potatoes. Sweet was rhubarb crumble and custard or ice cream or yoghurt. Dining tables were nicely set with tablecloths, placemats, cutlery, condiments and menu cards. Staff served the residents and portion sizes were as agreed with individual residents. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. A satisfactory complaints procedure is in place and clearly displayed to ensure that complaints are dealt with effectively and to the satisfaction of the complainant. Clear protection procedures are in place to protect residents. EVIDENCE: Since the last inspection there has been one formal complaint which the home responded to and concluded to the complainants satisfaction. Relatives spoken to said they were confident any concerns they might have would be properly addressed by the manager. The complaints procedure is displayed in the entrance area. Staff training for the Protection of vulnerable adults was found to be good. Two members of staff spoken to were aware of the procedures to follow. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 24 & 26. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The building is clean and generally reasonably well maintained however some work identified in this report requires to be carried out. EVIDENCE: A tour of the premises was carried out. The building was generally clean and odour free. A number of residents bedrooms were seen these were nicely decorated with lots of evidence of personal possessions and furniture. Lounge areas are generally well furnished and decorated. The downstairs dining room the French doors and windows here are very badly damaged and need to be renewed and replaced, The downstairs lounge exterior windowsills also need renewing. The assisted bathing chair in the downstairs bathroom is not working and needs to be repaired. The kitchen was very clean and tidy Springfield Park Residential Care Home DS0000069657.V338130.R01.S.doc Version 5.2 Page 16 cleaning schedules were checked and were being satisfactorily maintained. The laundry room was also clean and tidy however no COSSH information was displayed. Clinical waste bins outside of the home were found not to be locked. The garden area to the rear of the home is attractive with nice seating areas for the residents. To the side of the home old furniture and other items stored there need to be disposed of and removed. Relatives spoken to said the home was generally clean and looked after. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 & 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Sufficient numbers of staff are in post to meet the diverse needs of residents. Opportunities for training are good. This enables staff to learn new skills to better support residents in all aspects of their lives. Robust recruitment procedures are in place which help to prevent risk of harm to residents. EVIDENCE: Duty rosters were checked these showed that staffing levels were being maintained in line with the residents assessed needs. Staff on duty on the day of the inspection were as stated in the duty roster. Staff training records were checked and good levels of appropriate training is being carried out. Care staff in the home 53 have completed National Vocational Training level 2 or above. Two further staff are soon to commence NVQ level 2 training. Staff supervision records were appropriate and being carried out every two months. Springfield Park Residential Care Home DS0000069657.V338130.R01.S.doc Version 5.2 Page 18 Two recently recruited staff files were read these were well completed with two written references, proof of identity checks, job application forms, job descriptions and CRB checks carried out. Two members of staff were spoken to both had received good levels of training and were aware of procedures to follow for the protection of vulnerable adults. Both staff members were also satisfied with the levels of their supervision. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35 & 38. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The manager has an open style and is clearly present in the home to give support and direction to the staff in meeting the needs and wishes of residents. Quality assurance systems are in place but are still being developed. This will help the service in ensuring it is run in the best interests of the residents. The system for the management of residents monies is good ensuring the residents financial interests are safeguarded. Good systems and practises are now mainly in place that help ensure residents and staff are safe from risk of harm. Springfield Park Residential Care Home DS0000069657.V338130.R01.S.doc Version 5.2 Page 20 EVIDENCE: The homes manager has been in post 18 months and has almost completed the RMA, she is experienced in providing care for the homes client group. Relatives and staff spoken to say that the manager is approachable and responds to concerns appropriately. Relatives and staff meeting minutes had appropriate agendas and showed evidence of actions taken to resolve issues when they arise. A senior manager of the company visits the home at least monthly to check on standards of care her visit reports were read and were satisfactory. Relative questionnaires these have not been sent out for over a year to seek feedback about services provided. The manager of the home does monthly environmental audits and care plan audits these record checks are satisfactory. The system for the management of residents personal monies were checked. The records showed evidence of regular expenditures with two signatures for all transactions and receipts kept. Money balances were correct. The fire log book records appropriate checks and drills are being carried out. However in house fire training for staff is not at the present time being completed at specified intervals. The homes accident book records were satisfactory the manager audits these regularly to identify any possible trends in incidents occurring. The homes maintenance records were checked and all were upto date and satisfactory.
