What the care home does well

Residents spoken with confirmed that the staff provide a high quality of care and that they were aware of their care needs. Staff were observed to carry out their duties in a relaxed and professional manner. Residents who were asked confirmed that the food provided is of a good standard. The standard of cleanliness throughout the home was of a high standard.

What has improved since the last inspection?

The garden has recently been improved; residents said that ` since the boarders have been replanted it is lovely to sit in the garden`. Following the last inspection a Quality Audit System has been introduced, Sr. Angela also confirmed that a new quality audit system has been purchased with the intention of improving the existing system. A staff training programme has now been developed.

What the care home could do better:

Although residents spoken with confirmed that activities are available and that generally they felt there were enough, a number did comment that it would be nice to go out on small trips on a regular basis.

The servicee has charitable status and is registered to the Franciscan Missionary Sisters. St Josephs is a purpose built establishment, and is located in the town of Littlehampton. Accommodation is provided on two floors: the first foor can be accessed by a vertical lift. All rooms are for single occupancy. Sr Angela Baughen is the registered manager in charge of the day to day running of the home. The responsible individual on behalf of the service is Sr Catherine McLaughlin. This announced inspection took place over 6.5 hours and was carried out as part of the routine programme of inspections. Preparation for this inspection focused on a review of previous inspection reports, general correspondence, and pre inspection questionnaire material. Eighteen comment cards were received from residents, their comments regarding care provided included ‘Excellent I love it here’, ‘I am happy here and delighted to be settled in a home of love and happiness’. ‘ Excellent care and love’ and ‘I feel very safe and happy at St Joseph’s’. Fourteen comment cards were received from relatives. Their comments included a selection as follows: - ‘whenever my husband and I visit the home we are particularly impressed with the care and humour shown by all the staff towards their patients. We are always made to feel welcome by the staff’, ‘this is a very caring, loving home’, and ‘St Joseph’s continually provides a loving and efficient nursing environment for my relative, it is always spotlessly clean and a pleasure to visit’. One relative did comment that ‘the more active residents may benefit/welcome activities, outings, stimulation’. During the course of the inspection the inspector toured the home, spoke with residents either privately in their own bedrooms or within the communal areas of the home in order to gain a sense of how the home is being run and how they experienced living at the home. Two staff were spoken with in order to gain a sense of the support and training they receive in order to carry out their jobs and to gain insight into how their knowledge of the aims and objectives of the homes philosophy of care. From speaking with residents the inspector gained the impression that the standard of care provided by the management and the care team is of a high standard. The inspector also took the opportunity to observe the interaction between both residents and staff. It was noted that the atmosphere within the home was jovial and relaxed and that the staff carried out their duties in a respectful manner taking into account the dignity and privacy of residents. Following the last inspection carried out on the 12th October 2004 both requirements identified at that inspection have been addressed. One new requirement has been identified at this inspection. The registered provider is required to write to the Commission for Social Care Inspection by the 27th July 2005 to advise them of action taken with regards the requirement identified in this report. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3 Residents or their relatives are enabled to make an informed choice regarding whether the home will meet their or their relatives needs. The pre admission assessment enables the management to determine that the needs of person admitted to the home can be met. EVIDENCE: A copy of the Statement of Purpose and Service User Guide is made available to prospective residents or their relatives prior to or when they visit the home. Residents have a copy of the Service User Guide in their own bedrooms. The pre admission assessments seen on all new admission indicate that health, personal and social of a prospective residents reviewed, information gained at this time is used to devise a care plan for the prospective resident prior to them being admitted to the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 The care needs of residents are documented in individual Care Plans, which enables staff to monitor and maintain continuity of care. Resident’s privacy and dignity were observed to be maintained. EVIDENCE: Four care plans were reviewed. They clearly identified the individual needs of residents. Residents spoken with confirmed that they are aware that information regarding their care needs is being kept and that staff refer to them in order to carry out care tasks. Some residents spoken with told the inspector that they are not always involved in reviews, as they prefer to leave it to the nurses. Care plans contained all relevant information, including risk assessments for manual handling and nutritional assessments. All of the residents spoken with confirmed that they are able to have a bath each day. One resident said ‘I don’t know of any other place I would rather be’. During the course of the inspection the inspector was able to observe staff maintaining resident privacy and dignity by knocking on doors prior to entering their rooms. Residents who were asked confirmed that staff ensure that they are appropriately covered when being taken to and from the bathroom. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,15 Residents are enabled to satisfy their social, religious and recreational interests, which assists them to live a fulfilled life with in the home setting. Residents are offered a well-balanced and varied diet. EVIDENCE: Service users spoken with confirmed that they have the opportunity to take part in the homes programme of activities. Activities include quizzes, tea and chat clubs, tamarisk centre and music and movement. One resident confirmed that some trips out to places of interest and theatres are organised however, due to the fact that the home does not have its own transport, i.e. a vehicle with wheelchair access, it is difficult for Sr. Angela to organise regular trips as when she does, it has to be so far in advance that when the time comes for the outing those residents who said they would go decline the offer. Residents confirmed that they were able to take part in the planned activities or pursue their own interests as they wish. Residents told the inspector that a hairdresser attends the home weekly. Resident’s confirmed that food provided was of a good standard. A four weekly menu is followed, it was confirmed that the menus get updated at regular intervals. Alternatives to the main meal are available. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, Residents confirmed that they are enabled to raise concerns via the homes written complaints procedure. EVIDENCE: There is a clear complaints procedure, which is included in the Statement of Purpose and Service User Guide. The records of complaints were reviewed. Residents who were asked were aware of the compliant procedure and said that they would use it if required. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,26 The location and layout of the home meets the needs of the current service users. EVIDENCE: Whilst touring the home the inspector was able to establish that the physical environment is well maintained. The pre inspection questionnaire records that and from touring the building the inspector observed that 2 new baths, one new shower have been fitted. Residents rooms can be furnished with personal belongings which gives them a homely, individual atmosphere. All radiators have recently been fitted with covers to reduce the risk of burning. All communal areas of the home are accessible. There are ramps in place enabling service users easy access to out door communal facilities. The standard of cleanliness throughout the home was of an excellent standard. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,30 Staff were on duty in sufficient numbers to met the needs of the current resident. The skill mix of staff was observed to be appropriate. EVIDENCE: Information submitted with the pre inspection material and duty rotas seen at the inspection indicate that staff are employed in sufficient numbers to meet the needs of the current residents. Records seen and staff spoken with at this inspection evidence that staff receive induction training and are offered the opportunity to undertake an NVQ qualification. Staff were observed to be relaxed and confident whilst carrying out their duties. The inspector noted that they were also respectful when talking or assisting residents. A training programme was available. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,33,38 The home is being managed in an appropriate manner. This results in the promotion and safeguarding of service users. EVIDENCE: Sr. Angela is an RN who has many years experience in the care sector. She is currently undertaking the Registered Managers Award. Residents and staff spoken with told the inspector that they feel able to approach the manager with any concerns that they may have. Policies and procedures are in place to ensure the smooth running of the home. A quality audit system is in place and the record of the outcomes of the survey was available for inspection. Monthly reports submitted to the Commission for Social Care Inspection that have been carried out by the responsible individual along with the quality assurance audit indicates that residents views are sought and actioned if possible.
