Conditions of registration: 1. No person may be admitted to the home who falls within the category/combined categories DE/MD when 34 persons who fall within those categories are already accommodated within the home. No person under 40 years of age who falls within category DE, or MD may be admitted to the home. Service users who fall within categories DE, DE(E) are to be accommodated on the first floor. Service users who fall within categories MD, MD(E) are to be accommodated on the ground floor. 20th July 2005 2. 3. 4. Date of last inspection Brief Description of the Service: St Mathews is registered as a care home providing nursing and personal care for fifty four persons over the age of forty who have mental health needs or a dementia related illness. The home is situated in a residential area of Northampton and is close to local amenities. The accommodation, which can be found on two floors, is accessed by two shaft lifts. There are four lounge/diners on the ground floor and a further three lounge/diners on the first floor of the building. There are forty two single rooms in the home and six double rooms, all rooms offer ensuite facilities. This is an overview of what the inspector found during the inspection. This inspection took place over four hours and commenced at 10.30 am on the 16th January 2006. The main method of inspection used was ‘case tracking’ which involved selecting three residents and tracking the care they receive through the checking of their records, discussion with the residents, discussion with the staff and observation of care practices. The registered manager was most helpful during the inspection process. What the service does well: What has improved since the last inspection?
A nutritional risk assessment document has been developed as recommended at the last inspection. This document, which also enables the staff to keep a record of the resident’s weight, was found in all three care plans checked. The registered manager explained that the tiling in the newly refurbished bathrooms has been completed since the last inspection. The downstairs corridor has been redecorated and a down stairs bathroom has been adapted to enable a hoist to be used if necessary. All resident’s bedrooms are now ensuite.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,3,6 The admission process is well managed with all prospective residents having their needs assessed before entering the home. EVIDENCE: • • The Registered Manager confirmed that a Statement of Purpose document is available to all prospective and current residents. Three residents files were checked during the inspection. All three files looked at included an assessment of need completed by the registered manager prior to the resident’s admission into the home. Prospective residents and/or their relatives/advocates are invited to look around the home before moving in. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Residents are looked after well in respect of their health and personal care needs. EVIDENCE: • Care plans belonging to three residents were checked during the inspection. All three were up to date, had been reviewed on a regular basis and included the identified care/support needs of the individual residents. An admission sheet is also included in the care plan documentation. This gives a clear picture of the resident’s history and current support needs. The Registered Manager explained that he was currently in the process of further developing the care plan documentation. Relevant assessments were included in the care plans checked; these included a mental health needs assessments, moving and handling assessments, waterlow assessments and nutritional risk assessments. Evidence was seen to confirm that residents have full access to health care services. These include local GP’s and the local mental health team.
DS0000012645.V278202.R01.S.doc Version 5.1 Page 10 • • • St Matthews Nursing Home • The home employs its own Occupational Therapy (OT) team to assist in the Care Programme Approach to mental health needs and review, and OT assessments were evident in the three care plans checked. The procedures for the administration of medication were checked during the inspection. All records checked on this occasion were in order. Observations during the inspection showed that the staff had a good awareness of the residents individual care/support needs and how to ensure that a resident’s privacy and dignity are maintained at all times. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15 A wide range of activities are offered enabling residents to exercise choices on a daily basis. EVIDENCE: • • A wide range of activities are offered on a daily basis both in the home and in the day centre which is adjacent to the home. The OT manager has produced individual activity plans for all the residents living in the home. These show which activities each individual prefers and enjoys and the OT manager explained that these plans would ensure that all residents have access to activities throughout the week. Activity programmes are displayed throughout the home and residents were seen enjoying a game of scrabble and joining in a baking session during the inspection. The day centre, which is adjacent to the home, offers facilities for the residents to play pool, cook, take part in art and craft sessions and meet socially with other residents, family and friends. • • St Matthews Nursing Home DS0000012645.V278202.R01.S.doc Version 5.1 Page 12 • Family and friends are encouraged to visit. One relative spoken with stated “Its like your own home” • • Choices are offered on a daily basis. Choices include when to get up or go to bed, what to wear, where to eat meals and whether to join in activities. A varied menu is offered and a choice of meal is available on a daily basis. The food is served to suit the resident’s individual needs, whether this is in soft form, pureed form, or as a normal meal. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Staff awareness of actions to take should any form of abuse be suspected ensures the protection of the residents in their care. EVIDENCE: • • Training on abuse awareness has been provided to all staff in the home. Staff spoken with during the inspection were aware of what to do should they suspect any act of abuse and the Registered Manager is aware of his responsibilities with regard to adult protection. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,22,25,26 The standard of the accommodation within this home is good providing residents with a safe, clean and comfortable place to live. EVIDENCE: • • • • • The home is well maintained both internally and externally and the decoration and furnishings are presented in a comfortable and homely way. The day centre facilities are of a high standard. The furnishings within this building are domestic in character and in very good condition. The home is currently undergoing building work which will provide a further four ensuite bedrooms. Resident’s rooms seen on this occasion were clean, comfortable and included the resident’s personal belongings. All areas of the home seen on this occasion were clean and fresh.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,30 Staff are employed in sufficient numbers to meet the current needs of the residents. EVIDENCE: • The registered manager explained that there is a minimum of two qualified nurses and ten care workers on, on the morning shift, two qualified nurses and eight care workers on, on the afternoon shift and one qualified nurse and five carers on, on a night shift. There are also three OT staff available between the hours of 9am and 5pm. On the day of the inspection a relaxed atmosphere was evident with staff going about their work in an unhurried and professional manner. An induction programme, which encompasses the standards set out by Training Organisation for Personal Social Services Training (TOPSS) is in place and is used for all new care workers. Training is provided each Saturday by an external tutor. Sessions provided recently have included, Principles of care, management of violence and aggression, Abuse awareness and Care of the mentally ill. All mandatory training, including moving and handling and first aid, is covered by a rolling rota to ensure all staff complete/refresh themselves in the required areas. • • • St Matthews Nursing Home DS0000012645.V278202.R01.S.doc Version 5.1 Page 17 • Training on infection control was being provided on the day of the inspection and the Registered Manager explained that five staff members were due to complete a learn direct course on care the following day. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,35,36 Resident’s benefit from the ethos, leadership and management approach of the home. EVIDENCE: • The Registered Manager has fourteen years experience as a Registered Mental Nurse and has completed his NVQ (National Vocational Qualification) level four and Registered Managers Award. It was evident that he continues to update his clinical practice and is currently working towards a nursing degree. It was evident during the inspection that the residents benefited from the ethos, leadership and management that the registered manager provides. All staff spoken with during the inspection stated that the manager was approachable, supportive and always available. • St Matthews Nursing Home DS0000012645.V278202.R01.S.doc Version 5.1 Page 19 • • • • Positive relationships between staff and residents were evident throughout the inspection. Evidence was seen of staff supervision sessions and staff spoken with confirmed that these sessions take place on a regular basis. The Registered Manager stated that the home does not keep money on resident’s behalf. A comprehensive quality assurance survey carried out by an external consultant was completed in October 2005. A copy of the completed survey was supplied to the Commission for Social Care Inspection. On inspection of this document it was evident that there were many positive features surrounding the provision of services at St Matthews.
