Conditions of registration: 1. The home is registered for a maximum of 27 service users to include: *up to 27 service users in the category of DE(E) (Dementia over 65 years of age) *up to 27 service users in the category of PD(E) (Physical disability over 65 years of age) *up to 27 service users in the category of OP (Old age not falling within any other category). The service should at all times employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection. 9th June 2005 2. Date of last inspection Brief Description of the Service: Staley House is a large detached building, converted and extended to provide residential care for up to 27 older people, some of whom may have dementia or a physical disability. The property was previously a vicarage. The accommodation is provided on two floors. There are a total of 25 single rooms, 12 of which benefit from ensuite facilities, and one shared room. Located on the ground floor are two lounges and the dining room. In addition to aids and adaptations, there is a full passenger lift. Car parking is provided to the front and side of the building. To the rear is a secure patio area accessed from a lounge that overlooks pleasant lawns and woodland. Garden benches are also provided at the front of the house. The home is located in a conservation area close to the centre of Stalybridge. Domain Care Limited was registered with the Commission for Social Care Inspection as the providers of care at Staley House on 24th January 2005. This is an overview of what the inspector found during the inspection. Staley House provides personal care for up to 27 people over 65 years of age. Domain Care Limited owns Staley House in addition to two other homes located in the northwest. At the time of the inspection there were three vacancies. A new manager has taken up post since the previous inspection in June 2005. She is well experienced and has made an impact on the systems and documentation. Two individual care plans were examined as part of the inspection process in addition to a tour of the building and observation of staff practice. Two residents assisted with their comments and the inspector also interviewed the cook. Comment cards were left at the home for residents and visitors to complete and return to the Commission for Social Care Inspection. Three requirements made at the previous inspection had all been complied with. The registered providers continue with the programme of refurbishment and redecoration. The manager was present during the inspection, which was unannounced and carried out over a period of five hours. What the service does well: What has improved since the last inspection?
An experienced manager has been registered as a fit person with the Commission for Social Care Inspection. The carers now have access to training, including health and safety and National Vocational Qualifications. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 & 10 Standards 7, 8, 9 and 10 were inspected during the previous announced inspection (June 2005) when they were judged to meet fully with the standards. EVIDENCE: The manager has changed the care-planning document to incorporate all the relevant details into one system. It is understood this process is to be further changed in order that all three homes owned by the company are operating in a similar manner. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 The residents would benefit from a structured programme of activity. EVIDENCE: The individual care plans briefly document the residents’ social histories. A small group of people regularly attend a local community hall for an afternoon tea dance and there are regular visits from the clergy. A care plan examined during the inspection documented that the resident should be encouraged to carry out meaningful household tasks. It is understood that two members of care staff have been identified to carry out activities. Until this is put into practice, there is little in the way of stimulation (other than the television) particularly for the more dependent and mentally frail residents. JUDGEMENT – we looked at outcomes for the following standard(s): 18 The residents are protected from abuse. EVIDENCE: At the previous inspection there was a requirement for all the carers to receive training in the protection of vulnerable adults. The manager has attended the formal training provided by the local authority and has devised a programme to cascade the training to the other staff. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 24, 25 & 26 Standards 19, 24, 25 and 26 were inspected during the previous announced inspection (June 2005) when they were judged to meet fully with the standards. EVIDENCE: The registered provider continues to carry out a programme of refurbishment and redecoration throughout the home. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 & 30 Standards 27, 28, 29 and 30 were inspected during the previous announced inspection (June 2005) when they were judged to meet fully with the standards. EVIDENCE: The manger has previous experience in the delivery of training to the care sector. This has enabled her to identify and source areas of training for the carers. Fifty percent of carers have now achieved National Vocational Qualifications at level 2, with a further nine waiting funding. Induction training (to meet the requirements of Skills for Care) has been put in place by the manager. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 & 38 The home is well managed and the welfare of service users is protected. EVIDENCE: The manager of the home has several years’ experience of managing care homes. She has the appropriate skills and is enrolled on the National Vocational Qualification at level 4 (Registered Manager’s Award) as required. The manager was appointed to the post in July 2005 and has made improvements to the systems and documentation. She is a good communicator and has set up a programme of staff meetings with detailed minutes available for those who were unable to attend. First aid training as required from the previous inspection has been planned for later in the year.
