What the care home does well

St Vincents has a very relaxed, friendly atmosphere and it was evident that the Sisters enjoying living there. The home is beautifully decorated and the lounges and dining room have comfortable furniture. The home employs skilled and experienced staff and the Sisters reported that they were very pleased with all aspects of their care. The staff have a good training programme which covers courses in safe working practice areas and studies relevant to the care of the older person. The Sisters spoke highly of the care and support they receive and also stated that staff were respectful of their wishes. The standard of meals in the home was excellent and satisfaction surveys viewed confirmed this. The Sisters reported that the cook always prepared a good variety of foods. Care files include a good record of the Sisters` individual nursing and social needs and all care documentation is reviewed regularly to ensure the information is accurate. The Sisters are encouraged to take part in this process.

What has improved since the last inspection?

The complaint procedure has the address of the Commission and details of the 28 days response to a complaint. Monthly safety checks of the emergency lighting are being undertaken. The manager appointed by St Vincents is now registered with the Commission. Receipts are kept for purchases made on behalf of the Sisters.

What the care home could do better:

Recruitment procedures must include two written references and clearance from the Protection of Vulnerable Adults [POVA] register prior to commencing employment at the home.

The service should employ a suitably qualified and experienced manager who is registered with the CSCI Date of last inspection 25th September 2005 Brief Description of the Service: St Vincents is a care home providing personal care and support for up to 13 retired Sisters from the Community of St Vincent de Paul and Sisters from other religious communities. The home is situated in Southport town centre overlooking the marine lake and with easy access to public transport. The home has 4 floors served by a passenger lift and stannah chairlifts on the back stairs. The Sisters have single accommodation and a number of rooms have an ensuite facility. Bathrooms are adapted to assist those who are less able and the home also has a Jacuzzi room. Recreational areas comprise of 2 lounges and a dining room. All areas are very clean and well maintained. The Registered Providers are The Daughters of Charity of St Vincent De Paul and a new registered manager has been appointed by the Commission, Ms Rose Lowry. The inspection took place over six and half hours. It was an unannounced visit and conducted as part of the regulatory requirement for care homes to be inspected at least twice a year. A visit took place in June 2005 to register a bedroom thus increasing the number of registered beds to 13. There have been no other visits conducted since the last inspection. A tour of the building took place and a selection of care, staff and nursing home records were also viewed. The manager, administration, staff member and 6 of the 13 residents were spoken with and their views obtained of the home. Satisfaction cards were also given to the Sisters and relatives to complete at their leisure. Comments received have been very complimentary regarding the service and staff. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 Assessments are carried out to ensure the home can meet the Sisters’ care needs in full. EVIDENCE: Individual records are kept for each Sister and assessment information is collated prior to arriving at the home. Many of the Sisters are known to the Community and have visited prior to taking up residency. An assessment was seen for a Sister who had recently moved to St Vincents from another home within the Community, the Sister confirmed that she was settling in well and was very happy. Holistic (treating the whole person) assessments are completed once the Sisters have arrived and further care needs are then identified. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9 and 10 The Sisters’ health, personal and social care needs are addressed in care plans and the health care needs are met effectively which ensures a good overall standard of care in the home. Medicine charts viewed evidenced staff signatures following administration. Care was provided to the Sisters in a sensitive and respectful manner thus maintaining their privacy and dignity. EVIDENCE: The Sisters have an individual plan of care and care files viewed were organised and the information easy to read. Care plans seen were detailed giving clear instructions to staff regarding care delivery. The Sisters are involved with the care plan process and the reviews that are conducted regularly. Key areas identified included, health and well being, independence, mobility, hobbies, personal care and family contact. General risk assessments had been completed, for example, any restrictions to mobility and nutritional needs were being monitored. St Vincents 20050922 St Vincents X100015 UN Stage 4 S5292 V230654 F53.doc Version 1.30 Page 10 The care staff are appointed the role as key worker (taking on the overall responsibility for a number of Sisters) and staff interviewed were enthusiastic and knowledgeable regarding this role. The care staff are fully involved with implementing the care and reviewing existing documentation. A Sister reported, “the girls are very good and so helpful”. The Sisters spoke enthusiastically regarding the caring and polite manner of the staff and this was observed through out the time spent at the home. Staff were seen serving lunch and assisting the Sisters in a professional manner. A Sister reported, “the staff are just marvellous.” Emphasis is placed on respecting the Sisters’ faith and religious needs and this plays a very important part of the care provided at the home. This was discussed with a staff member who reinforced this view. