Conditions of registration: 1. Service Users admitted for respite care in the category (DE(E)), OP is limited to a maximum of four (4) service users at any one time. 10th May 2005 Date of last inspection Brief Description of the Service: Revelstoke is registered for a maximum of 22 service users of either sex and provides care for people over the age of 65, some of whom may have dementia. The home is located in the centre of Bridlington, nearby local facilities such as the spa theatre, leisure world, restaurants, pubs and shops. Transport facilities in Bridlington include a train station and bus service. The home is laid out over three floors and has a lift to all areas, accessible to wheelchairs users. There is a patio to the front of the building, and no rear garden. Eighteen rooms are single and two rooms are double. Six are en suite. The home has one communal assisted bath, and there are plans to consider a second bathroom on the first floor. Two toilets are situated on the ground floor near the communal areas. This is an overview of what the inspector found during the inspection. This unannounced inspection took place on 9th February 2006 and took five hours. This includes time for preparation and writing the report. This is the second report of the financial year and should be read in conjunction with the first. The first inspection covered most of the core standards and this report covers the remainder. Four service users and one member of staff were spoken to during the inspection. The manager was available throughout. There was a sociable and relaxed atmosphere throughout the morning, as staff and service users chatted with one another. All comments regarding the care offered were positive and complementary. This home continues to offer a good service to residents and the standard of care in the home is consistently high. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 10 Service users privacy and dignity is respected and upheld. EVIDENCE: Service users said that all personal care was given in private and that health care professionals carry out consultations in the service user’s own room. Staff were observed knocking on doors before entering and generally conversing with residents in a polite and friendly manner. Several service users have a private telephone line. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13 and 14. Service users are enabled to exercise choice and control over the way they live their lives. EVIDENCE: Service users interests are recorded on file and all leisure and social activities are recorded, often with photographs of outings. All service users said they had opportunities for going out either accompanied with a member of staff on a one to one basis or as part of a group on organised trips. Recent outings included a theatre trip at Christmas and a trip out to a local pub. One of the senior carers spends time organising indoor activities such as craft and artwork, and the service users get together to complete crosswords and watch videos. Visitors are welcome at any time. The service users were pleased to report that the proprietor visited the home often and took a great interest in every individual, taking the time to sit and chat with them and to find out if the care they were receiving was as they wished. They also said that the manager and staff were equally committed to making sure the care offered was good. They said they could get up and go to bed whenever they wished and could eat at times to suit them although most said they enjoyed the company of other service users and so would attend mealtimes together. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Service users live in a clean, well- maintained environment. EVIDENCE: At the last inspection it was noted that a carpet in one of the rooms would soon require replacing and that the flooring in another was in need of changing. The carpet in one has been replaced and the flooring in the other has also been changed. Service users commented that there were sometimes some difficulties with receiving the correct clothing back from the laundry and that despite labelling; some items were ending up in other service users rooms. The manager had already dealt with some clothing incidents, and a service user had been reimbursed for an item which had been damaged. Service users said that most of the time there were no problems and the manager said she would put some thought into how best to protect delicate items and to label clothes more clearly. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35 and 38 Service users health and welfare is safeguarded. EVIDENCE: The manager and a senior carer have completed NVQ Level 4 in care. The manager now has the equivalent of both the NVQ in care and management to level 4. The home runs a quality assurance system based on seeking service users and others’ views, and on planning, action and review. The results of service users surveys are fed back at service users meetings. Policies and procedures are reviewed centrally by Pennine care. The home keeps the personal allowances of some service users. This is well documented and on examination of the records, no discrepancies were found. The home has progressed well with health and safety documentation. Service users welfare is safeguarded through thorough staff training, the maintenance of electrical and other systems and risk assessments. Environmental risk assessments have been completed.
