This inspection was carried out on 14th February 2006.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Station View continues to provide good quality diverse services. The long-term beds provide service users with a home where they are happy to live. The rehabilitation unit provides people with the support they need to improve/develop their skills enabling them to return home and live independently. The respite beds provide service users and their families with a short break and the opportunity to recharge their batteries. This support allows people to continue living in the community. The interim beds allow time for service users to have their care needs properly assessed after which an informed decision can be made about their future care. Service users spoke highly of the staff team commenting that staff were "very good and nothing is too much trouble for them." They said that staff seemed to have enough time to talk to them and that they didn`t feel that they were ever being rushed. The routine of the home seemed very relaxed and service users looked well cared for.

What has improved since the last inspection?

The home is clean and free from offensive odours. General maintenance inside the home and the garden and patio areas is being done making the whole site safer and better for service users. To promote fire safety, fire alarms are tested weekly.

What the care home could do better:

The priority must be to recruit a manager and complete the registration process as quickly as possible. A service as diverse as Station view needs someone in charge who can lead and develop the staff team in what are relatively new ways of working with service users. The move from a traditional care home setting providing long term/respite care to now having anassessment function which feeds into the process deciding people`s future care package requires strong leadership if service users are to get the best out of the system.

All Service Users for Intermediate Care (up to a maximum of 8) will be accommodated within the Rehabilitation Unit Care services provided for Service Users under category PD should be on a short term basis only and for service users over the age of fifty (50) 16th November 2005 Date of last inspection Brief Description of the Service: Station View is a care home owned and operated by North Yorkshire County Council and registered to provide a service for up to 39 older people aged over 65 years. A condition applies to this registration, which allows a maximum of three people over the age of fifty to use the respite care beds. The home offers a range of different services, which include long stay care, respite care, interim care and intermediate care. The home also operates a day-centre, which offers up to 36 places per day. People from the home can join in with the day centre activities. The home is purpose built and is located in the Starbeck area. There is good range of amenities close by such as shops and public houses. Harrogate and Knaresborough centres are within a short distance. The home is a single storey building with level access. All bedrooms are single and arranged in small units. There are several inner courtyards and garden patio areas. This unannounced inspection was the second to be undertaken between April 2005 and March 2006. It took four hours including preparation time. The majority of time was spent talking to service users and observing life in the home. Some discussions were held with staff and managers. Most areas of the home, including bedrooms and shared areas, were looked at. The focus of this visit was on a number of key standards not assessed at the last inspection together with those subject to requirements or recommendations at that visit. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,and14 Service users receive a service that they are happy with from a helpful staff team. EVIDENCE: Service users were very complimentary about the staff and the service that they receive. The bedrooms, particularly of the people who live in the home permanently, included a lot of their own possessions giving a feel of real ownership and individuality. The inspector was shown by one service user the recent craft work that he was doing and which he sold to raise money for the homes amenity fund. The service user clearly enjoyed the occupation and was proud of his achievements. Two service users were seen enjoying a game of dominoes. One wing of the home is used as a rehabilitation unit. The sole purpose of this unit is to increase service users knowledge and skills to enable them to return home. Remaining service users can use the on site day centre to join in the activities offered there. Positive relationships were observed between staff and service users and staff was heard offering people a choice about their teatime meal. Some situations given were, rising and retiring time, clothes to wear, bathing times, choice of activities in and outside the home and food. Service users said that the staff are kind and helpful. Visiting is encouraged and a number of visitors were seen in the home during this visit. They seemed relaxed talking with staff and their relatives. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Service users live in a clean environment. EVIDENCE: A tour of the premises showed the home to be clean and free from offensive odours. A requirement made at the three previous inspections to upgrade some toilets remains outstanding. This requirement goes back to June 2004. At the December 2004 inspection, the inspector was told that the work had been allocated for completion by January 2005. The manager explained that they are still waiting for money to be allocated for this work. Whilst the state of these toilets does not have an adverse effect on the standard of care delivered, it is the registered provider’s duty to maintain all parts of the premises to an acceptable standard. It is now about time that this work was done. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 Service users would benefit from having a registered manager in post. To ensure consistent good quality care, the home needs a person in charge to lead and develop the service. EVIDENCE: Following the July 2004 inspection, a requirement was made to submit an application to register a manager. This issue has been raised at subsequent inspections and at the last inspection (November 2005) a number of shortfalls were identified directly attributable to the lack of a registered manager responsible for the day-to-day management of the home. Whilst the shortfalls have now been addressed, the home still does not have a registered manager. In the interim, Ms Sandra Mitchell, an Operations Manager, alongside her other duties has had oversight of the service. She has given assurances that a manager will be appointed before the end of March 2006 and an application to register the manager will be received by the Commission by the end of April 2006.
