Conditions of registration: 1. 2. The maximum number of service users to be accommodated is seventeen (17) Service users will be aged sixty-five (65) years or over on admission Date of last inspection 23rd May 2005 Brief Description of the Service: St Joseph’s is a detached purpose-built property situated in Kemp Town, Brighton, and is close to shops, public transport and other local amenities. The home is registered to provide accommodation for up to 17 older people; it does not provide nursing care. Access to the building and the well-maintained garden is level and the home has a passenger lift making it suitable for residents using wheelchairs. There is a separate lounge and dining room, and a bright conservatory looking out onto the street. The home also has its own chapel on the premises. Bristol Road and adjacent streets have restricted parking facilities; however, the home does have a car park with five spaces on site. The home has recently achieved its Investors in People Award and retained its Clean Food Award. The home is owned by a registered charity ‘The Institute of the Sisters of Mercy’ and is run by the trustees of this Catholic order. Admission to the home is on a non-denominational basis. This is an overview of what the inspector found during the inspection. The inspection, which was unannounced, took place over three and a half hours and was one of two inspections required over the year. A partial tour of the premises took place and records relating to care, staffing and training were inspected. Residents, staff and the manager were spoken to. Residents were very positive about the service with one lady saying she was so happy at the home she could ‘jump for joy’! The inspector would like to thank the residents and staff for their hospitality and co-operation during the inspection. What the service does well: What has improved since the last inspection? A recommendation was made that the manager attends the local authority adult protection training so that she is aware of local guidelines and the particular social work team she needs to contact if she has any concerns. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Prospective residents are assessed appropriately by the manager and by other agencies when relevant. The manager makes a decision based on each individual’s needs to ensure that the home can meet their particular care requirements. EVIDENCE: Since the last inspection, the home has revised its care plans and now ensures that self-funding residents are thoroughly assessed before admission for all their health, personal and social care needs. An initial care plan is drawn up, which gives staff guidelines on how to care for each new resident and help them to settle in. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 and 10 The homes’ policies, procedures and practices ensure that residents’ health, personal and social care needs are clearly identified and met. Residents are treated with respect and courtesy by staff. EVIDENCE: The home has worked hard on reviewing and revising its care planning process and care plans are now properly completed with the relevant information and are much more detailed than at the last inspection. Staff have received training in completing care plans and are now also including information on residents’ social needs, such as their hobbies and likes and dislikes. Residents are also being encouraged to contribute to and sign their care plans. Staff were seen to treat residents with kindness and respect and were described by residents as ‘very polite’. Residents also confirmed that staff knocked on doors before entering their rooms and called them by their preferred name. A separate medical room ensures that residents have complete privacy when receiving medical treatment.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 14 and 15 The home offers a flexible service and makes positive efforts to involve residents in activities to provide variation and interest, and provide choices in their daily lives. Meals are nutritious, varied and pleasantly presented with options always available. EVIDENCE: The home encourages residents to maintain control over their lives by inviting them to bring in personal possessions with them when they come to live in the home, and to keep control of their personal allowances. Relatives help residents with their finances if required, and are also given details of who to contact if professional help is needed. Residents spoken to said they enjoyed the meals in the home and were able to have their meals in their rooms if they liked. They said there were always different choices available if they did not like what was on the menu, and one lady, who was diabetic, said she had special food. The kitchen was spotlessly clean and food was stored safely. The required records were examined and were all up-to-date and accurate. The home has recently retained its Clean Food Award certificate. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 Policies and procedures ensure that residents are protected from abuse and that complaints are properly listened to and investigated. EVIDENCE: The home is in the process of dealing with a complex complaint and records were made available for examination. The home’s complaints procedure had been properly followed and professional advice had been sought where relevant. Records were complete and up-to-date and the Commission had been informed of the complaint. Another complaint made to the home had also been properly recorded and investigated. Adult protection training had been arranged for all staff earlier in the year and the manager had also attended this. The manager was aware of how to respond to an allegation of adult abuse, and the relevant policies to protect residents were in place. A recommendation was made that the manager attends the local authority adult protection training so that she is aware of local guidelines and the particular social work team she needs to contact if she has any concerns. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30 The home employs sufficient numbers of trained and experienced staff to ensure that residents’ needs are properly met. In order to protect residents, the home carries out the necessary checks on staff before they begin working at the home. EVIDENCE: The home has a full complement of care and support staff and the staffing rota shows that there are normally three care staff and a manager on duty in the mornings and two staff and a manager in the afternoons. At night, there is one waking member of staff on duty plus the manager or deputy manager who also sleeps-in on the premises. The home also employs a handyman, secretary, domestic person and a chef. Three staff have completed their NVQ 2 and another four are due to finish by the end of this year. Another senior staff member has completed her NVQ 3 and is now studying for her NVQ 4. A full training programme ensures that staff undertake core training, such as fire safety and food hygiene, and also additional courses, such as care plan and medication training. An external trainer had come to talk to the staff about looking after people with diabetes on the day of the inspection. Following on from a recommendation made at the last inspection, the home now receives information from its main house regarding CRB checks and information is now kept on file at the home.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33 and 35 The home ensures that residents, their families and other professionals are invited to comment on how the home is run. The manager and a senior representative from the organization also regularly monitor and review the service they are providing to ensure that it is run in the best interests of the residents. The home ensures that residents’ finances are properly safeguarded by its policies and procedures. EVIDENCE: The manager has substantial experience in care settings and has been in post as manager of St Joseph’s for over twelve years. She is CSS qualified in management and care, and undertakes regular training including an annual two-day training course arranged by the organization for all its house managers. She is currently undertaking her Registered Manager’s Award

The home has introduced service user satisfaction questionnaires, and there are also separate questionnaires for staff and relatives. Regulation 26 visits are made monthly, and a three-monthly visit by the sister in charge of all the care homes nationwide is also carried out. The organization has recently appointed an area manager who will have more contact with the home on a regular basis. The home has an annual business and financial plan, and policies and procedures are reviewed annually. The manager has her own budget for the home and submits the home’s accounts on a monthly basis; she is also responsible for drawing up a yearly budget for the home. A central budget is available for major works, and the home is audited annually in line with its status as a charity.
