What the care home does well

St George`s Lodge offers the residents who live there, good food and good care. All of the people who were spoken to responded very favourably with regard to the care on offer at St George`s Lodge. The Inspector received many comments from residents such as "it is very good here" and "the staff are very good, when you want help, they give it to you". A visiting professional confirmed that they had no problems at all with the care being provided at the home. The building is in good decorative order, and is well maintained, with the relevant records in place to ensure the safety of the people living there. Many of the staff members have worked at the home for many years, and the Inspector was informed that St George`s Lodge does not use agency staff, which means that the residents always know the staff member on duty. Residents told the Inspector that there were lots of activities to do at the home, with outside entertainers coming in weekly.

What has improved since the last inspection?

There was evidence that the outside of the home had been totally repainted recently, and the garden was looking in good shape. One of the residents, who has his own tools, helps out in the garden, thus ensuring the grounds always look their best. All the radiators in rooms used by residents have been covered, thus ensuring the safety of the people who live at St George`s Lodge. The Inspector was told of the purchase of a new chandelier, for the recently refurbished front entrance hall, and of a new fish tank for the lounge.

What the care home could do better:

The Inspector spoke at length to the majority of residents on the day of inspection, and most felt that St George`s Lodge was offering a really good service, and could not do anything better. However it was seen that not all fire training had been recorded appropriately, and that some of the staff members files were incomplete. The person in charge on the day of inspection assured the Inspector that these matters would be addressed.

