Conditions of registration: No person may be admitted to the home who falls within the category/combined categories DE/MD when 34 persons who fall within those categories are already accommodated within the home. No person under 40 years of age who falls within category DE, or MD may be admitted to the home. Service users who fall within categories DE, DE(E) are to be accommodated on the first floor. Service users who fall within categories MD, MD(E) are to be accommodated on the ground floor. Date of last inspection 21/10/04 Brief Description of the Service: St Matthews provides long stay nursing care for 50 people, over the age of 50 years, who have mental health needs or a dementia related illness.St Matthews provides 24 hour nursing and personal care.St Matthews is located in a residential area close to local amenities. The home provides both single and shared rooms, many of which have en suite facilities. The home has three separate lounges and three dining rooms. Residents have access to a small, furnished garden at the side of the home and has ample off road parking. This is an overview of what the inspector found during the inspection. One hour were spent prior to the inspection reviewing previous requirements and recommendations and comments from relatives. The inspection took place over a period of Two and a Half hours as part of the statutory inspection programme. Two residents were chosen in order that their experience in the home could be monitored. This included looking at their records, talking to them and also to the staff concerning the care received. In addition to this staff records, fire records and accident records were seen. 10 comment cards had been received from relatives. Eight of these cards were complimentary concerning the service provided with such statements as “Staff and level of care are excellent at all times. Cannot praise them enough.” and “ the various activities in the home have encouraged him to develop his abilities to his full potential” being made. One of the others raised concerns about the laundry and missing personal items. The Registered Manager stated that he had held a meeting with this relative a few days before the inspection and that she had stated she was happy with the care being provided. He undertook to investigate her concerns and speak to her again. The other person raised concerns over the choice of food for the younger residents in the home. The Registered Manager stated that choices are offered and one of this younger group spoken to confirmed that the standard of food was very good. What the service does well:
The home employs an Occupational Therapist, and activity and therapeutic programmes have been developed using research-based methods. At the time of the inspection, one group of service users were enjoying a quiz whilst another group were watching television. A new day centre has been opened by the company, adjacent to the home and some of the more able residents from the home spend time in this facility where they play pool, do craft work, cooking and play games. This new facility was being enjoyed, and three residents were involved in a game of pool, while others were doing glass painting and pottery. One resident stated that he used to be a champion pool player prior to his admission and enjoyed winning now. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 and 3 Thorough assessments and Terms and Conditions ensure that residents are confident that their needs may be met in the home. EVIDENCE: Case records seen demonstrated that thorough assessments were undertaken prior to residents being admitted to the home and that other healthcare professionals were involved in this assessment. Copies of Terms and Conditions of residence were in evidence in each file. Residents and their advocates are invited to visit the home prior to deciding to move in. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 and 10 Good systems are in place to ensure that all aspects of resident need are identified and documented for staff guidance, to ensure that these needs may be met in the home. EVIDENCE: Comprehensive assessments were available on the service user files seen, and these were being updated on a monthly basis. Resident weights were recorded but there was no evidence of nutritional assessments having been carried out in order to identify any areas of need. There was evidence that resident’s plans had been discussed with them but they were not always able or willing to sign these owing to their mental health conditions. Records of appointments and visits to the local General Practitioner and hospital were seen, and records of visits by specialist nurses were also available. The home employs its own Occupational Therapy team to assist in the Care Programme approach to mental health needs and their review. Residents spoken to confirmed that staff treated them well and that they enjoyed living in the home. All residents seen appeared well groomed and well care for. Staff were observed to be treating residents with respect and to be maintaining their privacy and dignity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 and 14 Social Activities are well managed, creative and provide daily interest and variation for people living in the home. EVIDENCE: Service users were seen to be sitting in various lounge areas or in their rooms according to individual preference. Records of individual hobbies and interests had been commenced on the service users case tracked and records of participation in activities were available. At the time of the inspection, one group of service users were enjoying a quiz whilst another group were watching television. A new day centre has been opened by the company, adjacent to the home and some of the more able residents from the home spend time in this facility where they play pool, do craft work, cooking and play games. This new facility was being enjoyed, and three residents were involved in a game of pool, while others were doing glass painting and pottery. An Occupational Therapy student explained the activities programme and the methods of recording and scoring residents enjoyment or participation in each activity offered. One resident had been shopping for new clothes the day before the inspection and an assessment and review of his ability to cope and behaviour in this community setting was available. Several residents like to go out for a walk in the morning to fetch the papers and staff are available to accompany them. Visitors are welcome in the home at any time and one gentleman was visiting his wife at the time of the inspection. JUDGEMENT – we looked at outcomes for standard(s) 16 and 17 Residents and relatives are assured that their complaints will be responded to in a professional manner. EVIDENCE: A complaints procedure is available for residents and relatives guidance. There have been no complaints received by the Commission for Social Care Inspection since the last visit. Care plans were in place to ensure residents had input into the political process where able. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 23, 24, 25 and 26 Residents live in homely surroundings, which are well maintained and clean. EVIDENCE: A limited tour of the environment was undertaken. This demonstrated that communal spaces are bright, airy and well decorated and maintained. In addition to the indoor facilities, residents have access to the garden and to an enclosed patio area, as well as the Day Centre mentioned in standard 12. Resident’s rooms seen showed evidence of personalisation with pictures, ornaments and small items of personal furniture on display. Housekeeping staff are employed to ensure that the home is kept clean and hygienic at all times. Some bathrooms have been refurbished since the last inspection and this has made them much more pleasant and homely for residents use. JUDGEMENT – we looked at outcomes for standard(s) 29 The homes recruitment policies protect residents from possible abuse. EVIDENCE: A selection of staff files seen, demonstrated that the information required by Schedule 2 of the Regulations was in place, in order to protect the service users. Work permits were available for overseas staff, and evidence of staff induction was available. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33 and 38 The Registered Manager ensures that the home is run efficiently, in a way that serves the best interests of the residents. EVIDENCE: The Registered Manager is a Registered Mental Nurse who has several years experience in caring for this service user group. He has recently completed his Registered Managers Award. He continues to update his clinical practice and is currently working towards a nursing degree. Records were available of resident meetings, which are held monthly, and a suggestions box is available in the home to enable the residents to take part in the running of the home. The Registered Manager is based in the reception area of the home and therefore freely available to staff, residents and visitors.

The Fire Officer had visited on the 4th July 2005 and was happy with the arrangements in place. Routine maintenance of the home is recorded in a book and signed when work is completed. Staff training has been provided for Moving and Handling and Service user files seen demonstrated that Moving and Handling assessments were in place. Health and Safety and Fire information was on display in the home. Residents had been assessed for moving and handling risks and staff are trained in the correct moving and handling procedures.
