Staffing will be in accordance with the Notice issued on 9th November 1998 The service should, at all times, employ a suitably qualified manager who is registered with the CSCI 6th July 2005 Date of last inspection Brief Description of the Service: St Andrews House is a large detached property located in the centre of Barnoldswick. It is registered to provide both nursing and personal care for twenty-four people who are physically disabled. Both men and women live in the home. The majority of rooms are single and all bar two include en-suite toilet and hand basin. There are three shared rooms that also have en-suite facilities. All rooms meet the minimum size requirement to allow people who use a wheelchair to be accommodated. There is a passenger lift to the first floor. The home is well provided with communal space. This includes a small gym and hydro pool that is open five days a week and managed by the physiotherapist and physiotherapy aide. There is limited outdoor space with a small garden area to the front of the building and a patio to the rear. However Barnoldswick is well adapted to the use of wheelchairs with many pavements being wheelchair friendly and local shops and facilities being easily accessible. This is an overview of what the inspector found during the inspection. The unannounced inspection was conducted at St Andrews House on the 19th January 2006. The inspection involved looking at records, talking to the registered manager, staff and five residents, a tour of the home and generally looking at what was happening in the home. This inspection looked at things that should have been done since the last visit and a number of areas that affect resident’s lives. There were twenty people living in the home on the day of the visit. The atmosphere in the home was relaxed, homely and informal. Residents were seen enjoying varied activities throughout the home. The home was assessed against the National Minimum Standards Care Homes for Adults (18 – 65). This report should be read with the inspection report of 6th July 2005 for the reader to get a complete overview of the home. What the service does well:
The home always completed detailed assessments before anyone was admitted to the home, this made sure the home could look after people and meet their needs. Residents were able to make decisions and choices about their own lives and were involved in decisions about the day-to-day running of the home. Every effort was made to help residents to take part in suitable activities both inside and outside the home. An activities co-ordinator was responsible for arranging activities, entertainments and outings. On the day of the inspection residents were involved in various activities of their choice. Relationships between staff and residents were good and friendly relationships had formed. There was a relaxed and informal atmosphere in the home. The home was accessible, clean, comfortable and safe. Since the last visit a number of furnishings had been replaced and areas of the home had been decorated. Residents had personalised their rooms and were comfortable in their surroundings. Management and staff had been given suitable training so they had the skills and knowledge to look after the residents in their care. The majority of care staff had obtained a qualification in care. Systems were in place to ensure that the health, safety and welfare of residents, visitors and staff was protected. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2. The home obtained detailed assessment information about prospective residents, prior to admission, to ensure they can meet their needs. EVIDENCE: Detailed assessment information had been obtained prior to admission to ensure the home was able to meet the resident’s needs. Individual service user plans had been generated from the assessment and the plans had been discussed with residents or their relatives. The plans showed that family members, residents and other health professionals had been involved in discussions about care. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7 and 9. Individual plans were developed from assessment information and contained detailed information about personal, social and healthcare needs; this ensured staff were aware of how to meet the residents needs. The home was good at involving residents or their representatives in decision making about all aspects of their lives in the home. EVIDENCE: Two individual plans were looked at. The plans had been developed from assessments and contained detailed information about personal, social and healthcare needs. There was evidence that residents, their family, friends or advocate had been involved in the development and regular review of the plan. One resident was able to discuss his involvement and discussions with his key worker. Any restrictions on choice and freedom had been fully discussed and documented in the plan. All residents had a key worker. One resident confirmed this. Residents were able to take responsible risks and had been involved in discussion about those risks. The use of bed rails was fully documented and had been discussed with residents and their relatives.

This was supported by documentation, discussion and observation. Residents were participated in all aspects of the home including meetings, selection of new staff, individual and group discussion and satisfaction surveys. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 16. The home ensured residents could take part in a range of appropriate and fulfilling leisure activities that ensured social and personal needs were met. The home promoted resident’s independence, choice and decision-making. EVIDENCE: All standards were assessed and met at the last visit. From observation and talking to people it was clear that every effort was made to help residents to take part in suitable activities. One resident said he had taken on extra responsibilities around the home and was due to start work that would involve him in the local community. An activities co-ordinator was responsible for arranging activities, entertainments and outings. Residents were involved in various activities inside and outside the home on the day of the inspection. There was evidence that residents were supported with their choice of activities. Staff were seen knocking on doors and waiting for permission to enter. Staff chatted with residents in a friendly manner.

The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20. The management of medication was generally good but the policies needed minor review and staff needed to follow these procedures to ensure residents safety. EVIDENCE: Medication management policies and procedures were clear and detailed. However minor additions needed to be included and these were discussed with the registered manager. The medication system was well managed and clear records were kept. Staff were reminded that handwritten charts must be witnessed and appropriate signage and link chains were needed where oxygen was in use or stored. Reviews of medication were recorded and regular advice visits by a pharmacist had been carried out. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 and 30. The home provided a clean, comfortable and attractive environment for residents to live in. EVIDENCE: The home was accessible, clean, comfortable and safe. A number of furnishings had been replaced and areas of the home had been decorated as part of the ongoing maintenance and renewal programme. The conservatory ceiling fans had been repaired and quotes were still being obtained to supply window blinds to reduce the heat in the summer months. One resident said the new lift had been ‘much better’. The frames to the rear of the house still needed painting. The home still did not have a sluicing disinfector to reduce the risk of cross infection and was unable to meet this standard. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 34 and 35. The home needed to improve the way it recruited new staff to ensure required checks were in place and residents were protected. The home ensured that management and staff were competent and qualified to meet the needs of the residents in their care. EVIDENCE: Two staff files were looked at. One file contained an undated testimonial that had been supplied as an employment reference. Evidence to support Protection of Vulnerable Adults checks had been requested, received, a record of the identification number, level of disclosure and outcome was not seen on either files. This was discussed with the registered manager. There was good evidence that residents were involved in the selection of new staff. All new staff had received thorough induction training that included a review of policies and procedures. All staff had training and development plans in place; these evidenced ongoing training. The majority staff had achieved an appropriate care qualification that gave them the skills and understanding to care for the residents in the home. The registered manager had achieved an appropriate management qualification and had also undertaken further training. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39 and 42. The home had safe systems in place to make sure the home was a safe place to live and visit. EVIDENCE: A service users survey had been completed in December 2005. The results of this were due to be made available to residents and their representatives. Systems were in place to ensure the health, safety and welfare of residents, visitors and staff was protected. All service and maintenance documentation was up to date.