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 15 Daily life, activities and routine were based around what the Sisters would like to do and the home had a friendly relaxed atmosphere. Lunchtime was a social occasion where the Sisters got together and enjoyed a nutritious well-balanced meal. EVIDENCE: The Sisters take part in a wide range of social activities either ‘in house’ or locally. A number of Sisters are very independent and maintain close contact with other Sisters from different homes within the Community. Many visit other homes for a holiday or go on retreat. A Sister discussed her interest in computer and the studies she is undertaking a Southport college. The home arranges trips out to local beauty spots and an excursion is planned to see Blackpool lights next month. A Sister confirmed that the social side of the home was excellent. The Sisters are involved with parish activities and a service of worship and prayer is conducted each day. Staff were seen spending time chatting with the Sisters and also assisting with personal care according to individual need. A number of Sisters reported that they enjoyed the level of independence offered by staff and that they could be as active as they wished. The menu states that alternatives are available at each meal. Comments from the Sisters regarding the food included, “excellent”, “good choice”, wonderful cooking” “lavish menu” and “served beautifully by the staff.” The choice of food available was of an excellent standard and satisfaction questionnaires completed by the Sisters endorse this. Home baking is provided and the Sisters have tea and coffee making facilities in the small lounge on the top floor of the home. Lunch was very pleasant and silver service provided by the staff. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 The home has a clear and accessible complaint procedure should a complaint arise. The Sisters were confident that their concerns would be listened to and acted upon. Safeguards were in place to protect the Sisters from abuse. EVIDENCE: The home has a complaint procedure and this was on display. There have been no complaints and any ‘niggles’ the Sisters may have are brought to the manager’s attention through daily contact and weekly meetings. A Sister reported “what would there be to complaint about, the home is excellent”. The concept of abuse is discussed during the induction of all new staff and formal training is also arranged. The home has a policy and procedure on abuse, a whistle blowing document and Sefton’s local guide. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,21 and 26 St Vincents provides very comfortable, clean accommodation and all areas of the home are subject to an ongoing programme of redecoration. EVIDENCE: St Vincents is a large house comprising of 4 floors. All areas are exceptionally clean and a programme of decoration has recently been completed. The home’s colour schemes are very pleasant and the 2 lounges, dining room and bedrooms have comfortable furniture and fittings. There is a computer in the lounge for the sisters to use however many have this equipment in their own rooms. A Sister confirmed that she was very happy with her bedroom and the excellent standard of maintenance in all areas. There is a patio area at the rear and car parking space to the front. Bathrooms viewed were very clean and the temperature of the hot water to the baths was being recorded to ensure it was delivered to a safe temperature. The home has a Jacuzzi room which the Sisters particularly enjoy. The laundry is taken care of by the staff and gloves and aprons are used where appropriate. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29 and 30 Satisfactory numbers of skilled and experienced staff were on duty to meet the needs of the Sisters. A training programme is provided for staff so that they are able to care for the Sisters in a safe manner. The procedures for the recruitment of staff did not always include necessary Protection of Vulnerable Adult checks and evidence of two written references prior to employment on all staff. This is needed to ensure protection to people living in the home. EVIDENCE: At the time of the inspection thirteen Sisters were resident and the manager was on duty with two care staff. There are always three members of staff available during the day and at night one to two care staff and one who ‘sleeps in’. The staffing rota was available and confirmed that these figures are maintained. A cook, housekeeper and the home’s administrator were also present. The manager is in the process of establishing a bank staff for the home as agency staff are not used. Since the last inspection three new staff have been employed and all have received or are going through an induction with the manager or a senior carer. The induction material viewed was of a good standard and is given over a six week period. All staff receive training to ensure they have the necessary skills and experience to care for the Sisters. Courses in safe working practice include St Vincents 20050922 St Vincents X100015 UN Stage 4 S5292 V230654 F53.doc Version 1.30 Page 17 manual handling, first aid, infection control, food hygiene and health and safety. The training programme for the home is very good and staff are encouraged to attend other courses of interest, for example, diabetes and care of the dying. Staff interviewed felt that there was never a problem with training and that it was arranged regularly. Staff files viewed evidenced the dates and certificates of attendance. Two staff files demonstrated the necessary recruitment checks prior to employment however one staff member had commenced her employment prior to receiving clearance from the Protection of Vulnerable Adults [POVA] register, which is a requirement prior to employment in order to protect residents from known abusers. A second reference was also required for this member of staff. This needs to be addressed in all cases of future staff employment. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, and 38 The home has a new manager in post whose management approach and leaderships skills were valued by the Sister and staff. The Sisters are asked for their views of the home to ensure they receive a quality service in comfortable surroundings. Safety certificates for fire equipment were in date. Staff had received fire prevention instruction and therefore aware of fire safety issues. EVIDENCE: Ms Lowry was appointed as the registered manager by the Commission earlier this year and is in the process of undertaking NVQ Level 4 in Management. Ms Lowry has also completed courses in safe working practice areas with the staff. Comments regarding her management of the home included, “very good support and “doing a really good job”. The Sisters hold regular meetings and are also provided with satisfaction questionnaires regarding the home and its facilities. Comments seen were very complimentary regarding all aspects with particular reference to the excellent food and kindness of the staff. Safety checks of the fire alarms are undertaken each week and the home’s equipment subject to an ongoing service contract. Records viewed were in date. Staff receive regular fire prevention training.