It is a detached property, located in East Worthing approximately one mile from the town centre and relatively close to the sea and parks. The accommodation of twenty-six single bedrooms, the majority of which are en-suite, is arranged on two floors. There is a passenger lift to the bedrooms on the first floor, and a large garden/parking area to the front and the sides of the building. The service is privately owned by Bushby Care Ltd., and the registered manager is Mrs.Susan Bushby. This was an unannounced inspection, the first of a minimum of two statutory visits that an inspector must make to each care home during the course of a year. A second inspection, which will be announced, will be undertaken later in the year. During this inspection the Inspector toured the building, visiting resident’s bedrooms, and the dining and sitting areas. The inspector spoke to the majority of residents, and a visiting professional, about what the care was like at St Georges Lodge. The Manager was not present on the day of inspection, and the person in charge was the Assistant Deputy Manager, who very ably assisted the Inspector throughout the inspection. In addition to the Assistant Deputy Manager, the inspector spoke to the Deputy Manager and five members of staff, about how they find working at St Georges Lodge. The Inspector examined records about care being provided to residents; as well as records of any accidents, concerns or complaints, to make sure that the residents at St Georges Lodge were being taken care of. Mr. Bushby, one of the owners of St Georges Lodge, was present on the day of inspection and showed the Inspector round the home and the grounds. At this inspection St Georges Lodge was audited against the National Minimum Standards for Older Persons. All the elements in each of the standards assessed were met, and one was exceeded. The Inspector would like to thank everyone who cooperated with her on the day of this inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 3, 5 and 6 Prior to moving into St Georges Lodge people are given sufficient information about what the home offers, to enable them to make a choice as to whether or not they want to live there. In addition before moving into the home, people are assessed, by a member of staff, in their own home or hospital whenever possible, to make sure they are suitable, and invited for a trial period. People know exactly what they can expect from St Georges Lodge and what is expected from them. EVIDENCE: A resident, who had only recently moved into St Georges Lodge, spoken to on the day of inspection, confirmed that they had been given information about the home, prior to admission, and that they had been invited for a visit before they came to stay. They also said that they had been made to feel most welcome and felt that they had settled in really well. Some residents could not remember if they had received information about the home before they came to live there, but told the Inspector how pleased they were with the care provided at the home. Residents told the Inspector that they would go to the Manager if they had any queries, and felt that they would be listened to, and St Georges Lodge H60-H11 S62147 St Georges Lodge V244447 160805 Stage 4.doc Version 1.40 Page 10 that they had no complaints whatsoever about the way they were being looked after. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10 and 11 Where possible, residents are involved with the drawing up of their care plans. All residents are informed of all services available under the National Health Service, and specialist equipment is provided where necessary. Correct medication procedures were seen to be in place, to protect the people living at St Georges Lodge from harm, and from the comments made by many residents and a visiting professional, it appears that the staff respect the privacy and dignity of the people living at the home. EVIDENCE: Where residents are not able to be involved with the drawing up of their care plans, the Inspector was informed that the residents relative or advocate is involved. This was confirmed by many of the residents who the Inspector spoke to on the day of inspection, one of who said, “I couldn’t wish for more”. Records of drug administration are maintained, and the correct procedures, relating to medication, are being adhered to. The person in charge confirmed St Georges Lodge H60-H11 S62147 St Georges Lodge V244447 160805 Stage 4.doc Version 1.40 Page 12 to the Inspector that staff members administering medication had all received the appropriate training. The Inspector looked at care records of some of the residents, including the most recent admissions and found them to be satisfactory. The Inspector was informed that family or friends are offered a spare bedroom, if available, or a comfortable chair in the resident’s room, at the end of the resident’s life. From comments made to the Inspector, it would appear that staff members treat residents who are dying, and their family and friends, with respect, care and sensitivity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14 and 15 People living at St Georges Lodge are able to make the day-to-day decisions about how they want to live their life. Residents are encouraged to maintain contact with all their old friends and family, wherever possible, and one of the residents regularly goes out to visit people. St Georges Lodge offers a choice of menu, with people able to eat either with other residents in the dining room, or in their own room should they prefer. EVIDENCE: St Georges Lodge H60-H11 S62147 St Georges Lodge V244447 160805 Stage 4.doc Version 1.40 Page 14 There were a variety of activities on offer at St Georges Lodge and the residents spoken to tell the Inspector that they felt that there was enough to do, should they wish it. One resident said she “loved listening to the lady with the accordion each week, when she comes in”. The Inspector shared a meal with the residents on the day of inspection, and there was a lively atmosphere throughout the dining room. The Inspector observed staff members assisting residents with their meal, in a sensitive and caring manner. Residents told the Inspector that the food was very good and that there was “always something nice on the menu”. One resident said to the Inspector that she would prefer poached eggs to scramble, and the person in charge said she would look into it immediately. She told the Inspector that if at any time residents do not fancy what is on offer, then they would always be able to have an alternative. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,17 and 18 Regular training sessions for staff, a complaints book, plus policies and procedures regarding abuse, ensure that, as far as is possible, the people who live at St Georges Lodge are protected from bad practice. All staff members have undertaken a satisfactory Criminal Record Bureau check. EVIDENCE: Complaints are recorded in the complaint’s book, although none were recorded at the time of the inspection. Residents told the Inspector that should they have any concerns at all, they would have no hesitation in going to any staff member, and know that they would be listened to. The person in charge told the Inspector that the home operates an “open door policy”, and residents, relatives and visitors are aware that they can always go to the Manager whenever they want to. Residents confirmed this to the Inspector. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,22,23,24 and 26 On the day of this unannounced inspection St Georges Lodge appeared clean and well maintained, and the location of the home is suitable for the residents who live there. St Georges Lodge was seen to provide aids, ramps and assisted bathing facilities. There is a lift at St Georges Lodge for those residents whose bedrooms are upstairs. EVIDENCE: During the course of the inspection all of the rooms were visited to ensure that the environment was safe and comfortable for people who live there, and it was noted that many residents had brought personal possessions into the home, including small items of furniture, ornaments and photographs. On the day of inspection, St Georges Lodge was seen to be clean, and free from offensive odours. The Inspector was assured that policies and procedures were available for staff regarding control of infection, and the safe disposal of clinical waste. St Georges Lodge H60-H11 S62147 St Georges Lodge V244447 160805 Stage 4.doc Version 1.40 Page 17 Residents told the inspection how much they liked their bedrooms, and one said that their room was “lovely and cosy”, whilst another said, “my room here is much better than the one I used to have”. There is a well-maintained garden, which residents were sitting in on the day of inspection. Mr. Bushby informed the Inspector that a resident helps out in the garden on a regular basis, and this was confirmed by the resident who told the Inspector that he really enjoyed being able to contribute. Other residents spoke highly of his efforts. It was seen that the radiators were covered, ensuring that no resident is at risk. Virtually the entire home has been redecorated since Mr. and Mrs. Bushby took over last year, and Mr. Bushby told the Inspector that when each room becomes vacant, it is completely redecorated, re-carpeted and refurbished, if it is felt necessary. Residents told the Inspector that whenever they rang the bell for assistance, a member of staff always came. During the course of the inspection several doors were seen to be wedged open, the home was made aware of the safety implications surrounding this. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29 and 30 St Georges Lodge is adequately staffed with employees who appear competent to care for older people. National Vocational Qualifications are ongoing, and further development training is expected. All training is to be recorded on staff member’s files, and all recruitment records completed correctly. EVIDENCE: Staffing records were examined and it was seen that all necessary procedures had been followed, with regard to recruitment of staff, including ensuring that all staff members have received Criminal Records Bureau enhanced checks, so that they are safe to work with vulnerable people. The Manager is to ensure that all the necessary paperwork is obtained prior to staff members commencing work at the home. Records were seen to be kept in a locked cabinet. Records showed that all staff members had received appropriate training, and the person in charge told the Inspector that these records were being updated. The Inspector was told that many of the staff have worked at the home for some time, and one staff member spoken to on the day of inspection told the Inspector that “ there is a happy atmosphere at the home ” and another staff member said “it feels good here, and I get all the training I need ”. A third staff member commented on how “well organised” everything was at St Georges Lodge. Residents told the Inspector that there always appear to be enough people on duty to assist them when they need it, and that the staff members “were lovely”. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31. 32, 33, 35 and 38 Mrs. Bushby, the registered Manager, is a qualified nurse, and is experienced in working with older people. Records needed for the safe running of a care home are kept up to date. Policies and procedures are available for staff members to refer to, to ensure the safety of the people who live and work at St Georges Lodge. Staff members receive regular supervision, which is recorded. EVIDENCE: All the residents confirmed to the Inspector, that the Manager is extremely approachable and supportive. All the staff members that the Inspector spoke to echoed this. One resident told the Inspector that the Manager was “a very nice woman” and another said they thought it “a very well run establishment.” Records of accidents, injuries and incidents are recorded and reported appropriately.
